Determining the minimal important difference of EQ-5D-5 L utility values in CIDP patients using data from a large clinical trial.

PURPOSE: Chronic Inflammatory Demyelinating Polyradiculoneuropathy (CIDP) is a rare autoimmune disorder characterized by limb weakness and sensory disturbances. This study aimed to estimate the minimal important difference (MID) of EQ-5D-5 L utility values in CIDP patients. METHODS: ADHERE, the largest CIDP trial to date, demonstrated the efficacy of efgartigimod. In Stage A (open-label; n = 322), patients received efgartigimod for up to 12 weeks or until evidence of clinical improvement (ECI). Patients with ECI entered Stage B, receiving efgartigimod (n = 111) or placebo (n = 110) for up to

Raw, centile and individualised minimal clinically important differences for the MMN-Rasch-built Overall Disability Scale© in the monitoring of multifocal motor neuropathy.

BACKGROUND: Optimal clinical application of the minimal clinically important difference (MCID) for the MMN-Rasch-built Overall Disability Scale© (MMN-RODS) scale is unknown. METHODS: We retrospectively studied subjects with MMN from 2 UK neuropathy centres. Raw and centile MMN-RODS scores were collected, at initial, two intermediate, and latest assessment, and distribution-based MCIDs determined at studied time-points. The sensitivities of raw, centile and individualised MCIDs, were compared. RESULTS: We included 32 consecutive subjects with MMN on individualised immunoglobulin dosing regime…

Minimally clinically important differences for the Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) following decompression surgery for lumbar spinal stenosis.

Decompression surgery is the standard treatment in lumbar spinal stenosis (LSS). Recent studies have shown that patient satisfaction following decompression surgery does not correspond well with outcomes measured by conventional patient-reported outcome measurements. Recent study reported that the Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) is the most accurate outcome measurement to reflect patient satisfaction. Nevertheless, the JOABPEQ was not studied enough, especially along with the minimally clinically important differences (MCIDs), as JOABPEQ is relativ

Pelvic floor distress inventory 20 and Wexner constipation scoring system responsiveness and minimal clinical importance difference in Persian women with functional constipation.

BACKGROUND: Functional constipation disproportionately affects women and impairs quality of life. Patient-reported outcome measures (PROMs) like the Wexner Constipation Scoring System (WCSS) and Pelvic Floor Distress Inventory-20 (PFDI-20) are widely recommended in clinical and research settings. However, responsiveness and minimal clinically important difference (MCID) after physiotherapy have not been validated in Persian speakers. This study aimed to evaluate these properties. METHODS: In this longitudinal study, 66 women with diagnosed functional constipation underwent 10 standardized phys…

The effect of risankizumab on achieving minimal clinically important differences in patient-reported outcomes in patients with psoriatic arthritis: results from KEEPsAKE 1 and 2.

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory disease that reduces the quality of life. This study assessed the effects of risankizumab (RZB) on the achievement of minimal clinically important differences (MCID) in patient-reported outcomes (PROs). METHODS: KEEPsAKE-1 and -2 are randomized, placebo-controlled Phase 3 clinical studies assessing RZB (150 mg) vs. placebo (PBO) in adult patients with PsA with inadequate response or intolerance to disease-modifying antirheumatic drugs and/or biologics. Patients were randomized 1:1 to receive RZB or PBO for 24 weeks; starting at W…

Measuring Clinical Treatment Response in Myasthenia Gravis.

In this article we provide an overview of health-related outcome measurement-to better understand what different outcomes used in myasthenia actually measure-and to provide some guidance when choosing measures based on the clinical context and question. In myasthenia, the most commonly used outcome measures are aimed at assessing the signs and symptoms. In this review, we provide a summary of the most commonly used outcome measures. We discuss instruments that gauge disease overall health impact, such as on disability and quality of life. Finally, we discuss other relevant outcomes such as ste

Hip and Knee Disability and Osteoarthritis Outcome Scores Minimal Clinically Important Differences Vary by Payer Type: Implications for Mandatory Patient-Reported Outcome Reporting.

BACKGROUND: Recent mandates from the Centers for Medicare and Medicaid Services have tied patient-reported outcome measure (PROM) metrics to hospital reimbursement for total hip arthroplasty (THA) and total knee arthroplasty (TKA). Concurrently, the proportion of patients meeting certain PROM metrics at each hospital will be publicly available. Threshold values such as the minimal clinically important difference (MCID) may vary across patient subpopulations. The Centers for Medicare and Medicaid Services proposal to apply blanket threshold values to all hospitals may skew results depending on …

From Data to Clinical Decisions in Chronic Wound Care: Evaluation of the Interpretability, Minimal Important Difference, and Smallest Detectable Change of the Wound-QoL-14 in a Spanish Population.

The Wound-QoL-14 questionnaire has proven to be a valid instrument for assessing patients with chronic wounds. However, cutoff scores are not established, and the smallest detectable change and the minimal important difference have not been defined. We aimed to assess the interpretability, smallest detectable change and the minimal important difference of the Wound-QoL-14 questionnaire in a Spanish population with chronic wounds. At baseline, patients responded to a 5-Likert global anchor question about how much their wounds had been a problem, and bands of severity were calculated. At a re-te

Minimally clinically important relative change instead of difference following surgery for degenerative cervical radiculopathy: a nationwide study on 5300 patients using prospectively collected data.

OBJECTIVE: The landscape of healthcare policies has undergone significant reforms, with a notable shift toward a patient-centered basis. This change has prioritized the use of metrics reflecting subjective patient-reported outcome measures (PROMs). However, these metrics are often limited, especially in terms of determining clinically relevant changes over time. The concept of the minimal clinically important difference (MCID) has been introduced to address this limitation. In this study, the authors aimed to assess the predictive capability of relative change and its corresponding threshold v…

Assessment of Mobility in Older People Hospitalized for Medical Illness Using the de Morton Mobility Index and Cumulated Ambulation Score-Validity and Minimal Clinical Important Difference.

BACKGROUND AND PURPOSE: Older adults acutely hospitalized for medical illness typically have comorbidity and disability, and in-hospital physical inactivity greatly increases the likelihood of developing new disability. Thus, assessment of the patients' mobility status is crucial for planning and carrying out targeted interventions that ensure mobilization during hospital admission. The aim of this study was to determine convergent validity, known group validity, floor and ceiling effects, and anchor-based minimal clinically important difference (MCID) of the more time-consuming de Morton Mobi…

Toward Incorporating Health-Related Quality of Life as Coprimary End Points in Clinical Trials: Time to Achieve Clinical Important Differences and QoL Profiles.

PURPOSE: Besides morbidity and mortality, quality of life (QoL) is a key outcome of cancer treatments. Trials on the basis of clinical outcomes have expectations that QoL outcomes can be either tolerated or improved. Simultaneously considering QoL and clinical outcomes is challenging with lack of suitable metrics allowing incorporation of QoL as coprimary end points in clinical trial design and utilization of hierarchical hypothesis testing. METHOD: We propose combining time to achieving a minimal clinically important difference (MCID) and probabilities of a MCID occurring in each QoL domain …
