PTCD and Peroral Electronic Cholangioscopy with Holmium Laser Lithotripsy for CBD Stones in a Super-Elderly Patient Post-Billroth II Gastrojejunostomy.

The management of impacted common bile duct stones in patients with prior Billroth II gastrectomy poses unique challenges. Altered gastrointestinal anatomy renders duodenoscope passage and biliary cannulation during conventional ERCP exceedingly difficult, resulting in a low success rate. Super‑elderly (over 90 years old) patients often have multiple comorbidities such as cardiovascular diseases and require rapid relief of biliary obstruction and infection control to create a subsequent therapeutic window. Meanwhile, traditional open biliary surgery and general anesthesia carry prohibitive ris

Flexible ureteroscopy and holmium laser lithotripsy for treatment of upper urinary tract calculi in patients with autosomal dominant polycystic kidney disease.

The objective of the study was to investigate the safety and efficacy of flexible ureteroscopy and holmium laser lithotripsy for the management of patients with autosomal dominant polycystic kidney disease (ADPKD) and associated nephrolithiasis. Between 2005 and 2010, flexible ureteroscopic stone treatment was attempted in 13 patients with ADPKD. Two patients had bilateral renal stones. Most of the stones were located in the renal pelvis and/or calices with a stone size 1.7 cm in the largest diameter. The success rate and morbidity and complications were recorded. A total of 45 intrarenal calc

Video. Laparoscopic common bile duct exploration and holmium laser lithotripsy: a novel approach to the management of common bile duct stones.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) and laparoscopic common bile duct exploration (LCBDE) have proved to be safe and effective ways of managing common bile duct (CBD) stones. Clearance of large or impacted CBD stones by routine endoscopic maneuvers can be challenging, often requiring more invasive techniques such as open CBD exploration, which increases morbidity. This report presents a novel approach to managing impacted CBD stones using laparoscopic transcystic common bile duct exploration and holmium laser lithotripsy with favorable outcomes. METHODS: This ret

Treatment of mid- and lower ureteric calculi: extracorporeal shock-wave lithotripsy vs laser ureteroscopy. A comparison of costs, morbidity and effectiveness.

OBJECTIVES: To determine the efficacy and costs of extracorporeal shock-wave lithotripsy (ESWL) compared with ureteroscopy (URS) in the treatment of mid- and lower ureteric calculi. PATIENTS AND METHODS: The records of patients treated primarily by ESWL and URS were analysed retrospectively. Treatment with ESWL included 63 patients (42 men and 21 women, mean age 52 years, range 23-78, 19 mid- and 44 lower ureteric calculi). All patients received 4000 shock waves at a mean energy setting of 18.1 kV. URS was used in 105 patients, with a 7.2 F miniscope or the 7.1 F flexible scope. Stones were f

Outcomes of 6.3 Fr flexible ureterorenoscopy and laser lithotripsy for paediatric kidney stone disease: a multicentre international study from EAU endourology.

OBJECTIVES: Paediatric urolithiasis is increasing in prevalence, and flexible ureterorenoscopy and laser lithotripsy (FURSL) is increasingly used for its management. Miniaturisation of single-use flexible ureteroscopes may facilitate ureteric access in paediatric patients. The 6.3 Fr HugeMed is among the smallest single-use flexible ureteroscope available, but paediatric data remain limited. We evaluated the safety and efficacy of the 6.3 Fr scope in a multicentre paediatric cohort. PATIENTS AND METHODS: A retrospective cohort study was undertaken across six international centres. Consecutive

Feasibility of a single session retrograde endoscopic laser lithotripsy of two large stones located in a bifid urinary tract. Presentation of a rare case.

A 76 year-old male presented with urosepsis and acute renal injury secondary to obstruction by a 13 mm stone located in the common segment of a bifid left ureter. A second 10 mm stone was detected in the mid calyx of the lower moiety of the kidney. Drainage of both moieties with two double-J stents was initially performed. Following recovery from urosepsis a retrograde endoscopic semirigid and flexible laser lithotripsy of the distal and proximal stone respectively was performed resulting in stone clearance. Although retrograde ureterolithotripsy has been presented in the past, to the best of

The use of laser lithotripsy status post cholecystostomy tube placement without interval cholecystectomy for calculous cholecystitis in a patient unfit for general anesthesia.

Acute cholecystitis in patients unfit for general anesthesia often initially requires cholecystectomy tube placement without cholecystectomy. The best way to definitively manage those patients with irreversible medical conditions, leaving them unable to undergo cholecystectomy, has yet to be defined. Laser lithotripsy is currently used in the management of stones of the genitourinary system. Extracorporeal shock wave cholelithotripsy has been extensively evaluated in Munich, Germany and since then, has been abandoned as an alternative treatment of cholelithiasis. This report discusses a novel
