Reconstruction of the radiated partial mastectomy defect with autogenous tissues.

As conservative surgery and radiation therapy have become accepted treatments for early-stage breast cancer, increasing attention has focused on the cosmetic results of this technique. When partial mastectomy--a term which encompasses a diversity of excisional techniques--is followed by radiation therapy, breast defects characterized by parenchymal loss, nipple-areola complex distortion, and cutaneous abnormalities can occur. From 1981 to 1990, eight patients sought reconstructive correction of a radiated partial mastectomy deformity. Patients were from 42 to 70 years of age (mean 49 years). A…

The post-mastectomy pain syndrome and the effect of topical capsaicin.

Eighteen patients with the post-mastectomy pain syndrome (PMPS) form the basis of this study. PMPS probably occurs in a minority of women after mastectomy. The onset of persistent pain usually occurred immediately or very shortly after the operation. The pain location or sensory findings implied involvement of the territories of other cutaneous branches of the intercostal nerves as well as the intercostobrachial nerve. A variety of treatment approaches were unsatisfactory. Twelve of 14 patients completing treatment with topical 0.025% capsaicin showed improvement after 4 weeks and 8 (57%) were…

Prophylactic mastectomy for precancerous and high-risk lesions of the breast.

The candidates for prophylactic mastectomy are those who have a family history of breast cancer, multiple fibrocystic masses that have been biopsied or are too numerous to biopsy appropriately, or patients who previously have had cancer in one breast. The decision to perform the subcutaneous mastectomy is one that must be made by the surgeons involved, in consultation with the patient and her husband. The data from subcutaneous mastectomies appear promising and the technical results have improved as experience has been gained. However, 5 to 10 more years are necessary to determine whether this…

Failure of subcutaneous mastectomy to prevent the development of breast cancer.

Three patients treated with bilateral subcutaneous mastectomies and implants for fibrocystic disease or carcinoma in situ subsequently developed invasive breast carcinoma. These cases emphasize that subcutaneous mastectomy does not prevent the development of breast cancer. If the circumstances in an individual patient justify prophylactic mastectomy, then the appropriate procedure would seem to be total mastectomy, which would accomplish thorough removal of all breast tissue, the subareolar region, and the nipple. Subcutaneous mastectomy for the patient who is likely to harbor occult invasive …

Reversed reconstruction in subcutaneous mastectomy: insertion of the prosthesis in advance.

A new approach to reconstruction in subcutaneous mastectomy has been presented. It involves insertion of the prosthesis in advance as the first stage. After a review of 22 consecutive cases of subcutaneous mastectomy, we are convinced that it is the method of choice. In our opinion, most of the complications following subcutaneous mastectomy with augmentation can be diminished by using this technique. Another outstanding advantage is that the patient has less psychological trauma.

[Reimplantation of the areola and nipple in mastectomy operations for breast carcinoma].

The technique for the application of areola and nipple in the mastectomy for the mammary carcinoma is proposed. It is pointed out that the preservation of the nipple with its areola is psychologically important for the patient. It is also showed how the application of a silicone prosthesis, volumetrically restoring the extirpated tissue, is afterwards possible. During the operation, a disk of tissue is drawn under nipple and areola and histologically tested; if this tissue certainly results without neoplastics cells, the reapplication is achieved.

[Psychological impacts of mastectomy in elderly people with breast cancer].

Aging is one of the main risk factors for breast cancer and one of the most common treatment options is surgery, including mastectomy, which can result in psychological impacts on women. The scope of this article is to conduct a bibliographical survey on the psychological impacts of mastectomy on elderly women with breast cancer. It involved an integrative literature review, conducted between August and September 2023, via the VHL portal, in the following databases: LILACS, BDENF, SciELO, MEDLINE and Google Scholar. A total of 2,021 articles were located, 22 of which were selected in line with…

Nipple margin assessment at the time of nipple-sparing mastectomy.

BACKGROUND: Documenting negative margins at the nipple-areolar complex (NAC) during nipple-sparing mastectomy (NSM) remains the standard, but how to achieve this and how to manage a positive margin is debated. We sought to review nipple margin assessments at our institution and to analyze the risk factors of a positive margin and rate of local recurrence. METHODS: Patients who underwent NSM between 2012 and 2018 were reviewed and divided into 3 groups based on indication - cancer, contralateral prophylactic mastectomy (CPM) and bilateral prophylactic mastectomy (BPM). RESULTS: Nipple-sparing…

Locoregional recurrence in skin-sparing and nipple-sparing mastectomies.

