Morphine intake and the effects of naltrexone and buprenorphine on the acquisition of methamphetamine intake.

Some common genetic factors appear to influence risk for drug dependence across multiple drugs of abuse. In previous research, mice that wer … compared morphine to quinine consumption and another that used a saccharin fading procedure. Next, naltrexone (0, 0.5, 1, 2, 5, 10 and 20 mg/kg), a μ-opioid receptor antagonist, and buprenorphine (0, 1, 2 or 4 mg/kg), a μ-opioid receptor partial agonist, were each examined for their effects on the acquisition of methamphetamine consumption. Low methamphetam …

Treatment of refractory vulvovaginal pruritus with naltrexone, a specific opiate antagonist.

OBJECTIVE: Chronic vulvovaginal pruritus can be refractory to standard treatment. Since opioids can induce itching and opioid receptor antagonists have been shown to suppress pruritus of different etiologies, we applied this treatment to patients with vulvovaginal pruritus refractory to conventional therapies. STUDY DESIGN: Five women between 24 and 54 years of age suffering from chronic vulvovaginal pruritus were treated with 50mg naltrexone orally once a day for 3 weeks. They documented itching episodes and possible side-effects. RESULTS: After 1 week of treatment the number of reported it…

Opioid antagonist naltrexone for the treatment of pathological gambling in Parkinson disease.

UNLABELLED: Pathological gambling (PG) is a potential complication related to the treatment of Parkinson disease (PD) with dopamine agonists (DA). The cause of this disorder is unknown, but altered dopamine neurotransmission may be involved. OBJECTIVE: We evaluated the efficacy and tolerability of the opioid antagonist naltrexone in the treatment of PG in PD. METHODS: Our cases included 3 patients with PD who developed PG after DA treatment. RESULTS: Pathological gambling did not improve after reduction or discontinuation of DA. These patients responded poorly to serotonin reuptake inhibito…

Predictors for the efficacy of naltrexone treatment in alcohol dependence: sweet preference.

AIMS: To analyse the possible associations between sweet preference and the efficacy of naltrexone treatment of alcohol dependence. METHODS: The preference for different concentrations of sucrose was evaluated in 78 participants diagnosed with alcohol dependence after treatment for 32 weeks with naltrexone or placebo without prior detoxification. RESULTS: A significant difference between naltrexone and placebo groups was found in the association between the preference for higher sucrose concentrations and relapses to heavy drinking. Higher sweet preference was significantly related to succes…

Long-acting injectable naltrexone for the management of patients with opioid dependence.

Opioid dependence is a condition with serious clinical ramifications. Treatment has focused on detoxification, agonist therapy with methadone or buprenorphine, or remission maintenance with the opioid antagonist, naltrexone. Treatment with oral naltrexone has been limited by poor treatment adherence and relapse. Studies with long-acting formulations have shown increased treatment adherence. Extended-release injectable naltrexone has been used for the treatment of alcohol dependence, and has recently received an indication for treatment of opioid dependence from the US Food and Drug Administrat…

Augmentation with naltrexone to treat compulsive sexual behavior: a case series.

BACKGROUND: Compulsive sexual behavior (CSB) is generally characterized by recurrent and intense sexually arousing fantasies, sexual urges, and behaviors, which cause individuals distress or impair daily functioning. Descriptive studies of individuals with paraphilic and nonparaphilic CSB indicate that they experience urges to engage in problematic sexual behavior. The opiate antagonist naltrexone has been successfully used to treat a number of disorders in which urges to engage in problematic behavior are a central feature, such as alcoholism. We hypothesized that naltrexone would reduce the …

Effects of naltrexone during the descending limb of the blood alcohol curve.

The neuropharmacological effects of alcohol are known to vary by limb of the blood alcohol curve, yet human laboratory studies of alcoholism pharmacotherapies have largely failed to consider limb of intoxication when examining medication effects on subjective responses to alcohol. This study examined the effects of naltrexone compared to placebo on subjective responses to alcohol at the descending limb of the blood alcohol curve following a controlled intravenous (IV) alcohol administration. Non-treatment-seeking hazardous drinkers (n = 38) completed two double-blind counterbalanced IV alcohol…

Naltrexone for the treatment of amphetamine dependence: a randomized, placebo-controlled trial.

