Surgically assisted rapid maxillary expansion (SARME) is a well-documented and established procedure indicated to treat maxillary transverse deficiencies in the adult patient. Currently, the most popular SARME technique consists of performing a Le Fort I osteotomy without downfracture and a midline osteotomy that splits the maxilla into 2 halves between the central incisors. It is supposed that the 2 halves expand equally during the activation phase. However, after completion of the osteotomies, the expander is supported by only mobile segments; thus, if 1 side remains more resistant than the

INTRODUCTION: Trans-sutural distraction osteogenesis (TSDO) has emerged as a promising treatment for midfacial hypoplasia. However, the mechanical effects on craniofacial structures remain insufficiently understood.
MATERIAL & METHOD: Finite element models (FEM) of two adolescent patients with cleft lip and palate (CLP) (aged 10 and 11) were reconstructed from CBCT data. TSDO was simulated by applying symmetrical traction forces to the maxilla. A modified surgical intervention, transanterior maxillary wall osteotomy, was also modeled to assess its biomechanical impact. Displacement and stress

Patients with cleft lip and palate (CLP) exhibit complex maxillary morphology, including hypoplasia, making Le Fort I osteotomy challenging and prone to complications. The aim of this retrospective, comparative study was to improve the safety of Le Fort I osteotomy by analysing the maxillary structure of patients with CLP. Fifteen unilateral CLP cases with maxillary hypoplasia (six males, nine females) and 15 non-CLP cases undergoing mandibular setback for prognathism (four males, 11 females) were evaluated. Axial computed tomographic (CT) images at 2.5 and 5 mm above the nasal floor were anal

INTRODUCTION: Maxillary surgery alone can be proposed for the surgical management of class III malocclusion, but anticipating outcomes for the labiomental muscle complex is challenging due to the mandibular autorotation phenomenon. The objective of this study was to quantify the mandibular and labiomental movements induced by maxillary osteotomy alone in the management of class III malocclusion according to different clinical and surgical variables.
METHODS: The post-operative changes in mandibular and labiomental shapes were studied by geometric morphometry from the pre- and post-operative l…

BACKGROUND: Le Fort III osteotomy represents the foundation of surgical correction for midface hypoplasia. One serious complication of Le Fort III osteotomy is severing the internal maxillary artery or its branches during osteotome advancement for pterygomaxillary dysjunction. This study sought to characterize the relevant surgical anatomy of the infratemporal fossa and of the internal maxillary artery as it enters the pterygomaxillary fissure.
METHODS: Bilateral midface dissections were performed on 15 fresh, normocephalic adult cadavers (30 hemifaces). Four superficial anatomical measuremen

An invitation to report for a follow-up was accepted by 74 patients (51 females and 23 males) who had undergone various types of osteotomy (50 mandibular and 30 maxillary osteotomies; 6 patients had undergone both mandibular and maxillary osteotomy). Their age at the time of operation ranged from 15 to 44 years. The follow-up period ranged from 3 months to 7 years. Maxillary osteotomy was never followed by a relapse, but 12 of the 50 patients treated by mandibular osteotomy showed some relapse: hyperplasia of the mandibular condylar process caused some relapse in 2 cases, and in 10 other cases…

Virtual surgical planning (VSP) and patient-specific implants (PSIs) are increasingly used to translate digital orthognathic plans to surgery with greater precision, particularly in surgery-first orthognathic (SFO) protocols where pre-surgical orthodontics no longer absorbs planning error. We report two SFO cases managed at a single centre using VSP, customized cutting guides, and titanium PSIs. Case 1 was a 19-year-old male with maxillary hypoplasia and mandibular hyperplasia treated by Le Fort I osteotomy, vertical ramus osteotomy, and genioplasty. Case 2 was a 30-year-old female with facial

The purpose of this case report is to describe and discuss a combined surgical and orthodontic technique for the management of transverse maxillary deficiency and mandibular prognathism in the treatment of skeletal Class III malocclusion in a mature patient. Skeletal Class III malocclusion can present with maxillary deficiency or retrognathism, mandibular excess or prognathism, or a combination. The maxillary arch is narrow and often requires expansion. A 25-year-old patient presented with a constricted maxilla, a skeletal Class III malocclusion with a large mandible, Angle's Class III maloccl

INTRODUCTION: Le Fort I and sagittal split ramus osteotomies are the most commonly performed orthognathic surgery procedures on the maxilla and mandible, respectively.
TECHNIQUES: Despite progress in the techniques, these procedures may still be associated with morbidity, expressed as inflammation, inadequate bony union, periodontal damages or in extreme cases even total bone loss.
DISCUSSION: Through a comprehensive review of the literature, the influences of maxillary and mandibular surgery on Pulpal Blood Flow (PBF), pulp sensitivity and pulp vitality are examined. Moreover, adverse effec

Le Fort I (LF1) osteotomy, a common orthognathic procedure for the maxilla aimed at achieving maxillary mobility by separating the pterygomaxillary suture, poses a risk of bad fracture that may lead to complications and inadequate mobility. Our study analyzed two- and three-dimensional computed tomography images to identify the anatomical factors associated with bad fractures due to an LF1 osteotomy. Point 'a' is where the lateral pterygomaxillary suture on the axial image aligns with the zygomatic alveolar line near the line used for an LF1 osteotomy, with the base line connecting the bilater

INTRODUCTION: This study evaluated postoperative stability after Obwegeser II osteotomy (transoral angle osteotomy, first reported by Obwege … able stability after the correction of open bite with mandibular prognathism without a simultaneous maxillary osteotomy. An isolated Obwegeser II osteotomy can be considered a reliable option in cases with moderate to severe open bite with mandibular prognathism when the maxillary osteotomy is not needed if the patients have a well-positioned maxilla.

OBJECTIVE: To provide a simplified treatment strategy for patients with maxillary transverse deficiency. We investigated and compared the fracture mechanics and stress distribution of a midline palatal suture under dynamic loads during surgically-assisted rapid palatal expansion.
METHODS: Based on the cone-beam computed tomography (CBCT) data of a 21-year-old female volunteer, a three-dimensional model of the cranio-maxillofacial complex (including the palatal suture) was constructed. A finite element analysis model was constructed based on meshwork. After the yield strength of the palatal su

This study describes the use of a segmental Le Fort I osteotomy for interpositional autogenous iliac bone grafts in 5 patients with severe unilateral atrophy of the maxilla. Unilateral pterygomaxillary disjunction and osteotomy of the lateral and medial walls of the maxillary sinus were performed. The mucosa of the maxillary sinus was preserved and lifted cranially. It was stabilized with autogenous cancellous bone grafts. One side of the maxilla was gently downfractured until it replaced the original morphology of the alveolar crest. The inter-arch space was therefore reduced to the ideal lev
