Exercise-induced ventricular tachycardia in association with a myocardial bridge.

Sustained ventricular tachycardia induced by exercise is uncommon. This is a report of a patient who has a positive exercise test at a high level of exercise. The electrocardiogram has classic ischemic ST depressions. Following the appearance of these ST depressions, the patient developed ventricular tachycardia at a rapid rate. Workup that included an echocardiogram and cardiac catheterization revealed myocardial bridging of the left anterior descending artery as the only structural abnormality. Electrophysiologic studies revealed the patient to have ventricular tachycardia only with isoprote…

Morphometric analysis of myocardial bridges in children with ventricular hypertrophy.

The nuclear cross-sectional area of the muscular fibers from a series of myocardial bridges was analyzed and compared to both the adjacent and underlying myocardial fibers. The series came from 12 hearts of children, ranging in age from 24 h to 2 years, with diverse forms of congenital heart disease, which caused left ventricular hypertrophy and had a myocardial bridge in the path of the anterior interventricular coronary artery. The cross-sectional areas of myocardial nuclei were measured for the bridging myocardial fibers, the adjacent myocardium, and the myocardial fibers underlying the bri

Myocardial infarction with distal embolization associated with myocardial bridging in a postoperative patient: a case report.

Myocardial bridging is associated with myocardial infarction (MI) on rare occasions. The mechanism by which this occurs is unclear; vessel occlusion in this setting may be due to superimposed thrombosis or vasospasm. An unusual case is presented of postoperative MI from thrombosis associated with myocardial bridging and intravascular volume depletion, with subsequent distal embolization. Endothelial injury as a result of myocardial bridging may predispose to thrombus formation. To avoid MI, adequate fluid hydration is recommended during the perioperative period in patients with myocardial brid…

Right coronary artery distribution of myocardial bridging: an unusual case presenting with ST-Elevation myocardial infarction.

Myocardial bridging has been observed to provoke symptoms in association with strenuous physical activity and hypertrophic cardiomyopathy; more recently, bridging has been thought to cause coronary vasospasm. Autopsy findings tend to reveal higher incidences of bridging than do angiography series, but both show an almost uniform predilection for left coronary artery distribution--especially in regard to bridging that manifests itself clinically. Herein, we report an unusual case of myocardial bridging with right coronary artery distribution and important clinical implications for physicians' m…

Exertional Syncope Caused by Hemodynamically Significant Myocardial Bridging With Coronary Vasospasm.

BACKGROUND: Myocardial bridging (MB) may cause myocardial ischemia through impaired diastolic coronary perfusion and coronary vasomotor dysfunction, occasionally presenting as exertional presyncope or syncope. CASE SUMMARY: We report 2 patients with exertional presyncope/syncope caused by hemodynamically significant MB and concomitant epicardial coronary spasm. Coronary angiography showed no obstructive disease. Acetylcholine provocation induced diffuse spasm in the mid- to distal left anterior descending artery, whereas intracoronary nitroglycerine unmasked systolic compression of a mid-left…

Clinically asymptomatic myocardial bridging in a child with familial subaortic stenosis.

Myocardial bridging is usually seen in the setting of hypertrophic cardiomyopathy or left ventricular hypertrophy. It is rarely reported in an asymptomatic patient with an otherwise structurally normal heart. Familial subaortic stenosis is also a rare entity, and its mode of inheritance is still unknown. Here, we described the case of a 13-year-old asymptomatic girl with a positive family history of sudden cardiac death and subaortic stenosis who was diagnosed with severe myocardial bridging concomitant with familial subaortic stenosis.

Atherosclerotic pattern of coronary myocardial bridging assessed with CT coronary angiography.

The aim of our study was to evaluate the atherosclerotic pattern of patients with coronary myocardial bridging (MB) by means of CT Coronary Angiography (CT-CA). 254 consecutive patients (166 male, mean age 58.6 ± 10.3) who underwent 64-slice CT-CA according to current clinical indications were reviewed for the presence of MB and concomitant segmental atherosclerotic pattern. Coronary plaques were assessed in all patients enrolled. 73 patients (29%) presented single (90%) or multiple (10%) MB, frequently (93%) localized in the mid-distal left anterior descending artery. The MB segment was alway…

Myocardial bridging in Taiwan: depiction by multidetector computed tomography coronary angiography.

