End-of-life decisions for children can be complicated by disagreements between families and health care teams. These conflicts can lead to moral distress in providers. In addition, difficulties in prognostication aggravate the problem. How teams and institutions address potential staff distress is essential to providing effective palliative care for children. Through a case study of a child with a severe life-limiting syndrome, an analysis of both the ethical and legal implications of parental and team conflict are discussed. An ethics team can help provide guidance for teams and help mediate

Many factors contribute to homelessness, including extreme poverty, extended periods of unemployment, disruption of regular sources of income and employment, deinstitutionalization and substance abuse. As a result, the needs of the homeless are both broad and complex. This assessment is based on literature reviews and reviews of local documents and reports. For homeless people, healthcare competes with more immediate needs, such as obtaining adequate food and shelter and it is our duty as a society to help.

The transplant system has been criticized for not paying enough attention to efficiency in distributing scarce organs. But little research has been done to see how the general public views tradeoffs between efficiency and equity. We surveyed members of the general public to see how they would distribute organs among patients with varying chances of benefiting from them. In addition, we asked subjects to explain their decisions and to tell us about any other information they would have liked in order to make the decisions. We found that the public places a very high value on giving everyone a c

This two-part paper discusses moral and ethical questions raised by future trials of human fetal gene therapy. The first part examines broad moral issues to explore whether fetal gene therapy is a morally praiseworthy goal. Ought it be done at all? These issues include (i) how the concept of fetal gene therapy originally arose as a goal envisioned at the beginning of prenatal diagnosis, (ii) preimplantation genetic diagnosis as a better preconceptual alternative for parents at higher genetic risk, (iii) alternatives to genetic abortions, (iv) the social and economic priority of fetal gene ther

The paper opens with a discussion about the recognition of "whole-brain death" as the end of life in North America in order to perform solid organ transplants. This situation is contrasted with Japan, where, despite no financial or technological restrictions, brain death is not recognized, and transplants from brain-dead bodies cannot be performed. The Japanese cultural debate over the past twenty-five years about the "brain-death problem" is presented, followed by an analysis of Japanese attitudes towards technological intervention into what is taken to be the "natural" domain, together with

For many persons, 'Holocaust-abomination' is a fixed point on their moral compass: if anything can be evil, it was. Yet at least one of the justifications deployed by its perpetrators (the eugenics argument) involves widely-held values concerning human health and procreation. Hence persons endorsing many current activities based on those values (e.g. genetic counselling) have been charged with being on a morally deplorable slippery slope. This paper sketches the necessary structure of a moral position capable of consistently embracing those values without placing its occupants on that slippery

Infants with Down syndrome often present with a familiar orofacial disorder which exists at birth or becomes more pronounced by the end of the first year. The primary pathology includes hypotonicity of the perioral muscles, lips, and masticatory muscles and a protruding tongue, later followed by active tongue protrusion. This results in problems with sucking, swallowing, drooling and dentition. Early intervention methods employing the combination of Castillo-Morales Manual Orofacial Therapy and his specially designed palatal plate, can improve orofacial function, facial appearance and prevent

Background: Moral injury emerges when someone perpetrates, fails to prevent, or witnesses acts that violate their own moral or ethical code. Nash et al. [(2013). Psychometric evaluation of the moral injury events scale. Military Medicine, 178(6), 646-652] developed a short measure, the Moral Injury Events Scale (MIES) to facilitate the empirical study of moral injury in the military. Our study aimed to develop a civilian version of the measure (MIES-CV) and examine its psychometric properties in a sample of psychiatric inpatients .Methods: In this cross-sectional study, the sample comprised 24

PURPOSE: Oncology nurses face significant emotional and ethical challenges, particularly in delivering palliative care. While patient-centered communication (PCC) is essential for high-quality end-of-life care, its effectiveness may be hindered by moral distress-a psychological response to institutional or ethical constraints. This study examines how moral distress mediates the relationship between patient-centered communication and palliative care competence among oncology nurses.
METHODS: A cross-sectional survey was conducted with 200 oncology nurses from four oncology centers across Egypt

Background: Healthcare workers (HCWs) across the globe have reported symptoms of Post-Traumatic Stress Disorder (PTSD) during the COVID-19 pandemic. Moral Injury (MI) has been associated with PTSD in military populations, but is not well studied in healthcare contexts. Moral Distress (MD), a related concept, may enhance understandings of MI and its relation to PTSD among HCWs. This study examined the independent and combined impact of MI and MD on PTSD symptoms in Canadian HCWs during the pandemic. Methods: HCWs participated in an online survey between February and December 2021, with questions

AIMS: To generate a taxonomy of potentially morally injurious events (PMIE) encountered in veterinary care and develop an instrument to measure moral distress and posttraumatic growth following exposure to PMIE in the veterinary population.
METHODS: Development and preliminary evaluation of the Moral Distress-Posttraumatic Growth Scale for Veterinary Professionals (MD-PTG-VP) employed data from veterinary professionals (veterinarians, veterinary nurses, veterinary technicians) from Australia and New Zealand across three phases: (1) item generation, (2) content validation, and (3) construct va

Since January 2025, several executive orders have been issued that aim to dismantle diversity, equity, and inclusion (DEI) initiatives in public and private institutions and national medical education accreditation agencies. Many of the DEI initiatives were developed to address existing healthcare inequities, and as a result, the executive orders have had a profound impact on physicians, patients, and medical educators. Physicians may be experiencing moral distress due to the conflict between their ethical responsibility to patients and learners and institutional compliance with anti-DEI direc

BackgroundNurses frequently encounter ethical dilemmas that, when they are unable to act in accordance with perceived ethical standards, may lead to moral distress. Moral distress is associated with a wide range of adverse psychological, physical, and professional outcomes, and its development is influenced by multiple individual, team-related, and organizational factors. Although various interventions have been proposed to reduce moral distress, the existing evidence regarding their effectiveness remains fragmented and heterogeneous.Research question/aim/objectivesThis systematic review aimed

BackgroundChina's rapidly aging population and the implementation of long-term care insurance have increased the demand for long-term care givers (LTCGs), a certified non-nursing workforce with both practical and ethical responsibilities. Research on moral sensitivity has predominantly focused on nurses, leaving the experiences of LTCGs underexplored, particularly through qualitative inquiry.Research aimThis study characterizes how moral sensitivity is experienced and enacted by certified Chinese LTCGs and examines how the provisional dimensions of moral duty, moral burden, and moral strength

The Psychiatric Consultation Service at Massachusetts General Hospital sees medical and surgical inpatients with comorbid psychiatric symptoms and conditions. During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. These discussions have given rise to rounds reports that will prove useful for clinicians practicing at the interface of medicine and psychiatry.