BACKGROUND: Skin- and nipple-sparing mastectomies (SSMs/NSMs) present as an alternative for patients requiring mastectomy, with better aesthetic results. We aimed to evaluate the locoregional recurrence (LRR) rate and its predictive factors. METHODS: Retrospective analysis of all consecutive cases of SSM and NSM for a primary diagnosis of in situ or invasive breast cancer, at a national cancer center, from January 1st, 2013 to May 31st, 2019. The primary outcome was LRR. Secondary outcomes included LRR predictive factors, overall survival (OS), and disease-free survival (DFS). RESULTS: There…

Nipple-sparing mastectomy: A review of outcomes at a single institution.

Nipple-sparing mastectomy (NSM) offers patients who are not candidates for breast-conserving treatment an aesthetically pleasing alternative to traditional mastectomy. Some studies have demonstrated its oncologic safety while others have demonstrated residual occult tumor cells at the nipple-areolar complex (NAC). These data prompt further review of oncologic outcomes after NSM.A single institution retrospective chart review was performed of all NSMs performed by 4 breast surgeons at Thomas Jefferson University Hospital over a span of 2012-2019. In this cohort, we review the reconstruction per…

Prophylactic mastectomy for the prevention of breast cancer: Review of the literature.

The high incidence and recurrence rate of breast cancer has influenced multiple strategies such as early detection with imaging, chemoprevention and surgical interventions that serve as preventive measures for women at high risk. Prophylactic mastectomy is one of the growing strategies of breast cancer risk reduction that is of a special importance for breast cancer gene mutation carriers. Women with personal history of cancerous breast lesions may consider ipsilateral or contralateral mastectomy as well. Existing data showed that mastectomy effectively reduces breast cancer risk. However, car…

Risk-reducing mastectomy for the prevention of primary breast cancer.

BACKGROUND: Recent progress in understanding the genetic basis of breast cancer and widely publicized reports of celebrities undergoing risk-reducing mastectomy (RRM) have increased interest in RRM as a method of preventing breast cancer. This is an update of a Cochrane Review first published in 2004 and previously updated in 2006 and 2010. OBJECTIVES: (i) To determine whether risk-reducing mastectomy reduces death rates from any cause in women who have never had breast cancer and in women who have a history of breast cancer in one breast, and (ii) to examine the effect of risk-reducing maste…

Body Image in Transmen: Multidimensional Measurement and the Effects of Mastectomy.

INTRODUCTION: Transmen are generally dissatisfied with their breasts and often opt for mastectomy. However, little is known about the specific effects of this procedure on this group's body image. AIM: To prospectively assess the effect of mastectomy on the body image of transmen, including cognitive, emotional, and behavioral aspects. METHODS: During a 10-month period, all transmen applying for mastectomy were invited to participate in this study. The 33 participants completed assessments preoperatively and at least 6 months postoperatively. MAIN OUTCOME MEASURES: Participants were surveye…

[Operative management and techniques of mastectomy in female-to-male transsexuals].

BACKGROUND: Subcutaneous mastectomy in female to male transsexuals is one first important step in gender adjustment. Beside proper surgical technique, operative management of mastectomy procedures is important to allow for an optimised process of gender transformation including all necessary individual operations. METHODS: 126 female-to-male transsexuals were prospectively included in the observation period, all undergoing a step 1 operation of gender adjustment. The mastectomy procedure is conventionally embedded in an overall approach for gender transformation and was regularly combined wit…

Mastectomy, body deconstruction, and impact on identity: a qualitative study.

OBJECTIVES: This qualitative study aims at understanding the consequences of body deconstruction through mastectomy on corporality and identity in women with breast cancer. DESIGN: Nineteen women were contacted through the hospital. All had to undergo mastectomy. Some were offered immediate breast reconstruction, others, because of cancer treatments, had no planned reconstruction. A qualitative reflexive methodological background was chosen. METHOD: Women were invited to participate in three semi-structured interviews, one shortly before or after mastectomy, and the other interviews later in…