OBJECTIVE: Currently there is no approved pharmacotherapy for amphetamine dependence. Recent human laboratory studies have demonstrated that naltrexone modulates some of the reinforcing effects of amphetamine. The aim of this study was to investigate the efficacy of naltrexone in comparison with placebo in reducing relapse to amphetamine use in amphetamine-dependent patients. METHOD: Eighty patients who met DSM-IV criteria for amphetamine dependence were randomized to 12 weeks of double-blind naltrexone (50 mg) or placebo treatment. Patients visited the clinic twice weekly to receive medicati…

Pharmacological mechanisms of naltrexone and acamprosate in the prevention of relapse in alcohol dependence.

Naltrexone and acamprosate may ultimately prove to be useful additions to pharmacotherapy for alcoholism by reducing relapse. Naltrexone is a relatively selective competitive antagonist at mu-opioid receptors, and this activity may explain its anti-relapse action either because endogenous opioids are involved in the positively reinforcing effects of alcohol and/or because these same transmitters are involved in the conditioned anticipation of these effects. In contrast, the pharmacology of acamprosate is still poorly understood. This is not surprising because it is a small flexible molecule wi…

Use of oral and implantable naltrexone in the management of the opioid impaired physician.

Doctors are at an increased risk for prescription drug use, particularly opioids and benzodiazpines. This use can interfere with work function and has major potential negative implications for patient safety. Oral naltrexone, an opioid antagonist, has been used as part of a management strategy for opioid dependent physicians. While some patients stabilize on oral naltrexone, others relapse to opioid use. An alternative method of naltrexone maintenance involves the injection or surgical insertion of a sustained release preparation of naltrexone. This approach dramatically improves compliance, r…

Effects of naltrexone and isradipine, alone or in combination, on cocaine responses in humans.

Preclinical studies suggested that combination of naltrexone and isradipine may be useful for the treatment of cocaine addiction. This study examined whether naltrexone and isradipine, alone or in combination, would attenuate the subjective and physiological effects of cocaine in humans. Seven cocaine users participated in a randomized, double-blind, placebo-controlled inpatient study. Before each of the seven experimental sessions, subjects were treated orally with naltrexone (50 mg or placebo), isradipine (10 mg or placebo), or naltrexone plus isradipine. Subjects then received a single dose…

Naltrexone's effect on cue-elicited craving among alcoholics in treatment.

BACKGROUND: Advancing knowledge of biobehavioral effects of interventions can result in improved treatments. Thus, a standardized laboratory cue reactivity assessment has been developed and validated to assess the cognitive and psychophysiological responses to a simulated high-risk situation: alcohol cues. The present study investigates the effects of a pharmacotherapy (naltrexone) on a laboratory-based, cue-elicited urge to drink among abstinent alcoholics in treatment. METHODS: Alcohol-dependent subjects were randomized to 12 weeks of naltrexone or placebo after completing a partial hospita…

Naltrexone effects on insulin sensitivity and insulin secretion in hyperandrogenic women.

A total of 12 women (24.2 +/- 1.6 years old, BMI 36.7 +/- 1.5 Kg/m2) with hyperandrogenism (HA) and with normal glucose tolerance test were studied to evaluate the involvement of endogenous opioids in the pathophysiology of insulin secretion and insulin sensitivity in HA by administering naltrexone, an oral opioid receptor antagonist. Six patients received naltrexone orally (75 mg daily) and another six received placebo for 12 weeks (double-blind study). Before and after therapy a frequently sampled intravenous glucose tolerance test (FSIVGTT) was performed. The insulin sensitivity index (SI) …

Differential naltrexone antagonism of hydromorphone and pentazocine effects in human volunteers.

Naltrexone is more potent in blocking mu receptor activity than kappa receptor activity. This study examined the interactions of naltrexone with the mu agonist, hydromorphone, and with the mixed agonist-antagonist, pentazocine, to assess the apparent receptor mechanisms of their effects. After oral, 2-hr pretreatment with placebo or naltrexone (12.5 or 25 mg), a within-session cumulative dosing procedure (four intramuscular injections at 1-hr intervals) was used to assess the effects of hydromorphone (0, 0.75, 1.5 and 3 mg) and pentazocine (0, 15, 30 and 60 mg) vs. saline placebo.
