Finding benefit in stressful uncertain circumstances: relations to social support and stigma among women with unexplained illnesses.

Living with a chronic illness can be challenging, but the ability to derive benefits and grow from this experience may enhance well-being. However, the possibility of obtaining such benefits may be dependent on the levels of stigmatization and lack of social support experienced by an individual as a result of the illness. Chronic fatigue syndrome (CFS) and fibromyalgia are chronic conditions that remain largely unexplained and those with these conditions must often contend with stigma and skepticism from others. Individuals with CFS/fibromyalgia often display stress-related biological alterati

Prevalence and patterns of multiple long-term conditions among lymphatic filariasis patients in Odisha, India: a community-based cross-sectional study.

INTRODUCTION: Neglected tropical diseases (NTDs), such as lymphatic filariasis, mimic other chronic conditions and share common risk behaviors with parallel health system issues that require unique interventions. Multiple long-term conditions (MLTCs or multimorbidity), defined as two or more chronic conditions has become increasingly common in low- and middle-income countries (LMICs) such as India. However, data on the prevalence of the interface between NTDs and other chronic conditions are lacking. We estimated the prevalence and correlates of MLTC, assessed the commonly occurring patterns,

The Epidemiological Landscape and Burden of Chronic Disease in Australia: Contextualizing the Distinct Profile of Tasmania.

Chronic diseases are the leading causes of morbidity and mortality worldwide and impose a substantial and long-lasting burden on individuals, families, communities, and healthcare systems. In Australia, more than 47% of the population had at least one chronic condition in 2018, and the prevalence rose sharply with age: approximately 80% of adults aged 65 and over lived with one or more chronic diseases. These conditions are complex and driven by multiple factors, with modifiable behavioral risks such as poor diet, physical inactivity, tobacco use, and harmful alcohol consumption playing a majo

"There's no one-size-fits-all kind of solution": An interpretative phenomenological analysis of the experiences of autistic individuals living with Ehlers-Danlos syndrome.

OBJECTIVE: Approximately a third of individuals live with multiple health conditions and this number is rising. Research suggests that living with a chronic condition can profoundly impact upon one's life and identity, however little attention has been paid to the experiences of those with multiple conditions. Ehlers-Danlos syndrome (EDS) is a rarely-diagnosed connective tissue disorder causing extensive debilitating symptoms and while these symptoms are primarily physical, EDS often co-occurs with autism. This study sought to gain insight into the experience of autistic individuals living wit

Autologous hematopoietic stem cell transplantation in multiple sclerosis: a phase II trial.

OBJECTIVE: To assess in multiple sclerosis (MS) the effect of intense immunosuppression followed by autologous hematopoietic stem cells transplantation (AHSCT) vs mitoxantrone (MTX) on disease activity measured by MRI. METHODS: We conducted a multicenter, phase II, randomized trial including patients with secondary progressive or relapsing-remitting MS, with a documented increase in the last year on the Expanded Disability Status Scale, in spite of conventional therapy, and presence of one or more gadolinium-enhancing (Gd+) areas. Patients were randomized to receive intense immunosuppression

Survival and mortality in adults with cancer and pre-existing mental illness across multimorbidity types: a decade of population-based data from Alberta, Canada.

BACKGROUND: Adults living with cancer and pre-existing mental illness (PMI) represent a particularly vulnerable population. This study examined whether multimorbidity protects against or increases mortality among adults with cancer and PMI. METHODS: We used Alberta Health Services administrative data (n = 15,144), to identify adults with both PMI and cancer diagnosed between 2013-2022, including demographic, comorbidity, and cancer information. The analysis used Cox proportional hazard models (HR) to evaluate the impact among different multimorbidity categories on all-cause mortality. The mul

Health-related quality of life and preferred health-seeking institutions among rural elderly individuals with and without chronic conditions: a population-based study in Guangdong Province, China.

The aim of this study was to examine health-related quality of life (HRQL) as measured by SF-36 and to identify these factors and the preferred health-seeking institutions of 12,800 persons aged 60 and older with and without chronic conditions in rural areas of Guangdong Province by multistage stratified cluster sampling method. HRQL among rural elderly subjects with chronic conditions was lower than that of elderly subjects with no chronic conditions. Multiple linear regression showed that marital status, living with children, cardiovascular disease, osteoporosis, cataract disease, and mental

Neuro-Behcet's disease misdiagnosed and treated as multiple sclerosis: a deceiving masquerader.

Behcet's disease is a chronic polysymptomatic systemic vasculitis disorder of unknown etiology characterized by several clinical manifestations in multiple organ systems. Involvement of the nervous system occurs in ∼9% of patients with Behcet's disease (ranging from 3 to 30%). Neuro-Behcet's disease is a great masquerader of multiple sclerosis. Diagnosing this disorder might be challenging, especially in a patient who does not fulfill the criteria of Behcet's disease while having a neurological presentation. We report a case of neuro-Behcet's disease who was misdiagnosed as having multiple scl

What makes a prognostic biomarker in CNS diseases: strategies for targeted biomarker discovery? Part 2: chronic progressive and relapsing disease.

