Inability to hydrolyze lactose does not always cause symptoms. The lactose tolerance test commonly used in diagnosis pinpoints the biochemical anomaly but does not establish whether it causes a functional disability. I therefore compared a milk tolerance test (500 ml milk) with the standard lactose tolerance test in 40 healthy adult volunteers for rise in blood sugar. A maximum glucose rise of 9 mg/100 ml or less indicated lactose malabsorption. A patient with contact urticaria to cow milk is presented. Contact sensitivity was documented to whole cow milk, skim milk, commercial condensed milk, and milk heated to 80 degrees C for 30 min. Contact sensitivity was also documented to casein and lactalbumin. Unlike a previous report our patient's sensitivity to heated milk precluded its use in the diet. Thus, heating milk does not necessarily reduce allergenicity to its component proteins.

An adolescent, contemplating a military career, was found to have orthostatic proteinuria. A thorough evaluation failed to reveal any renal abnormalities. He was tested on a milk-free diet at the request of the parents. The proteinuria cleared, but recurred on an initial open challenge; it did not recur subsequently on a blinded challenge. This experience suggests that patients with orthostatic proteinuria should be evaluated with an elimination diet trial.

Breast milk from 99 Sudanese mothers was analysed for aflatoxins. Aflatoxins M1 and/or M2 were detected in 37 of the milks. No other aflatoxin was detected. M1 occurred alone in 13 milks, (mean 19.0 pg/ml), M2 in 11 milks (mean 12.2 pg/ml), and in 13 samples both M1 and M2 were detected. Contact urticaria developed in a child as a reaction to cow's milk. After heating the milk to 80 degrees C for thirty seconds, the child did not have a similar reaction. To my knowledge, this is the first case of contact urticaria to cow's milk in which there were no systemic manifestations of allergy.

The clinical features, the results of gastric secretory function tests, and the duodenojejunal morphology of six infants (aged 0.42-1.23 years) with anemia and melena considered to be due to latent cow's milk intolerance (LCMI) were compared with the findings in nine infants (aged 0.19-0.87 years) with cow's milk-induced malabsorption (CMI).
