Retroperitoneal tumors of lipomatous origin are usually liposarcomas. Only a minority of adipose tumors of the retroperitoneum show histological criteria of benignity. We present the case of a 52-year-old woman, diagnosed of a retroperitoneal lipoma with areas of myxoid degeneration. The tumor involved the left kidney and was resected completely.

INTRODUCTION: Liposarcomas are malignancies of adipocytic lineage and represent one of the most common types of soft tissue sarcomas. They encompass multiple histologies, each with unique molecular profiles. Treatment for localized disease includes resection, potentially with perioperative radiation or systemic therapy. We have reconsidered our material on tumors of adipose tissue, which were observed for 10 years, from 1979 to 1988, at Istituto di Anatomia Patologica dell'Università degli Studi - Arcispedale S. Distant metastases of the parotid gland are uncommon. They arise from primary tumors located in the head and neck, mainly melanoma and epidermoid carcinoma. Other histological types of metastasis are very rare. We report an exceptional case of parotid metastasis of myxoid liposarcoma in a 42-year-old man and insist on the worse prognosis of this entity.

Liposarcomas are common soft-issue sarcomas arising predominantly in deep soft tissue and the retroperitoneum with varied mortality and recurrence rates, largely dependent on histologic type. A low-grade dedifferentiated liposarcoma in the retroperitoneum of a 52-year-old woman is described. The excised specimens contained six nodules of lipoma-like well-differentiated liposarcoma and a nodule of dedifferentiated liposarcoma. Twenty patients with primary liposarcoma of the lower extremity were treated over a 25-year period. The adequacy of the initial surgical procedure and histological grade of malignancy both influenced survival rates. In addition local recurrence was noted only in patients who had undergone "inadequate" initial excision. Inguinal lymph nodes were uninvolved by tumor in all cases. A 44-year-old female patient presented to us with fever and increased abdominal girth. A CT scan showed a very large retroperitoneal mass displacing the right kidney. At operation three enormous retroperitoneal tumours displacing the ascending colon and the right kidney and compressing the inferior vena cava were removed. Histopathological examination revealed a myxoid retroperitoneal liposarcoma.

Paraesophageal omental herniation (POH) is uncommon. CT scan and MRI are complementary in diagnosis. We present a posterior mediastinal mass in a 43 year old male with a history of myxoid liposarcoma raising the suspicion of latent secondary tumor. Subsequently, at thoracotomy he was found to have a POH. Differential diagnosis, work-up and surgical approach are discussed.
