BACKGROUND: Some users of the etonogestrel contraceptive implant experience bothersome bleeding, which can reduce contraceptive satisfaction and continuation. Few strategies exist to manage this bleeding. The exact mechanism of progestin-induced bleeding is unknown, but it is likely multifactorial (eg, impaired angiogenesis, "leaky" fragile vasculature, and inflammation). OBJECTIVE: Compare bleeding patterns for levonorgestrel 52 mg, 19.5 mg, and 13.5 mg intrauterine system (IUS) products using the World Health Organization Belsey definitions.
STUDY DESIGN: We extracted available data on bleeding patterns from published sources. Lower dose products had published data at 1 and 3 years; the 52 mg IUS had available data for 1, 2 and 3 years for amenorrhea and 1 and 2 years for other bleeding patterns. We interpolated 2-year data for the lower dose products based on 1- and 3-year data and compared bleeding pattern rates using Fisher exact testing.

Submucous myomas can cause heavy bleeding, pain, and infertility. Sonohysterography and office hysteroscopy can provide accurate diagnosis. Classification systems have been developed to help predict the difficulty of resection. Patient selection and proper surgical technique are essential to achieve good results and reduce the risk of complications. Hysteroscopic treatment can in many cases provide excellent results and eliminate the need for major surgery.

From January 1987 to December 1993, 247 patients underwent endometrial ablation as an alternative to hysterectomy in our practice. All these patients had abnormal uterine bleeding, and were evaluated by diagnostic office hysteroscopy and endometrial biopsy and then treated in the hospital outpatient center. The histories of 139 patients who received treatment for tubal pregnancy between January 1975 and December 1981 in our department were reviewed. 12% of the patients had a prior contralateral tubal pregnancy.
