Computed tomographic reconstruction of a fetus with the dysgnathia complex (agnathia-otocephaly).

The dysgnathia complex (agnathia-otocephaly) (AO) is a lethal malformation that consists of congenital absence of the lower jaw with union or close approximation of the lower ears on the front of the neck, microstomia, and hypoglossia. We present a novel case of agnathia-otocephaly with associated organomegaly and the use of postmortem 3-D computed tomographic rendering.

Chromosome 1p36 deletion syndrome: prenatal diagnosis, molecular cytogenetic characterization and fetal ultrasound findings.

OBJECTIVE: To present prenatal diagnosis and molecular cytogenetic characterization of de novo partial monosomy 1p (1p36.23→pter) and partial trisomy 20p (20p12.1→pter) associated with ventriculomegaly, ventricular septal defect and midface hypoplasia. MATERIALS, METHODS AND RESULTS: A 31-year-old, primigravid woman was referred for amniocentesis at 20 gestational weeks because of ventriculomegaly, ventricular septal defect, and midface hypoplasia. Amniocentesis revealed an aberrant derivative chromosome 1, or der(1). Parental karyotypes were normal. Spectral karyotyping analysis revealed tha

Genesis of alcohol-induced craniofacial dysmorphism.

The initial diagnosis of fetal alcohol syndrome (FAS) in the United States was made because of the facial features common to the first cohort of patients. This article reviews the development of an FAS mouse model whose craniofacial features are remarkably similar to those of affected humans. The model is based on short-term maternal treatment with a high dosage of ethanol at stages of pregnancy that are equivalent to Weeks 3 and 4 of human gestation. At these early stages of development, alcohol's insult to the developing face is concurrent with that to the brain, eyes, and inner ear. That fa

[Considerations of the classification and treatment for fused teeth].

Fused teeth were a phenomena of teeth anomalies in shape, which can affect the dental teeth both in primary and permanent dentition. Fused teeth do not only cause problems on crowding of dentition, abnormal occlusion and aesthetic, but also increase risks of dental caries, endodontics diseases, periapical diseases and periodontal diseases. Fusion of deciduous teeth may lead to abnormality of subsequent permanent teeth. Treatment of fused teeth may require multidisciplinary approach in endodontics, periodontics, oral and maxillofacial surgery, prosthodontics and orthodontics. The aim of the pre

Mandibular infarction occurring during a sickle cell crisis.

The genetic abnormality and clinical problems associated with sickle cell disease are described and exemplified by a report of a patient who presented during a crisis with facial pain and sensory changes in the distribution of the inferior dental nerve; investigations revealed that these were due to infarction of part of the mandible. Although similar cases have been described, it is believed that the radionuclide scan of the mandible in the investigation of this patient uniquely demonstrated the position and extent of the infarcted area. The management of patients with sickle cell disease in

The psychological and social aspects of orthognathic treatment.

Cooperative management of dentofacial abnormalities by orthodontists and oral and maxillofacial surgeons has experienced tremendous growth in recent years. Frequently these patients have suffered considerable emotional trauma from ridicule of their deformity. This history of emotional abuse may influence the patient's attitude and expectations regarding corrective treatment and even lead to dissatisfaction in spite of technical success. Review of the literature and interviews with patients previously treated for dentofacial abnormalities revealed a very high rate of satisfaction. This high deg

Breathing challenges in achondroplasia: a comprehensive review of pediatric respiratory complications.

Achondroplasia is the most common skeletal dysplasia, characterized by disproportionate short stature and associated craniofacial, musculoskeletal, and neurological abnormalities. Respiratory complications are frequent and represent a major contributor to morbidity and mortality. Sleep-disordered breathing (SDB), encompassing obstructive (OSA) and central sleep apnea (CSA), is the most prevalent respiratory complication with pediatric prevalence reported between 22% and 93%. OSA results primarily from multilevel upper-airway obstruction related to craniofacial abnormalities and adenotonsillar

Cuffed oropharyngeal airway for difficult airway management.

Difficulties with airway management are often caused by anatomic abnormalities due to previous oral surgery. We performed general anesthesia for a patient who had undergone several operations such as hemisection of the mandible and reconstructive surgery with a deltopectoralis flap, resulting in severe maxillofacial deformation. This made it impossible to ventilate with a face mask and to intubate in the normal way. An attempt at oral awake intubation using fiberoptic bronchoscopy was unsuccessful because of severe anatomical abnormality of the neck. We therefore decided to perform retrograde

[Primary report of single rib-cartilage recombination transplantation in Binder syndrome treatment].

