Thymoma and solid-organ transplantation.

Solid-organ-transplant recipients are at high risk of developing cancer, including thoracic malignancies. Herein, we report on two cases of thymoma in solid-organ recipients. One case was a female kidney-transplant recipient who had a type B2, Masaoka stage IVA thymoma that was treated by extended surgical resection followed by postoperative radiation therapy. The second case was type A Masaoka II thymoma that occurred after a liver transplant. The role of EBV in the oncogenesis of thymoma is controversial. We suggest that viruses may induce neoplasms in this specific population, as observed i

A "patient-like" nude mouse model of parietal pleural human lung adenocarcinoma.

Histologically intact human adenocarcinoma of the lung maintained in nude mice by serial subcutaneous transplantations (4 generations) was harvested and transplanted to the parietal pleura of 11 other nude mice via a novel thoracotomy procedure. Local and regional tumor growth of transplanted tumors occurred in all cases within 31 days, resulting in weight loss, decreased physical activity and death. This rapid-growth "patient-like" model of direct transplantation of a human neoplasm to the parietal pleura may facilitate basic research of pleural malignancies, and stimulate studies of pleural

Budd-Chiari syndrome: reassessment of a step-wise treatment strategy.

BACKGROUND AND STUDY AIMS: The Budd-Chiari syndrome is a rare disorder characterized by hepatic venous outflow obstruction. A step-wise management was recently proposed. The aim of this study is to reassess our treatment approach and long-term outcome. PATIENTS AND METHODS: The data of 37 Budd-Chiari patients, seen in our unit, were critically analyzed and compared with the ENVIE (European Network For Vascular Disorders of the Liver) data. RESULTS: Most patients had multiple prothrombotic conditions (41%), of which an underlying myeloproliferative neoplasm was the most frequent (59%). The JA

Long-term Outcomes (10 years) of Hepatic Transplantation in a Patient with Hepatic Adenomatosis: A Case Report.

Hepatocellular adenoma is a rare benign liver neoplasm, and hepatic adenomatosis is associated with risks such as intertumoral hemorrhage or necrosis. The imaging features of adenomatosis closely resemble those of solitary adenomas, differing only in the number of lesions. This study reports a case of an 18-year-old woman with perturbed liver function enzymes and multiple hepatic nodules. She had no significant medical history, chronic liver disease, or diabetes and was not taking any regular medications or anabolic steroids. Abdominal ultrasound revealed hepatomegaly and multiple hepatic nodu

The role of transplantation in liver disease.

Liver transplantation is rapidly emerging as the most effective treatment pathway for a growing number of acute and chronic liver disease states. Indications and contraindications to transplant are undergoing continuous revision and clarification as experience is accrued in the expanding number of treatment centers. For some disorders such as primary biliary cirrhosis, sclerosing cholangitis, and chronic active hepatitis with cirrhosis, the role of transplantation in patient management is obvious. For other hepatic diseases such as primary hepatic neoplasm, clear definition of the role of tran

Update on primary plasma cell leukemia.

Plasma cell leukemia (PCL) is a rare and highly aggressive plasma cell neoplasm developing in 0.5?4% of patients with multiple myeloma (MM). The diagnostic criteria were recently revised from 20% to ≥5% of circulating plasma cells in peripheral blood smears. PCL is classified as primary or secondary; primary PCL is when it presents in patients with no MM. Primary PCL shows clinical and laboratory features at presentation that differ from MM and exhibits a dismal prognosis even with the use of effective agents against MM. Therefore, intensive chemotherapy should be initiated immediately after d

Cellular Blue Nevus of Perilimbal Conjunctiva: A Case Report and Review of Literature.

Cellular blue nevus is an uncommon neoplasm in the conjunctiva. Here we present an unusual case of a cellular blue nevus that clinically resembled conjunctival melanoma. A 29-year-old Chinese male was found to have a giant pigmented lesion of the conjunctiva around the limbal area of right eye from birth. Excisional biopsy with no-touch technique, lamellar corneal transplantation, amniotic membrane transplantation and adjuvant cryotherapy were performed. Histopathology revealed a nodular, well-defined tumor, which was composed of heavily pigmented dendritic cells and less pigmented spindle cel

Photobiomodulation therapy to prevent oral mucositis and functional impairment in adult patients with haematological cancer undergoing haematopoietic stem cell transplantation: randomised trial protocol.

IntroductionOral mucositis is a highly prevalent condition in individuals treated for haematological neoplasms, primarily during haematopoietic stem cell transplantation (HSCT). The condition is known to delay recovery processes, increasing the risk of infection, the number of interventions and the length of hospital stays. The proposed Photobiomodulation Therapy for Oral Mucositis and Functional Impairment Transplantation Trial aims to assess the effectiveness and acceptability of using photobiomodulation in the oral cavity to prevent oral mucositis and functional impairment in adult patients

Venetoclax in relapsed/refractory blastic plasmacytoid dendritic cell neoplasm with central nervous system involvement: a case report and review of the literature.

