[Role of Anopheles funestus in the transmission of malaria in the south of Madagascar (Ampanihy district)].

The authors reported the results of paludometric and entomological studies carried-out for two years: 1995-1996 in two localities: Ampanihy and Ankilimivory located in the South of Madagascar. These studies followed a suspect malaria epidemic in Ankilimivory in June and July 1994; the population plasmodic index was of 45%. In April 1995, this data was of 35% in Ampanihy and of 15% in Ankilimivory. Entomological studies carried out in April 1996 allowed to find Anopheles funestus in Ankilimivory and Anopheles gambiae l. s. in the two localities. Both the endemicity of malaria and the role of A.…

[Preliminary results in the clinical, epidemiological and parasitological study of Bancrofti filariasis in Madagascar].

In 1958, a large study on the distribution of Bancroft filariasis was set up in Madagascar. In order to update these data, the authors have studied in the same areas the parasitological and clinical prevalence of Bancroft filariasis. Here are the preliminary results of this study, concerning 1862 people, aged 10 years and more. The mean prevalence rate of microfilaria-carriers was of 22.9%, and the average parasitic density was of 3.6 parasits/20 microliters of blood. The prevalence rate of chronic clinical symptoms was of 14.7% for men (elephantiasis, hydroceles, chyluria) and of 2% for women…

[Rift Valley fever in Madagascar in 1991. Sero-epidemiological studies in cattle].

Rift Valley fever (RVF) virus was detected for the first time in Madagascar in 1979, but without any impact on human and animal populations. However, in 1990 and 1991, several outbreaks with massive cattle abortions were described. Since that period, seroepidemiological surveys have been conducted on the East coast and in the highlands (outbreak areas), in the high cattle density regions, and in the national slaughter house in Antananarivo. A high RVF IgM antibody prevalence was detected in the outbreak areas during the epizootics. This IgM prevalence thereafter decreased and was followed by a…

[Thyphoid fever in Tananarive (Madagascar): a review of 20o cases (author's transl)].

A clinical and biological study of 200 cases in Tananarive. The conventional clinical form is still frequent because patients have no immunity and report too late to the hospital. The main complications concern the heart and the digestive tract. Hemoculture is the main clue for the diagnosis. Biochemical and specially enzymatic unconventional data whichs have been recently reported are frequently observed. No resistance to chloramphenicol has yet been detected in Tananarive.

One Health Disparities and Blastocystis infection among smallholder farmers in northeastern Madagascar.

Blastocystis is a globally transmitted gastrointestinal protozoa that commonly infects people living in low- and middle-income countries. Transmission is thought to occur via consumption of contaminated water or food and through contact with infected animals, although the specific factors that influence infection in low-resource, rural settings remain unclear. We applied a One Health Disparities framework that considers the interconnectedness of human, nonhuman animal, and environmental health to investigate disparities in Blastocystis spp. infection in rural northeastern Madagascar. We focuse…

Health Status of Tsimihety Women: Sexually Transmitted Infections and Schistosomiasis, Northern Madagascar.

Background: Madagascar is one of the lowest-income countries in Africa, and it has a poorly developed healthcare system. Malagasy women face limited access to sexual and reproductive health services, which is a serious risk factor facilitating the spread of sexually transmitted infections (STIs). The aim of the present study was to assess the prevalence of STIs (Trichomonas vaginalis, Neisseria gonorrhoeae, Treponema pallidum, and HIV-1/HIV-2) and urogenital schistosomiasis, as well as to evaluate hematological parameters and nutritional status, in a group of women from northern Madagascar. Me…

Multiple Introductions of Yersinia pestis during Urban Pneumonic Plague Epidemic, Madagascar, 2017.

Pneumonic plague (PP) is characterized by high infection rate, person-to-person transmission, and rapid progression to severe disease. In 2017, a PP epidemic occurred in 2 Madagascar urban areas, Antananarivo and Toamasina. We used epidemiologic data and Yersinia pestis genomic characterization to determine the sources of this epidemic. Human plague emerged independently from environmental reservoirs in rural endemic foci >20 times during August-November 2017. Confirmed cases from 5 emergences, including 4 PP cases, were documented in urban areas. Epidemiologic and genetic analyses of cases as…

SARS-CoV-2 infection rate in Antananarivo frontline health care workers, Madagascar.

BACKGROUND: Health care workers (HCWs) represent a vulnerable population during epidemic periods. Our cohort study aimed to estimate the risk of infection and associated factors among HCWs during the first wave of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in Madagascar. METHODS: A prospective cohort study was carried out in three hospitals that oversaw the first cases of COVID-19. Monthly ELISA-based serological tests were conducted, and nasopharyngeal swabs were collected in the case of symptoms linked to COVID-19 for RT-PCR analysis. Survival analyses were used to determi…

Mixed Leptospira Infections in a Diverse Reservoir Host Community, Madagascar, 2013-2015.