OBJECTIVE: To explore the surgical methods and advantages of single rib-cartilage recombination transplantation in Binder syndrome treatment. METHODS: Five patients were treated with single rib-cartilage recombination transplantation. We harvested only the seventh costal cartilage including partial costal bone of the same rib to correct all the deformities. The rib was divided into 3 parts: 1 I-shaped cartilage, 1 inverted-T-shaped cartilage, and 1 C-shaped implant (later divided into a C and a reversed-C implant) composed of rib bone and cartilage. The first 2 parts were constructed to an L-

Binder phenotype in mothers affected with autoimmune disorders.

OBJECTIVE: To report four foetal cases of the Binder phenotype associated with maternal autoimmune disorders. PATIENTS AND METHODS: In three mothers with autoimmune diseases, 2D and 3D ultrasonographic measurements were made on four foetuses with the Binder profile, and were compared with postnatal phenotypes. RESULTS: The Binder phenotype can be detected in early pregnancy (14.5 WG). All foetuses had verticalized nasal bones and midfacial hypoplasia. Punctuate calcifications were found in almost all the cases. No specific maternal auto-antibody has been associated with foetal Binder phenoty

Freeman-Sheldon syndrome: dental and orthodontic implications.

The manifestations of the Freeman-Sheldon syndrome (FSS) in four members of a South African family of Xhosa stock have been documented. Orofacial manifestations are a major syndromic component and warrant early, specialized orthodontic intervention. Our protocol for dental management is outlined and suggestions for holistic oro-dental care are provided.

The dentofacial manifestations of XXXXY syndrome: a case report.

This paper presents a six-year-old patient with XXXXY syndrome, whose oral findings included a cleft soft palate, hyper- or meso-taurodontism in eight primary molars and in the mandibular permanent first molars, five congenitally missing premolars, and delayed development of the permanent tooth germs. The maxillary and mandibular primary central incisors were in a cross-bite relationship. Cephalometric findings showed a short ramus of the mandible and a short maxilla in the anterioposterior plane. The anteroposterior jaw relationship was in harmony. The cross-bite was considered to be due to t

[Use of the laryngeal mask for airway control in difficult intubations in children].

LMA was introduced in clinical practice by Arthur Brain in 1983 as a valuable substitute of tracheal tube in adult who underwent general anaesthesia; since then its applications have been extensively studied. LMA is a relatively new non-invasive ventilatory device which has allowed a radical change in the management of modern general anaesthesia. In this study, the application of LMA is assessed during induction and maintenance of general anaesthesia in children affected by severe facial deformities that could render the placement of the tracheal tube difficult. Three patients were affected by

[Reconstruction of frontal bone, periorbital and calvarial defects with autogenic bone].

The repair of bony defects in the craniomaxillo-facial region in 40 patients is reported. In 22 patients with autogenous calvarian bone grafts no changes in the grafted bone were found. However, in 4 out of 11 patients with iliac bone grafts, partial bone resorption was observed. Repair of bony defects of the face with autogenous calvarian bone is the therapy of choice. It is superior to any other transplant or implant.

Autosomal dominant osteopetrosis and maxillomandibular osteomyelitis.

Osteopetroses represent a heterogeneous group of rare, hereditary bony dysplasias. They range from a devastating neurometabolic disease (including severe malignant infantile osteopetrosis) to 2 more benign conditions principally affecting adults: autosomal dominant osteopetrosis (ADO) type I and type II. The present study describes the maxillofacial manifestations associated with the 2 subgroups of ADO. In this paper, we present the oldest patient described in the literature with ADO type I (76 years old). We also document the first ADO type II patient described in the literature with synchron

The heritability of malocclusion: Part 1--Genetics, principles and terminology.

The relative contribution of genes and the environment to the aetiology of malocclusion has been a matter of controversy throughout the twentieth century. Genetic mechanisms are clearly predominant during embryonic craniofacial morphogenesis, but environment is also thought to influence dentofacial morphology postnatally, particularly during facial growth. Orthodontic and orthopaedic techniques are used in the treatment of malocclusion and other dentofacial deformities, but with limited effectiveness. The key to the determination of the aetiology of malocclusion, and its treatability lies in t
