[Muscle involvement of Stormorken's syndrome].

We described two patients, a mother and daughter, of Stormorken's syndrome. The syndrome is characterized clinically by autosomal dominant inheritance, congenital miosis, thrombocytopenia, asplenia and muscle weakness. Both patients had bleeding tendency, ichthyosis of arms, and muscle weakness. The daughter additionally had short stature (146 cm), low body weight (32 kg) and muscle cramp. Neurological findings of the patients included migraine-like headache, cognitive dysfunction, limitation of upward and lateral gaze, and amydriasis. Femoral muscle MRI of the daughter demonstrated decreased …

Idiopathic generalized myokymia.

Idiopathic generalized myokymia (IGM) is a rare, heterogeneous, and poorly understood syndrome. We present analysis of 75 reported cases in the world literature. IGM affects men and women equally, with a mean age of onset 29 +/- 19 years. Patients' common presenting complaints are stiffness (60%), cramps (12%), weakness (12%), and muscle twitching (4%). Family history is positive in 30%. In addition to generalized clinical myokymia (92%), abnormal neurologic findings include: hyporeflexia (70%), weakness (45%), grip myotonia (39%), and calf hypertrophy (16%). Electrical activity consisting of

Disturbed sensorimotor processing during control of precision grip in patients with writer's cramp.

The aim of the study was to investigate force regulation in patients with writer's cramp when performing a drawer-opening task using the precision grip. Experimental conditions included intervening load pulses and vibratory manipulations for examining grip force responses to sensory disturbances. The data revealed that grip force was increased in patients with writer's cramp compared with normal subjects, with a stronger modulation in the symptomatic compared with the asymptomatic hand. This denotes a change in force scaling capabilities and most notably for the preferred hand used in manipula

Dystonia as a presenting feature of the 3243 mitochondrial DNA mutation.

A variety of neurologic phenotypes have been described in patients with mitochondrial disorders. We report a 32-year-old man in whom dystonia was the salient and presenting feature of a mitochondrial DNA mutation. He presented at age 23 with writer's cramp and progressed over 5 years to exhibit dystonia in facial muscles and lower limbs. He also has exercise intolerance, mild, bilateral ptosis, proximal muscle weakness, and sensorineural hearing loss. Molecular genetic analysis of blood, urine, and muscle biopsy demonstrated the presence of a heteroplasmic point mutation at nucleotide position

Sensorimotor reorganization by proprioceptive training in musician's dystonia and writer's cramp.

OBJECTIVE: The sensorimotor organization (SMO) of the motor hand area is abnormal in focal hand dystonia and likely contributes to symptom manifestation. In healthy subjects SMO is changed by training with proprioceptive stimulation. Here we test whether similar interventions reverse the abnormal SMO in musician's dystonia and writer's cramp. If so, they could be developed for therapeutic application. METHODS: In six non-musicians, six professional musicians, six patients with musician's dystonia, and six patients with writer's cramp, SMO was explored by measuring changes in short-interval-in

The two sides of associative plasticity in writer's cramp.

Neuronal plasticity is to be kept within operational limits to serve its purpose as a safe memory system that shapes and focuses sensory and motor representations. Temporal and spatial properties of motor cortical plasticity were assessed in patients with writer's cramp using a model of long-term potentiation (LTP) and long-term depression (LTD) of synaptic efficacy. Paired associative stimulation (PAS) combined repetitive electric stimulation of the median or ulnar nerve (MN or UN) with subsequent transcranial magnetic stimulation of the contralateral dominant motor cortex at 21.5 ms (MN-PAS2

Spastic syndrome in a Holstein bull: a histologic study.

A 4-year-old Canadian holstein bull developed the spastic syndrome, an episodic but progressive disorder causing pelvic limb muscular spasms. A post-mortem study, including morphometry of skeletal muscles and teased peripheral nerve fibers of the pelvic limb, revealed mild type II skeletal muscle fiber atrophy and minimal, focal segmental demyelination with remyelination, and axonal degeneration in peripheral nerves. Such alterations are probably incidental or age-associated. Idiopathic muscular cramps is the most probable explanation of the clinical disease and is consistent with the absence

Symptom in search of a toxin: muscle spasms following bites by Old World tarantula spiders (Lampropelma nigerrimum, Pterinochilus murinus, Poecilotheria regalis) with review.

BACKGROUND: Tarantula spiders are widely kept and bred in captivity by both adults and children. Their bites are generally considered harmless. AIM: To explore the effects of envenoming by Old World tarantulas. DESIGN AND METHODS: Clinical studies and review of conventional literature and hobbyist web sites. RESULTS: Two men bitten on their index fingers by pet Old World tarantula spiders, Lampropelma nigerrimum (Ornithoctoninae) and Pterinochilus murinus (Harpactirinae) in England, developed intense local pain, swelling and episodic, agonising, generalised muscle cramps. In one of them, cr…

Symptom network analysis in maintenance hemodialysis patients: a multicenter study.

OBJECTIVE: To identify symptom clusters and construct a symptom network in multicenter maintenance hemodialysis (MHD) patients, with the aim … ymptoms, uremic toxins symptoms, and sleep disturbance symptoms. Network analysis demonstrated that muscle cramps exhibited the highest strength centrality (1.758), followed by nervousness (1.332) and worrying (1.238), identifying muscle cramps as the most central symptom within the network structure. The correlation stability (CS) coefficient of strength cent …

A Systematic Review of Management of Cramping Pain in Patients with Amyotrophic Lateral Sclerosis.

INTRODUCTION: Pain, particularly cramping, in people living with amyotrophic lateral sclerosis (ALS) is often underrecognized and under-treated. Despite affecting over 70% of people living with ALS (plwALS), cramping pain remains inadequately managed due to its complex nature and the difficulties plwALS face in communicating their symptoms as the disease progresses. This systematic review explores both pharmacological and non-pharmacological treatments for cramping pain in ALS, aiming to assess and compare their efficacy. METHODS: The systematic review was conducted following PRISMA guideline