BACKGROUND/PURPOSE: Myocardial bridging (MB) is a condition in which a segment of the major epicardial coronary artery is tunneled within and surrounded by the myocardium. This condition has been linked to severe complications. The aim of this study was to evaluate the incidence of MB in Taiwanese subjects examined with electrocardiogram-gated, 16-slice, multidetector computed tomography (MDCT) coronary angiography, as well as to determine the location, depth, and length of the bridged segments and the concomitant atherosclerosis of MB. METHODS: From August 2004 to May 2005, 276 consecutive s…

Myocardial bridging, a common anatomical variant rather than a congenital anomaly.

The recent introduction of coronary computed tomographic angiography into clinical use as a noninvasive tool for coronary artery evaluation has evoked a new interest in myocardial bridging. Recent studies using multidetector computed tomography for the evaluation of coronary anatomy have shown myocardial bridging to be more prevalent than previously reported in angiographic series. The purpose of this article was to provide an overview of the current knowledge on the prevalence and features of myocardial bridging as shown by multidetector computed tomography and to discuss the potential clinic…

Detection of myocardial bridging with ECG-gated MDCT and multiplanar reconstruction.

OBJECTIVE: The aim of this study was to evaluate the incidence of myocardial bridging in 626 patients examined with MDCT angiography of the coronary arteries. MATERIALS AND METHODS: Six hundred twenty-six patients who were referred to Florence Nightingale and Atatürk University Hospitals were involved in this study. These patients had atypical chest pain, symptoms suggestive of coronary artery disease, or no significant cardiac complaint. Patients were in sinus rhythm and were premedicated with metoprolol tartrate (5 mg/mL IV bolus) to decrease the heart rate and nitroglycerin (5 mg sublingua…

Surgical repair of Wolff-Parkinson-White syndrome complicated with myocardial bridging.

Myocardial bridging causes myocardial ischemia during supraventricular tachycardia. We present a case of Wolff-Parkinson-White syndrome combined with myocardial bridging. The patient complained of angina pectoris during paroxysmal supraventricular tachycardia because of severe constriction of the left anterior descending coronary artery during systole. A myocardial scintigram revealed myocardial ischemia in the anteroseptal wall during paroxysmal supraventricular tachycardia. Myotomy to prevent myocardial bridging and interruption of the accessory conduction pathway was successfully accomplish…

Extracorporeal membrane oxygenation as a "bridge to recovery" in a case of myotomy for myocardial bridge complicated by biventricular dysfunction.

The incidence of cardiac dysfunction after routine cardiac surgical procedures is quite high (3-5%), but the majority of patients improve using inotropic drugs or intraaortic balloon counterpulsation. However, approximately 1% of these patients do not benefit from using these supports, and they need more invasive strategies, such as ventricular assist devices. Extracorporeal membrane oxygenation (ECMO) is one of them, and it offers biventricular support, can be managed very easily and is one of the cheapest devices. We describe our experience with ECMO in a case of postocardiotomy failure afte

Stress cardiomyopathy in a patient with hypertrophic cardiomyopathy and myocardial bridging.

Stress cardiomyopathy is an acquired cardiomyopathy of unknown aetiology. It usually occurs in women over the age of 70 who have experienced physical or emotional stress. It is most commonly characterised by a transient, left ventricular systolic dysfunction in the apical portion and hyperkinesia in the basal segments, without obstructive coronary artery disease. Its association with obstructive hypertrophic cardiomyopathy and myocardial bridging is rare. Herein, we present such a case.

Multimodality Imaging in the Assessment of the Physiological Significance of Myocardial Bridging.

In myocardial bridging (MB) a segment of the coronary artery is covered by the myocardium. MB can be seen as a systolic compression by invasive coronary angiography (ICA) or as an intramural course by computed tomography angiography (CTA). Intramural course is a common incidental finding in CTA studies. Only minority of the bridging segments are associated with systolic compression causing a possible impairment of myocardial perfusion. The relationship between myocardial blood flow and MB is complex and poorly evaluated by anatomic imaging. Furthermore, provocation tests are frequently needed …
