PURPOSE: This article describes a surgical approach to extensive tumors in the pterygopalatine fossa extending into the maxillary sinus.
TECHNIQUE: The Barbosa approach was modified by adding a lateral incision in the mandibular gingivobuccal fold from the canine tooth to the retromolar area.
RESULTS: This technique allowed a large, inferiorly based flap to be raised, which includes the parotid gland. The masseter and temporalis muscles was divided horizontally, and the ascending ramus of the mandible was osteotomized between the mandibular angle and the sigmoid notch and reflected to expose

Between the years 1973-1979 173 operations were carried out at the ORL-Hospital Oldenburg for the rehabilatation of the maxillary sinus. Primary, only 48 patients had indications of Caldwell-Luc radical operation by either purulent or purulent-polypous sinusitis, but 43 cases were operated again because of different complications after primary operation in other hospitals. Due to this fact, the following questions were raised: 1. Were the indications to the intervention right? 2. What is the optimal technique of operation? 3. What are the reasons for complaints after the operation? Caldwell-Lu

Haemangioma originating in the paranasal sinuses are a rare entity. In the case of unilateral sinusitis the differential diagnosis should include tumors. The following case of a 30-year-old female patient with a therapy-resistant sinusitis showed bone destruction and a maxillary shadow on computed tomography. The histological exam resulted in a cavernous haemangioma.

Inflammatory pseudotumor (IP) is a clinically aggressive but histologically benign condition of unknown cause. It rarely appears in the nasal cavity and sinuses. Here, we describe a 24-year-old female with the main symptom of right nasal obstruction. Examinations showed dilation in the right maxillary sinus and a pale neoplasm in the nasal cavity. The neoplasm was completely excised under endoscope. Postoperative pathology showed significant proliferation of plasma cells and lymphocytes, indicating the presence of IP. No recurrence was found during 20 months of follow-up. Only 28 cases (10 mal

Malignant schwannomas are rare neoplasms that are seldom found in the head and neck. Few cases have been reported involving paranasal sinuses and none of them was of the "epithelioid" type. In this report, an unusual case of epithelioid malignant schwannoma involving the maxillary sinus, nasal cavity and orbit is presented. The patient was a 27-year-old male with a history of headache, nasal obstruction and epistaxis. Histologically, the tumour had a biphasic pattern with spindle and epithelioid elements which led to a differential diagnosis with malignant melanoma. It had also to be distingui

INTRODUCTION: Osteomas are slow- growing, benign tumors. They are the most common neoplasms of the paranasal sinuses. They are usually originates from the frontal and ethmoid sinus and much less frequently seen in the maxillary and sphenoid sinuses. Although the lamina papyracea is a part of ethmoid bone, a giant osteoma originated from the lamina papyracea is very uncommon. An osteoma of the paranasal sinus is usually asymptomatic. Headache, proptosis, epiphora, diplopia, dizziness, facial deformity, face pain and cerebral complications are possible symptoms. The treatment of the paranasal os

Metastases of malignant tumors to the nasal cavity and paranasal sinuses are rare. A review of the world's literature reports only four cases of antral metastases from carcinoma of gastrointestinal tract. We present a case of a stenosing adenocarcinoma of the sigmoid colon with metastases within the maxillary sinus. The ENT physical examination revealed a mass involving the right alveolar ridge, the right hard palate. CT scan of the head and the neck showed a large and irregular mass involving the right maxillary sinus, the infratemporal fossa and the pterygoid muscles with resorption of the b

Ameloblastomas are benign but aggressive odontogenic tumors that most commonly affect the posterior mandible. Approximately 15% occur in the maxilla, with a subset thought to originate from the epithelial lining of the sinonasal cavities. Histologically, sinonasal ameloblastomas are identical to those of the oral cavity, with classical features of palisaded columnar basilar cells surrounding a central proliferation that resembles the stellate reticulum of a developing tooth. Unlike the gnathic variant, sinonasal ameloblastomas tend to affect males more than females, and the incidence of diagno

An 11 month old Caucasian male presented with swelling of the right side of the nose and buccal vestibule of unknown duration. Radiographic images revealed a well-circumscribed, hypodense soft tissue mass with a peripheral hyperdense circumference involving the anterior aspect of the right maxilla. The lesion extended from the inferior aspect of the orbital rim superiorly to the maxillary bone inferiorly without invading any nearby structures. The lesion was completely resected via combined extraoral and intraoral approach. The clinical, radiographic, histologic and immunohistochemical feature

Neurofibromas are benign tumours of peripheral nerve tissue, frequently associated with neurofibromatosis type I. Their isolated occurrence in the maxillary sinus is rare, with only 6 cases described in the literature. We present the case of a 70-year-old woman who consulted for left hemifacial paresthesias of 10 years' evolution. CT showed a heterogeneous lesion in the left maxillary sinus, eroding its lateral wall without signs of infiltration. Endoscopic surgery with nasalisation of the maxillary sinus was performed. The pathology was neurofibroma (positive for S-100 protein and vimentin).

Leiomyosarcoma is a malignant smooth-muscle tumour that has a predilection for the gastrointestinal and female genital tract. It is locally fast-spreading and highly aggressive, and the prognosis is poor. We report a rare case of leiomyosarcoma of the maxilla in a patient who sought treatment for maxillary swelling, nasal obstruction with no epistaxis, orbital involvement or cervical lymph node metastasis. The patient underwent subtotal maxillectomy followed by radiotherapy. At present, he is symptom free with no recurrence and under regular follow up.

The inflammatory pseudotumor of the head and of the neck is a lesion of unknown etiology which affects the orbita more frequently but cases have also been described in the paranasal sinuses, larynx, mouth, cervical column and by direct extension from the orbita in the Central Nervous System. In the majority of cases, it behaves as a limited benignant inflammation within the osseous walls of the area where it is to be found; other times it destroys the osseous limits and invades the surrounding areas, behaving as a neoplasia. It must be distinguished from sinusitis, granulomatous inflammations,

Psammomatoid ossifying fibroma (POF) is a diagnostic designation for a sinonasal or orbital fibroosseous lesion characterized by numerous small round mineralized spherules lying in a fibroblastic stroma. We report 3 cases. One occurred in the frontal sinus of an 18-years-old male; 1 in the frontal sinus of a 20-year-old male and 1 in the maxillary sinus of a 36-year-old male. Rather limited excision resulted in a disease-free follow-up time from 2 to 6 years. The histomorphology of POF was compared with other fibro-osseous lesions of the craniofacial skeleton including the jaws. It appeared th

The objective of this report is to present an unusual case of a gigantiform cementoma manifesting with gross expansion of the maxilla in a 6-year-old black female. No history of similar lesions in the family of the patient was obtained. Radiographic examination revealed a well-circumscribed lobular radio-opaque mass surrounded by a radiolucent margin. The macroscopic specimen showed lobular mineralized masses with a ″ginger root″-like appearance. Microscopic examination showed a lobular calcified mass with a peripheral zone of fibropsammomatous tissue corresponding with the radiolucent margin.

Lymphomas are the malignant neoplasms of lymphocytes and their precursor cells. Their diagnosis can sometimes be difficult due to their similarity to various other entities. A 10-year-old female reported swelling on the right side of the upper jaw for a month which was associated with mild continuous pain. On examination, a mild diffused swelling was noted on the right middle third of the face region which was firm in consistency and slightly tender. Intraorally, a firm tender swelling was noted on the right side of the hard palate. A proximal caries was noted with 55. A provisional diagnosis
