Central Mandibular Reconstruction by Semiopen Wedge Osteotomy Double-barrel Fibula Flap for a Slim Aesthetic Appearance.

Mandibular reconstruction involving the central segment after malignant tumor resection requires the achievement of both functional and aesthetic quality. Three-dimensional reproduction based on the concept of a double arc composed of a marginal arc and an occlusal arc is important. Most reports of fibula flaps applied three-segmented closed wedge fibula osteotomy (bilateral and central segments); however, the aesthetic outcome sometimes became too stout for female patients because of the large central segment. We performed four-segmented fibula osteotomy for a 78-year-old woman using a semiop

Posterior atrophic jaws rehabilitated with prostheses supported by 5 x 5 mm implants with a novel nanostructured calcium-incorporated titanium surface or by longer implants in augmented bone. Preliminary results from a randomised controlled trial.

PURPOSE: To evaluate whether 5 × 5 mm dental implants with a novel nanostructured calcium-incorporated titanium surface could be an alternative to at least 5 × 10 mm-long implants placed in bone augmented with bone substitutes in posterior atrophic jaws. MATERIALS AND METHODS: Forty patients with atrophic posterior (premolar and molar areas) mandibles having 5 to 7 mm of bone height above the mandibular canal and 40 patients with atrophic maxillae having 4 to 6 mm below the maxillary sinus, were randomised according to a parallel group design to receive one to three 5 × 5 mm implants or one t

A modified technique for expanded polytetrafluoroethylene shaping in chin augmentation: parallel groove carving technique.

OBJECTIVE: In the current study, we restricted our focus on a modified approach to address the poor curved attachment of expanded polytetrafluoroethylenein chin augmentation. METHOD: The implant is shaped generally in accordance with the chin, followed by 5-to-8 longitudinally parallel "V"-grooves carved in the median of posterior site where the implant directly attaches to the mandible. Thus, it enhances bend ductility of the prosthesis and renders better attachment from the curved surface of prosthesis to the mandibular embedded region. RESULTS: This procedure was performed in 15 patients.

One stage treatment of temporomandibular joint complete bony ankylosis using total joint replacement.

INTRODUCTION: Temporomandibular joint ankylosis is a joint disorder due to bone or fibrous adhesion of the joint components that cause loss of function. There are many causes, such as trauma, infections and systemic diseases. To date, no uniform treatment protocol has been established. MATERIALS AND METHODS: We enrolled in the study patients that underwent single stage alloplastic total joint replacement for complete bony ankylosis. The subjective and objective variables were as follow: TMJ pain, diet, jaw function, quality of life, maximum interincisal opening (MIO) and occlusion. The minimu

Endovascular coils mimicking accidental ingestion of a dental-related foreign body in radiographic imaging.

If a foreign body is seen on chest or abdominal radiographs, accidental aspiration or ingestion of a dental-related foreign body may be suspected. This report describes a case in which vascular embolization coils seen on radiography were suspected to represent a swallowed dental prosthesis. A 72-year-old man with a history of endovascular embolization of portosystemic shunt was admitted for mandibular fracture. On hospital day 2, a foreign body was noted on chest radiographs taken to confirm pleural effusion. No foreign body had been evident on radiographs of the same area the previous day. Th

CT-based retrospective cohort study for the evaluation of mandibular condylar dimensions - A pilot study.

BACKGROUND: The temporomandibular joint (TMJ) is a unique structure of the body where the mandible, one of the important facial bones, articulates with the temporal part of the skull bone. Obtaining morphometric dimensions for mandibular condyle is important for performing an accurate pre/postoperative assessment, planning temporomandibular and orthognathic surgeries, and applications in forensic sciences in context to the Indian population, which is presently based on dimensions of Caucasian population from available literature. Several investigators noticed the variation in the craniofacial

Biomechanically reduced expression of Derlin-3 is linked to the apoptosis of chondrocytes in the mandibular condylar cartilage via the endoplasmic reticulum stress pathway.

OBJECTIVE: The present purpose was to investigate the involvement of Derlin-3 in the endoplasmic reticulum stress pathway-mediated apoptosis of chondrocytes in biomechanically stimulated mandibular condylar cartilage. DESIGN: First, fluid flow shear stress (FFSS) was applied to ATDC5 cells with or without overexpression of Derlin-3 by lentiviral transduction or silencing of Derlin-3 by siRNA transfection. Apoptosis was evaluated by TUNEL assay. Molecular markers related to the endoplasmic reticulum stress-apoptosis pathway, including GRP78, CHOP, ATF6, Caspase-12, and cleaved Caspase-3, were

Implant-prosthetic rehabilitation of a patient with nonsyndromic oligodontia: a clinical report.

Oligodontia is defined as the absence of 6 or more permanent teeth due to the hypodevelopment of tooth germs. This familial abnormality is attributable to various mutations or polymorphisms of genes and associated with malformative syndromes. This clinical report presents the multidisciplinary dental treatment planning for a 27-year-old woman who exhibited oligodontia. After radiographic and clinical evaluations, 7 retained primary teeth and 2 mandibular third molars were extracted. Three dental implants were placed into the maxillary alveolar process. After the osseointegration period, comple

Temporomandibular joint reconstruction after condylar fracture complication related to osteosynthesis material.

Total condylar resorption and the failure of bone synthesis material as postoperative complications of condyle fracture are seldom reported in the literature. When these occur, they may severely limit temporomandibular joint (TMJ) function. In such cases, a reconstruction of the joint is indicated, which can be achieved by means of an alloplastic prosthesis. This article reports a rare case of impaction of osteosynthesis material in the region of the base of the skull associated with a mandibular condyle fracture treated with rigid internal fixation, which resulted in complications. There was

[Anatomy of the maxillary and mandibular retromolar area: effect on complete dentures. 2. The mandibular retromolar region].

Some confusion exists in the anatomical description of the mandibular retromolar area. The anatomical terms of retromolar area, papilla, tubercle, trigone, triangle, fossa, dimple, are often confused in the literature. This article specifies the terminology as well as the anatomical organization of this area, the use of which in prosthodontics is the subject of scholastic discussions, in full dentures. This clinical examination based on a precise anatomical knowledge, will determine the participation of the support area and its structures bordering this area. Compression of the lingual nerve b

[Distraction osteogenesis in correcting facial asymmetry malformation caused by temporomandibular joint ankylosis: a case report].

A facial asymmetry patient caused by temporomandibular joint ankylosis was operated by putting a distraction osteogenesis (DO)-appliance. One week after surgery, the right mandible was distracted. Twenty days after distraction, the height of right mandibular ramus and body increased obviously. Four months after retain of DO-appliance, the right maxilla down-grew automatically and the occluding relation recovered to normal. The DO-appliance was taken out. MEDPOR prosthesis was imbedded in new bone and genioplasty was operated. The facial asymmetry and abnormal occluding relation of the patient

Junctional epidermolysis bullosa associated with hypoplastic enamel and pervasive failure of tooth eruption: Oral rehabilitation with use of an overdenture.

This report describes a previously unreported case of generalized hypoplastic enamel and failure of eruption of the permanent maxillary teeth and only partial eruption of the permanent mandibular teeth in an 18-year-old male diagnosed with junctional epidermolysis bullosa. Similar anomalies were reported to have affected the deciduous dentition. Beginning at 4 years of age, oral rehabilitation has been conservatively managed with the fabrication of various maxillary complete overdentures. The use of this prosthesis has provided an economical, nonsurgical treatment option when oral soft tissue
