Access to medical care: how do women in Canada and the United States compare?

OBJECTIVES: The purpose of this study is to determine if access to medical care and utilization of cancer screenings differs between women in the United States and Canada. This study examined this question by comparing women in Canada to women in the United States who have insurance coverage and those who do not. METHOD: This study used data from the 2002/03 Joint Canada United States Survey of Health and examined access to medical care and cancer screenings. A binary probit model was used to address several measures of access to medical care and cancer screening utilization. RESULTS: This s

Implementation of cocooning against pertussis in a high-risk population.

BACKGROUND: In 2006, the Advisory Committee on Immunization Practices recommended tetanus, diphtheria, acellular pertussis (Tdap) vaccination of all caregivers of infants aged <1 year ("cocooning") to prevent pertussis-related complications and deaths. We implemented cocooning in a predominantly Hispanic, medically underserved, uninsured population at a Houston hospital. Phase 1 (January 2008-January 2010) provided maternal postpartum Tdap vaccine; Phase 2 (June 2009-January 2010) also vaccinated infant contacts on-site. METHODS: Pertussis education was provided to health care personnel and m

Patient and hospital characteristics associated with inappropriate percutaneous coronary interventions.

OBJECTIVES: This study sought to examine whether rates of inappropriate percutaneous coronary intervention (PCI) differ by demographic characteristics and insurance status. BACKGROUND: Prior studies have found that blacks, women, and those who have public or no health insurance are less likely to undergo PCI. Whether this reflects potential overuse in whites, men, and privately insured patients, in addition to underuse in disadvantaged populations, is unknown. METHODS: Within the National Cardiovascular Data Registry CathPCI Registry, we identified 221,254 nonacute PCIs performed between Jul

Geographic and Socioeconomic Predictors of Early Follow-Up Loss in Patients Undergoing Pituitary Neuroendocrine Tumors (PitNETs) Resection: A Retrospective Review of 1143 Cases.

OBJECTIVE: Postoperative follow-up after pituitary neuroendocrine tumor (PitNET) resection is essential for monitoring tumor recurrence and endocrinologic deficits. Despite advances in telehealth, loss to follow-up remains a persistent challenge across neurosurgical patients. This study aims to identify factors to identify patients are at higher risk of being lost to follow-up after surgery. METHODS: A retrospective analysis was conducted on 1143 adult patients who underwent PitNET resection between 2012 and 2019 at a single institution. Clinical and radiological data were analyzed to assess

Increased clinical trial enrollment among adolescent and young adult cancer patients between 2006 and 2012-2013 in the United States.

BACKGROUND: Stagnant outcomes for adolescents and young adults (AYAs) 15-39 years of age with cancer are partly attributed to poor enrollment onto clinical trials. Initiatives have focused on increasing accrual, but changes at the population-level are unknown. We examined patterns of clinical trial participation over time in AYA patients with cancer. PROCEDURE: We utilized medical record data from AYAs in two population-based National Cancer Institute Patterns of Care Studies identified through the Surveillance, Epidemiology and End Results Program. Among 3135 AYAs diagnosed with non-Hodgkin

Factors associated with failure to complete isoniazid treatment for latent tuberculosis infection in Rhode Island.

BACKGROUND: The treatment of latent tuberculosis infection (LTBI) is essential for tuberculosis elimination in the United States, but the major limitation is poor adherence to therapy. To aid the design of targeted adherence interventions, we investigated the factors associated with noncompletion of isoniazid (INH) therapy for LTBI. METHODS: A retrospective analysis of patients with who failed to complete vs those who completed 9 months of INH therapy at the RISE TB Clinic (Miriam Hospital; Providence, RI) in 2003 was performed. Factors associated with treatment noncompletion were examined us

Nutrition risk considerations in uninsured adult patients in a free primary care clinic.

This study examines patient and provider perceptions of nutrition risk, compares these perceptions with objective measures, and suggests practices to support improved nutrition-related health outcomes. Using observational methods, surveys were disseminated to uninsured, low-income patients (N = 150) and providers (N = 5) to assess diet quality, malnutrition risk, and food insecurity. Analyses, which used descriptive statistics and Spearman's Rho correlations, included three objective nutrition risk indicator scores based on validated tools, perceptions of nutrition risk, and perceptions of nut

Achieving public health goals through Medicaid expansion: opportunities in criminal justice, homelessness, and behavioral health with the Patient Protection and Affordable Care Act.

States are currently discussing how (or whether) to implement the Medicaid expansion to nondisabled adults earning less than 133% of the federal poverty level, a key aspect of the Patient Protection and Affordable Care Act. Those experiencing homelessness and those involved with the criminal justice system--particularly when they struggle with behavioral health diagnoses--are subpopulations that are currently uninsured at high rates and have significant health care needs but will become Medicaid eligible starting in 2014. We outline the connection between these groups, assert outcomes possible

Practice-Level Severity Case Mix and Treatment Patterns for Premenopausal Noncancerous Hysterectomy.

Background: Hysterectomy for noncancerous conditions is a patient-preference-sensitive procedure. Therefore, gynecological practices may provide hysterectomy at varying levels of symptom severity. We assess whether practice-level severity case mix associates with segregation of patients by race and ethnicity or insurance status. Methods: In this case series, we analyzed electronic health records of 1,590 noncancerous hysterectomy patients across 20 clinical practices within a large health care system in the U.S. South (2014-2017). By abstracting 12-month presurgical medical notes, we developed
