BACKGROUND: Non-invasive respiratory support for neonates using bubble continuous positive airway pressure (bCPAP) delivery systems is now widespread owing to its safety, cost effectiveness and easy applicability. Many innovative solutions have been suggested to deal with the possible shortage in desperate situations like disasters, pandemics and resource-limited settings. Although splitting of invasive ventilation has been reported previously, no attempts to split non-invasive respiratory support have been reported.
OBJECTIVE: The primary objective was to test the feasibility of splitting th

BACKGROUND: Neonatal emergencies contribute significantly to under-five morbidity and mortality in developing countries, partly due to poverty and limited access to quality healthcare in rural communities.
AIMS: The aim is to evaluate the spectrum, outcome, case fatality rates (CFRs), and prognostic factors of neonatal emergencies seen in the two free health-care facilities in Ondo State.
METHODS: This was a cross-sectional descriptive study of neonatal emergencies. Data were collected on eligible consecutive infants using a self-designed questionnaire. Neonatal emergencies were classified b

INTRODUCTION: Annual eye examinations are recommended for diabetic patients in order to detect diabetic retinopathy and other eye conditions that arise from diabetes. Medically underserved urban communities in the US have annual screening rates that are much lower than the national average and could benefit from informatics approaches to identify unscreened patients most at risk of developing retinopathy.
METHODS: Using clinical data from urban safety net clinics as well as public health data from the CDC's National Health and Nutrition Examination Survey, we examined different machine learni

This article describes the Parish Nurse Faculty Practice Model (PNFPM), which provides care to a medically underserved, high-risk homeless population at a community-based, multipartner service center. The PNFPM offers a holistic integrated approach to care of the mind, body, and spirit and encourages those who are homeless to draw on their faith to improve their health. The faculty practice integrates faith and spirituality as a way to improve health and decrease health disparities using Healthy People 2010's Focus Areas to guide the practice. A variety of approaches are used including art the

BACKGROUND: Live transmission of echocardiography can provide a rapid and accurate diagnosis in identifying neonates who need emergent transfer to a tertiary care center. This prevents a delay in management and/or unnecessary transport.
OBJECTIVE: The purpose of this study was to review the utility, accuracy, and benefits of tele-echocardiography in primary care hospitals in South Dakota to assess neonates with suspected congenital heart disease.
METHOD: Two primary care hospitals established a telemedicine link to our tertiary care center. A pediatric cardiologist interpreted neonatal tele-

Visceral leishmaniasis, or kala azar (KA), affects the rural poor, causing significant morbidity and mortality. We examined the epidemiologic, social, and economic impact of KA in a village in Bangladesh. A population-based survey among 2,348 people demonstrated a KA incidence of 2% per year from 2000 to 2002, with a case-fatality rate of 19% among adult women, compared with 6-8% among other demographic groups. Kala azar cases were geographically clustered in certain sections of the village. Anti-leishmanial drug shortages and the high cost of diagnosis and treatment caused substantial emotion

BACKGROUND: Cervical cancer remains one of the leading causes of cancer for women in medically underserved areas. This is in part due to a lack of trained clinicians to provide the necessary diagnosis and treatment of precancerous lesions to prevent cervical cancer. Increasing medical provider knowledge and skills is important for the early detection and prevention of cervical precancer and cancer in medically underserved areas of the United States and globally.
METHOD: LUCIA is a low-cost, universal cervical cancer instructional apparatus that can be used to teach and practice a variety of e…

In general, inhabitants of low socio-economic areas are unhealthier than inhabitants of high socio-economic areas, but some areas are an exception to this rule. These exceptions imply that other factors besides the socio-economic level of an area contribute to the health of the inhabitants of an area, e.g. environmental factors. In our study we concentrate on areas within the Netherlands that are healthier or unhealthier than could be expected based on their socio-economic level. This study first identifies these areas and secondly determines which area characteristics distinguish these areas

Despite the known developmental benefits of early intervention for autism spectrum disorder (ASD), diagnosis before age 5 years is often not achieved. Research suggests that lack of health insurance and living in rural areas and areas of severe provider shortages contribute significantly to these delays. The purpose of this project was to conduct a geospatial evaluation of potential gaps in early ASD diagnosis of uninsured children in Virginia. A secondary purpose was to show the use of geospatial analysis by pediatric nurse practitioners for policy advocacy. We mapped data from a statewide pr

CONTEXT: Public policymakers and their advisers struggle with the problem of specifying criteria by which health care providers in rural areas are eligible for special consideration in payment policies and for special grant programs. A means of designating places can provide a basis for assistance and can help target public resources for any providers who deliver services in those places.
PURPOSE: This paper provides the details underlying a place-based approach to identifying rural areas that are at risk for not being able to provide requisite health services.
METHODS: A population size cri

BACKGROUND/OBJECTIVES: Women with prior gestational diabetes mellitus (GDM) are at higher risk for prediabetes, particularly when inactivity or poor diet persists after childbirth. These behaviors often co-occur, and their combined effect is greater than the sum of individual risks. This study aimed to identify physical activity and dietary profiles among women with prior GDM in underserved areas, examine their association with impaired fasting glucose (IFG) and impaired glucose tolerance (IGT), and investigate their associated factors.
METHODS: A cross-sectional analysis of baseline data col

Screening mammography is the best available means of detecting breast cancer in its early, more treatable stages. Yet many women in the United States who are of screening age have not had a recent mammogram. This article explores the reasons women are underserved and underscreened, including cost, examination-associated pain, and inconvenience, as well as possible solutions to these and other barriers to screening mammography. The needs of special patient populations, such as women with disabilities, also are addressed, along with general measures for encouraging mammography screening particip

Developmental disabilities, emotional disorders and disruptive behaviour disorders are the leading mental health-related causes of the global burden of disease in children aged below 10 years. This article aims to address the treatment gap for child mental disorders through synthesising three bodies of evidence: the global evidence base on the treatment of these priority disorders; the barriers to implementation of this knowledge; and the innovative approaches taken to address these barriers and improve access to care. Our focus is on low-resource settings, which are mostly found in low- and m

A primary care cancer genetics project, funded by Macmillan Cancer Support and the Department of Health, has been running in North Kirklees since 2004. North Kirklees has a high ethnic minority population (mainly South Asian) and 50% of its wards lie in the most deprived quintiles in England. Previous audits in the department of genetics have shown lower than expected referral rates for patients from lower socio-economic classes and no referrals from ethnic minority patients. The aims of the project included improving access to cancer genetics services for disadvantaged patients from lower soc
