Biologic Definition of Parkinson Disease: A Pivotal Role for Molecular Imaging.

In this article, we outline the ongoing conceptual transition of Parkinson disease (PD) from a clinically defined syndrome to a biologically defined disorder. We synthesize recent advances in neuropathology, genetics, and biomarker research, with particular emphasis on the central role of molecular imaging within emerging biology-based frameworks. We discuss how established imaging biomarkers-including dopaminergic imaging, [18F]FDG PET, and cardiac [123I]MIBG scintigraphy-provide objective in vivo measures of neurodegeneration that complement α-synuclein seed amplification assays and genetic …

Ultrasound molecular imaging of prostate cancer via PSMA-targeted biosynthetic GVs.

Purpose: Early diagnosis of prostate cancer is critical for improving prognosis, but current detection techniques face limitations such as l … could bind to prostate cancer cells with higher specificity, generating stronger and longer-lasting molecular imaging signals in tumors, which presented significant advantages over Con-GVs. Immunofluorescence confirmed that PSMA-GVs crossed the vascular wall, entered the peritumoral vascular space, bound to tumor cells, and enabled PSMA-targeted molecular imaging. …

Serial Molecular-Microstructural Multimodal Intravascular Imaging of Atherosclerotic Plaques in Mice.

Murine models are essential for cardiovascular translational research for developing new therapies to reduce the risk of heart attacks. However, assessment of murine atherosclerotic plaques by traditional histological methods fails to capture the entirety of plaque development across the length of a blood vessel over time. Here, the authors have developed and validated the first longitudinal molecular and microstructural intravascular imaging approach to study murine atherosclerosis. This approach combines a unique murine surgical model and a miniaturized multimodal intravascular imaging devic

Clinical translation of photoacoustic imaging using exogenous molecular contrast agents [Invited].

Photoacoustic imaging (PAI) combines optical contrast with acoustic detection to enable high-resolution, molecular imaging at clinically relevant depths. This review outlines the current status and future potential of contrast-enhanced PAI in human applications. We begin by discussing regulatory considerations surrounding both imaging devices and exogenous contrast agents, highlighting safety concerns, lack of standardized validation protocols, and barriers to the approval of novel agents. To accelerate clinical adoption, many studies have focused on repurposing FDA-approved agents such as ind…

MR molecular imaging for monitoring and predicting tumor responses to immunotherapy.

Background: While immunotherapies show great promise in cancer treatment, variability in patient responses warrant the need for improved methods to assess early responses and guide precision therapy. The tumor microenvironment (TME) plays a critical role in antitumor immunity and tumor response to immunotherapy. Critically, TME components and their crosstalk with immune cells can be leveraged as a prognostic marker for therapeutic response. This study evaluated the use of magnetic resonance molecular imaging (MRMI) targeting the TME protein extradomain B fibronectin (EDB-FN), which is a lympho…

Comprehensive linkage between molecular biology and imaging radiomics for thyroid nodules.

Thyroid nodules are common, with a prevalence of approximately 70% on thyroid ultrasonography; approximately 5% of these nodules are malignant. Distinguishing malignant and benign thyroid nodules is critical for clinical management. Clinicians can judiciously select patients for fine-needle aspiration, understand the cytology results and subsequent follow-up, and determine surveillance strategies for non-operated nodules. The challenge in selecting thyroid nodules for fine-needle aspiration is to avoid the diagnosis of small, clinically insignificant cancers without missing more severe disease

Molecular Imaging Probes for Early Detection and Staging of Liver Fibrosis.

Liver fibrosis, a progressive condition marked by excessive extracellular matrix deposition and activation of hepatic stellate cells, often develops asymptomatically in its early stages, leading to delayed clinical intervention. Conventional imaging techniques typically fail to detect mild fibrosis, resulting in diagnosis only at advanced stages such as cirrhosis, when therapeutic efficacy is significantly compromised. Recent advances in molecular imaging have facilitated the development of targeted contrast agents that enhance diagnostic sensitivity by selectively binding to fibrosis-specific…

Safirinium Fluorescent "Click" Molecular Probes: Synthesis, CuAAC Reactions, and Microscopic Imaging.

Fluorescent labeling utilizing Cu(I)-catalyzed azide-alkyne cycloaddition reactions (CuAAC) is among the leading applications of the "click" chemistry strategy. Fluorescent probes for this approach can be constructed by linking an azide or alkyne group to a fluorophore, such as the recently developed Safirinium derivatives. These compounds are water-soluble, highly fluorescent heterocycles based on 1,2,4-triazolium, with significant potential for various labeling applications, although they have not yet been converted to azide or alkyne probes. Herein, we report the synthesis of Safirinium-bas

FAPI-Targeted Molecular Imaging: Transforming Insights into Post-Ischemic Myocardial Remodeling?

Post-ischemic myocardial remodeling significantly impacts clinical outcomes after acute myocardial infarction (MI), involving structural and functional changes such as ventricular dilation, infarct wall thinning, and fibrosis development. These processes, driven by inflammatory cascades, neurohormonal activation, and extracellular matrix remodeling, result in impaired cardiac output and an increased risk of heart failure. Imaging with fibroblast activation protein inhibitors (FAPI) has emerged as a promising non-invasive tool for assessing myocardial fibrosis via positron emission tomography (

Molecular Imaging of Nitric Oxide Surrogates with Organelle-Specific Fluorescent Probes.

Nitric oxide is an important signalling molecule responsible for maintaining body's homeostasis. Any dysregulation in NO can lead to many pathological conditions like atherosclerosis, cancers, neurodegenerative disorders, hypertension and inflammation. Several, sensing technologies are used for sensing NO. Among these, fluorescent imaging is considered to be one of the most efficient. Till date, approximately 123 fluorescent probes are reported related to nitric oxide (NO) sensing fluorescent probes for the sensitive, selective, and real-time detection of NO at both the cellular and subcellula

Tailored Molecular Imaging of Pheochromocytoma and Paraganglioma: Which Tracer and When.

Pheochromocytoma (PCC) and paraganglioma (PGL) are rare neoplasms that fall within the category of neuroendocrine tumors. In the last decade, their diagnostic algorithm has been modified to include the evaluation of molecular pathways, genotype, and biochemical phenotype, in order to correctly interpret anatomical and functional imaging results and tailor the best therapeutic choices to patients. More specifically, the identification of germline mutations has led to a three-way cluster classification: pseudo-hypoxic cluster, cluster of kinase receptor signaling and protein translation pathways

Molecular Imaging of Chimeric Antigen Receptor T Cells by ICOS-ImmunoPET.

PURPOSE: Immunomonitoring of chimeric antigen receptor (CAR) T cells relies primarily on their quantification in the peripheral blood, which inadequately quantifies their biodistribution and activation status in the tissues. Noninvasive molecular imaging of CAR T cells by PET is a promising approach with the ability to provide spatial, temporal, and functional information. Reported strategies rely on the incorporation of reporter transgenes or ex vivo biolabeling, significantly limiting the application of CAR T-cell molecular imaging. In this study, we assessed the ability of antibody-based PE…
