OBJECTIVE: To compare the frequency of intestinal metaplasia (IM) in patients with portal hypertensive gastropathy (PHG) to the control group with functional dyspepsia.
METHODS: Two-hundred and eighty-nine cases were prospectively evaluated in three groups (controls:group I--123 patients; cirrhotics: group II--135 patients; noncirrhotic portal hypertensives: group III--31 patients). Myelofibrosis with myeloid metaplasia (MMM) is an infrequent disease usually affecting elderly people. Due to the lack of a specific marker for MMM, efforts are being made to refine the diagnostic criteria. Intestinal metaplasia (IM) foci in 19 antral and 14 fundal gastric biopsies from patients with chronic atrophic gastritis were studied immunohistochemically for the presence of CALLA antigen. In only 2 cases were metaplastic glands completely negative, in 14 cases they were all positive, and in 17 cases variable proportions of CALLA positive and negative metaplastic glands were present. Tests for a dysimmune state were done in an unselected group of 67 patients with agnogenic myeloid metaplasia (AMM). A patient with angogenic myeloid metaplasia in blastic transformation with unusually prominent lymphadenopathy simulating lymphoma is described. Interestingly, the initial evidence of myeloblastic transformation was present in the lymph nodes but not in the marrow. Serial bone marrow biopsies showed fibrosis throughout the course of the disease, in spite of a gradual increase in the percentage of peripheral blood myeloblasts.