We identified mixed infections of pathogenic Leptospira in small mammals across a landscape-scale study area in Madagascar by using primers targeting different Leptospira spp. Using targeted primers increased prevalence estimates and evidence for transmission between endemic and invasive hosts. Future studies should assess rodentborne transmission of Leptospira to humans.

Dermatophyte infection caused by Nannizzia gypsea: A rare case report from Madagascar.

We report a rare case of dermatophyte infection of the glabrous skin (Tinea corporis) caused by Nannizzia gypsea (formerly Microsporum gypseum). A 22-year-old Malagasy female who reported close contact reportedly with cats, presented a single round lesion with a peripheral, active, squamous and pruriginous inflammatory bead. Morphologic species identification was confirmed by sequencing the internal transcribed spacer (ITS) region of the genome. Specific treatment with oral loratadine and topical miconazole cream was effective.

Current knowledge on fleas (Siphonaptera) associated with human plague transmission in Madagascar.

Fleas are ectoparasites of mammals and birds. Some species are known for their medical and veterinary importance. Particularly for humans and domestic animals, fleas are often merely nuisance species, but may also be disease vectors. Some well-known infectious diseases are transmitted from animals to humans by flea bites: murine typhus, spotted-fever, bartonelloses and plague. Particularly for plague, more than 80 species were reported vectors in the world and some are internationally renowned and have been the focus of studies over decades. In Madagascar, fleas are associated with two disease…

Hematological and histopathological evaluation of meat-type quails fed Madagascar cockroach meal.

Aiming at the sustainability of meat production, insects can replace traditional ingredients in the diet of poultry. Studies evaluating performance in birds have emerged to ensure this ability, but few address the health parameters of animals. This study was carried out to evaluate the effects of the inclusion of Madagascar cockroach meal in traditional diets on hematological and histopathological traits of meat-type quails. The inclusion of Madagascar cockroach meal in the diet was evaluated in four levels: 0%, 6%, 12%, and 18%. Observations for hematological and histopathological traits from…

Epidemic of plague amidst COVID-19 in Madagascar: efforts, challenges, and recommendations.

The plague has been wreaking havoc on people in Madagascar with the COVID-19 pandemic. Madagascar's healthcare sector is striving to respond to COVID-19 in the face of a plague outbreak that has created a new strain on the country's public health system. The goal and activities of the gradual epidemic of plague in Madagascar during COVID-19 are described in this research. In order to contain the plague and the COVID-19 pandemic in this country, we have suggested long-term recommendations that can help to contain the outbreak so that it may spread to non-endemic areas.

Seroprevalence of Leptospira spp. Infection in Cattle from Central and Northern Madagascar.

Leptospirosis is a zoonotic disease of global importance, especially in tropical countries. The current Leptospira spp. seroprevalence in cattle from central and northern Madagascar is unknown. Thus, the aim of this study was to determine the seroprevalence resulting from infections with pathogenic Leptospira spp. in zebu cattle from these areas. Serum samples from 194 animals were tested by microscopic agglutination test (MAT) using a panel of 12 serovars as antigens. Samples with a titer of ≥1:100 were considered positive. The overall seroprevalence was 59.3% (95% CI; 52.0-66.2%) with titers…

Ecological plasticity and commercial impact of invasive marbled crayfish populations in Madagascar.

BACKGROUND: The marbled crayfish (Procambarus virginalis) is a monoclonal, parthenogenetically reproducing freshwater crayfish species that has formed multiple stable populations worldwide. Madagascar hosts a particularly large and rapidly expanding colony of marbled crayfish in a unique environment characterized by a very high degree of ecological diversity. RESULTS: Here we provide a detailed characterization of five marbled crayfish populations in Madagascar and their habitats. Our data show that the animals can tolerate a wide range of ecological parameters, consistent with their invasive…

Needs assessment for a formal emergency medicine residency program in southern Madagascar.

INTRODUCTION: World Health Organization data for Madagascar reveal that the nation's under age five mortality rate is 56/1000, and that its maternal mortality rate is 440/100,000. Malaria, leprosy, plague, and tuberculosis remain significant communicable disease threats. Malnutrition rates are improving but continue to impact negatively on the general health of the Malagasy population, especially in the southern region with its 1.9 million inhabitants. There are no emergency medicine (EM) training programs to serve the southern half of Madagascar, which has a large urban population in Fianaran…