INTRODUCTION: Compared with acute neurological conditions, biomarker discovery in chronic progressive and relapsing neurodegenerative diseases may be more difficult. First, the degenerative process may be so slow that the biomarker discovered is released only in small quantities making it a fishing exercise with very little to catch. Second, it may be difficult to differentiate acute new damage, relevant to prognostic estimates, from already existing background damage. A potential advantage in chronic conditions is that there may be sufficient time for the disease to leave a specific signature

Posturographic profile of patients with persistent postural-perceptual dizziness on the sensory organization test.

BACKGROUND/OBJECTIVE: Persistent postural-perceptual dizziness (PPPD) was defined by expert consensus for the forthcoming International Classification of Diseases, 11th edition. Its diagnostic criteria were derived from phobic postural vertigo and chronic subjective dizziness. Two key symptoms are postural unsteadiness and visually induced dizziness. We observed that patients with PPPD tended to perform poorly on multiple conditions of the EquiTest® Sensory Organization Test (SOT) and sought to investigate this phenomenon systematically. METHODS: We examined cross-sectional, pair-wise group d

Exploring reasons for state-level variation in incidence of dialysis-requiring acute kidney injury (AKI-D) in the United States.

BACKGROUND: There is considerable state-level variation in the incidence of dialysis-requiring acute kidney injury (AKI-D). However, little is known about reasons for this geographic variation. METHODS: National cross-sectional state-level ecological study based on State Inpatient Databases (SID) and the Behavioral Risk Factor Surveillance System (BRFSS) in 2011. We analyzed 18 states and six chronic health conditions (diabetes mellitus [diabetes], hypertension, chronic kidney disease [CKD], arteriosclerotic heart disease [ASHD], cancer (excluding skin cancer), and chronic obstructive pulmona

The Immunological Intersection of Psoriasis and Autoimmune Diseases: Shared Pathways and Clinical Implications.

Psoriasis is a chronic inflammatory disease associated with multiple autoimmune conditions. This article reviews the relationship between psoriasis and autoimmune blistering dermatoses, vitiligo, alopecia areata, autoimmune thyroid disorders, systemic lupus erythematosus, and celiac disease, highlighting shared genetic susceptibility and overlapping immune pathways. The coexistence of these conditions may influence clinical presentations, prognosis, and treatment selection. In addition, various psoriasis therapies have been associated with the onset or exacerbation of concomitant autoimmune di

Impact of Multimorbidity Subgroups on the Health Care Use and Clinical Outcomes of Patients With Tuberculosis: A Population-Based Cohort Analysis.

Background: Multimorbidity is defined as the existence of two or more chronic health conditions in the same individual. While patients with tuberculosis commonly have multiple conditions at diagnosis, such as HIV, diabetes, and depression, to the authors' knowledge, there is limited information on the patterns of multimorbidity, and how the types and combinations of conditions could impact the healthcare utilization, expenditure, and TB outcomes. Methods: An observational cohort study of adult patients diagnosed with tuberculosis was conducted using the Chinese Center for Disease Control and P

[Mechanisms of neural circuit remodeling in the primary somatosensory cortex in mouse models of chronic pain].

Chronic pain is caused by injury or inflammation of the peripheral tissues and by abnormal activities in the central nervous system including the spinal cord area related to generation and maintenance of chronic pain. In the cerebral cortex, several areas respond to noxious stimulation. Since activities of these cortical areas are enhanced in chronic pain conditions, not only a single cortical area, but also the interaction between multiple cortical areas seems to contribute to chronic pain. The primary somatosensory cortex (S1), which is one of the pain-related cortical areas, plays a role in

New developments in the diagnosis and treatment of chronic prostatitis/chronic pelvic pain syndrome.

PURPOSE OF REVIEW: To describe new developments in the diagnosis and treatment of chronic prostatitis/chronic pelvic pain syndrome (CPPS). RECENT FINDINGS: Symptoms in men with chronic prostatitis/CPPS appear to cluster into a group with primarily pelvic or localized disease, and a group with more systemic symptoms. Several other chronic pain conditions can be associated with chronic prostatitis/CPPS, including irritable bowel syndrome, fibromyalgia, and chronic fatigue syndrome. Markers of neurologic inflammation and autoimmune disease parallel changes in symptoms after treatment. Treatment

Implementation of a model integrating primary and oncology pharmacists' care for patients taking oral anticancer agents (OAA).

Improvements in chronic myeloid leukemia (CML), chronic lymphocytic leukemia (CLL), and multiple myeloma (MM) treatment options have increased the 5-year survival rates for patients with these hematologic malignancies. In addition to cancer management, these patients may need help to manage multiple chronic conditions (MCC). The overall objective of this study is to examine the impact and implementation of a model that coordinates care between oncology and primary care pharmacists for people taking an oral anti-cancer agent (OAAs) and medications for comorbid chronic conditions. This is a mult…
