Peroneal neuropathy after weight loss.

The objectives of this study were to evaluate the clinical and electrophysiological findings in peroneal mononeuropathies following a weight-reduction diet. Thirty patients with acute peroneal palsy and weight loss were studied. Complete nerve conduction studies (NCS) were performed in upper and lower limbs. NCS showed conduction block (CB) of the peroneal nerve at the fibular head that recovered in 29 patients within 3 weeks to 3 months. Severity of CB was correlated with clinical weakness. Three patients had abnormalities consistent with polyneuropathy (PNP). NCS in asymptomatic relatives co…

Neuropathy in the hemodialysis population: a review of neurophysiology referrals in a tertiary center.

This was a retrospective observational study of neurophysiology referrals over 8 years from a tertiary referral center in Ireland. A total of 68 of the 73 referrals yielded one or more abnormalities. Thirty-nine (53%) patients had one or more mononeuropathies; iatrogenic mononeuropathies believed to be associated with arterio-venous fistula creation occurred in 15 patients. Polyneuropathy was identified in 43 patients (59%). Access to an experienced neurophysiology department offers valuable insight into dialysis-associated neuropathies, especially when associated with arterio-venous fistulae.

Effective Management of Hypertensive Emergencies with Aliskiren Treatment in a Patient before and after Introducing Hemodialysis Secondary to Scleroderma Renal Crisis-like Condition under Corticosteroid Treatment for Sjögren Syndrome-associated Multiple Mononeuropathy.

A middle-aged woman presented with hypertensive emergency after corticosteroid treatment for Sjögren syndrome-associated multiple mononeuropathy with suspected systemic sclerosis. Hypertensive heart failure with hyperreninemia improved with antihypertensives, including aliskiren; however, she became hemodialysis-dependent. Clinical findings and biopsy-proven thrombotic microangiopathy indicated conditions resembling scleroderma renal crisis (SRC). Severe hypertension and heart failure with hyperreninemia occurred after stopping aliskiren for hypotension due to diverticular bleeding, which impr

Neuromuscular Ultrasound for Peripheral Neuropathies.

Neuromuscular ultrasound (NMUS) is a diagnostic technique that is now commonly used to complement electrodiagnostic studies when evaluating for suspected peripheral nerve pathology. The use of NMUS has been described in focal mononeuropathies, polyneuropathies, and motor neuron disease. This article provides a general overview of NMUS for the evaluation of peripheral neuropathies based on clinical practice guidelines, primary literature, and expert opinion. The characteristic changes detected by NMUS are described in focal mononeuropathies (including entrapment and other causes), polyneuropath…

Proximal neuropathy in Colles' fracture.

BACKGROUND: Peripheral nerve injury at the wrist following Colles' fracture is rare and usually located in the region of the fracture. Mononeuropathies in the proximal forearm have not been reported. METHOD: We present two patients with Colles' fracture with proximal forearm neuropathies. RESULTS: Both cases were associated with mononeuropathies in the forearm as proximal as the elbow, involving the median, ulnar and radial nerves in one, and the median and ulnar nerves in the other. CONCLUSION: Following Colles' fracture proximal nerve involvement may occur and, with increased awareness, t…

Sympathetic skin response. Glabella stimulation may be more useful than peripheral nerve stimulation in clinical practice.

The aim is to verify whether glabella electrical stimulation evokes sympathetic skin responses (SSR) without inter-side differences in latency and area of the responses and is more useful in mononeuropathies than peripheral nerve stimulation. SSRs were recorded in 25 healthy subjects from right palm, third (M3SSR) and fifth fingers and contralateral third finger. The inter-side differences of grand mean area and mean of largest area of M3SSR were significant only by ulnar nerve and not by glabella stimulation. Therefore glabella stimulation may be used in mononeuropathies comparing SSR area re…

High resolution neurography of the brachial plexus by 3 Tesla magnetic resonance imaging.

The study of the structures that make up the brachial plexus has benefited particularly from the high resolution images provided by 3T magnetic resonance scanners. The brachial plexus can have mononeuropathies or polyneuropathies. The mononeuropathies include traumatic injuries and trapping, such as occurs in thoracic outlet syndrome due to cervical ribs, prominent transverse apophyses, or tumors. The polyneuropathies include inflammatory processes, in particular chronic inflammatory demyelinating polyneuropathy, Parsonage-Turner syndrome, granulomatous diseases, and radiation neuropathy. Gasoline sniffing multifocal neuropathy.

The polyneuropathy caused by chronic gasoline inhalation is reported to be a gradually progressive, symmetric, sensorimotor polyneuropathy. We report unleaded gasoline sniffing by a female 14 years of age that precipitated peripheral neuropathy. In contrast with the previously reported presentation of peripheral neuropathy in gasoline inhalation, our patient developed multiple mononeuropathies superimposed on a background of sensorimotor polyneuropathy. The patient illustrates that gasoline sniffing neuropathy may present with acute multiple mononeuropathies resembling mononeuritis multiplex, …

Peripheral nervous system involvement as the only neurological manifestation of infective endocarditis.

We report a case of acute endocarditis with the unusual presentation of mononeuritis multiplex at onset. Mononeuropathy was the only detected neurological sign. The presence of muscular abscesses compressing the peripheral nervous system could lie at the basis of the pathophysiological mechanism responsible for acute mononeuropathy.

Upper limb nerve entrapments in elite wheelchair racers.

The prevalence of upper limb nerve injuries has been reported to be as high as 73% in individuals who rely on manual wheelchairs for mobility. Many authors hypothesize that the repetitive trauma to carpal canal structures caused by propelling a wheelchair is the reason for this high prevalence. The purpose of this study was to determine the prevalence of nerve conduction abnormalities in a group of elite wheelchair racers whose wrists are exposed to additional propulsion-related trauma during training and competition. We performed bilateral upper limb nerve conduction studies on each athlete (

Other median neuropathies.

Median mononeuropathy is common, with carpal tunnel syndrome the most frequently encountered acquired mononeuropathy in clinical practice. However, other disorders of the median nerve and many known anatomical variants can lead to misdiagnosis and unexpected surgical complications if their presence is not correctly identified. A number of inherited and acquired disorders can affect the median nerve proximal to the wrist, alone or accompanied by other affected peripheral nerves. Recognizing other disorders that can masquerade as median mononeuropathies can avoid misdiagnosis and misguided manag…

Referral diagnosis versus electroneurophysiological finding. Two years electroneuromyographic consultation in a rehabilitation clinic.

This study describes the referral practice to an electrodiagnostic unit in a rehabilitation clinic and compares the provisional diagnoses of different referring sources with the outcome of the neurophysiological tests. The study comprises data of 495 patients which were examined over a two years period. General practitioners, the author, physiatrists and surgeons, followed by occupational health physicians and neurologists used the electrodiagnostic service. Two thirds of the cases were referred to rule out upper limb mononeuropathies or cervical root and plexus neuropathies. Carpal tunnel syn…

Sparing of the second lumbrical in a Riche-Cannieu anastomosis: the nearly all-ulnar hand.

The Riche-Cannieu anastomosis (RCA) is an anatomic variant wherein a deep branch of the distal ulnar nerve innervates muscles of the thenar eminence. A few cases in the literature describe an "all-ulnar hand" when the muscles of the thenar eminence get innervation solely from the ulnar nerve without any contribution from the median nerve. Clinically, patients with ulnar mononeuropathies in the setting of an RCA would also have weakness/atrophy of thenar muscles. Conversely, patients with median mononeuropathies would have sparing of these muscles. This case highlights RCA in the setting of car…