BACKGROUND: We describe a patient with blastic plasmacytoid dendritic cell neoplasm with central nervous system involvement and the outcome of venetoclax use in this setting. CASE PRESENTATION: A 54-year-old Caucasian male was referred to the Haematology Unit with an enlarged inguinal lymph node which was diagnostic of a blastic plasmacytoid dendritic cell neoplasm. The staging revealed disseminated disease (skin, visceral, lymph nodes, and bone marrow). He received chemotherapy with an acute myeloid leukaemia-like regime. Afterwards, he underwent allogeneic haematopoietic stem cell transplan

A cyclin D1-negative mantle cell lymphoma with an IGL-CCND2 translocation that relapsed with blastoid morphology and aggressive clinical behavior.

Mantle cell lymphoma (MCL) is a B cell neoplasm characterized by cyclin D1 overexpression; its prognosis is poor, especially when it exhibits a blastoid morphology. Cyclin D1-negative MCL is rare, and its pathogenesis and progression remain unclear. Herein, we describe a cyclin D1-negative, cyclin D2-positive MCL with a CCND2 and immunoglobulin lambda light chain (IGL) translocation. The patient was initially diagnosed with cyclin D1-negative MCL and achieved complete remission via combination chemotherapy and autologous stem cell transplantation. After relapsing, he was diagnosed with a blast

Subungual squamous cell carcinoma after organ transplantation.

It is accepted that immunosuppressant therapy after organ transplantation is associated with an increase risk of neoplasm. Subungual squamous cell carcinoma of the toe is a rare condition and has not previously been reported, to our knowledge, in patients undergoing immunosuppressant therapy. The objective of this case study is to report the clinical and histopathologic findings and the multidisciplinary treatment approach for a case of subungual squamous cell carcinoma of the toe in an organ transplant recipient undergoing immunosuppressant drug therapy.

An unusual presentation of extramedullary plasmacytoma occurring sequentially in the testis, subcutaneous tissue, and heart.

Extramedullary plasmacytoma (EMP) is a rare neoplasm of soft tissue that usually arises in the respiratory tract, nasal cavity, sinuses, and nasopharynx. It is even more uncommon for it to arise either in the testis or heart. We report the presentation of a case where plasmacytomas were found sequentially in the testis, subcutaneous tissue, and heart. EMP usually has a good prognosis except when it involves the heart. Our patient survived for only 15 months post autologous hematopoietic stem cell transplantation.

JAK2 V617F impairs hematopoietic stem cell function in a conditional knock-in mouse model of JAK2 V617F-positive essential thrombocythemia.

The JAK2 V617F mutation is found in most patients with a myeloproliferative neoplasm and is sufficient to produce a myeloproliferative phenotype in murine retroviral transplantation or transgenic models. However, several lines of evidence suggest that disease phenotype is influenced by the level of mutant JAK2 signaling, and we have therefore generated a conditional knock-in mouse in which a human JAK2 V617F is expressed under the control of the mouse Jak2 locus. Human and murine Jak2 transcripts are expressed at similar levels, and mice develop modest increases in hemoglobin and platelet leve

Pure red cell aplasia preceding malignant lymphoma in a renal transplant patient.

Malignant disorders are one of the major causes of morbidity and mortality in transplant patients. We present herein a renal transplant recipient with malignant lymphoma which preceded by pure red cell aplasia (PRCA). Acquired PRCA is a rare hematologic disorder in renal transplant recipients. It has been associated with a variety of disorders of immunologic dysfunction and neoplasms, exposure to drugs and toxins, infectious diseases, pregnancy and severe nutritional deficiency. This is the first case with PRCA preceding the malign lymphoma in a renal transplant patient. Treatment of lymphoma

[Biliary obstruction due to adenocarcinoma of the ampulla of Vater ten years after orthotopic liver transplantation].

The main causes of death in patients with liver transplants are derived from the graft itself. Given the increased survival in this group of patients, the rate of de novo neoplasms in other locations is increasing. We present the case of a 60-year-old man who underwent orthotopic liver transplantation 10 years earlier for terminal cirrhosis caused by hepatitis B. Imaging studies showed dilation of the intra- and extra-hepatic bile ducts with no evidence of lithiasis, anastomotic stenosis, or other biliary complications that are common in patients with liver transplants. The definitive diagnosi

Blastic plasmacytoid dendritic cell neoplasm: a clinicopathologic review.

Blastic plasmacytoid dendritic cell neoplasm is a rare entity grouped with the acute myeloid leukemia-related precursor neoplasms in the 2008 World Health Organization classification. It was previously postulated to originate from natural killer cells, T cells, or monocytes but is now believed to arise from the plasmacytoid dendritic cell. The pathogenesis of blastic plasmacytoid dendritic cell neoplasm is not well understood, although the neoplasm demonstrates frequent deletion of tumor suppressor genes, including RB1, CDKN1B, CDKN2A, and TP53. Blastic plasmacytoid dendritic cell neoplasm is

De novo malignances in pediatric organ transplant recipients.

A study of 10813 types of cancer that occurred in 10151 organ transplant recipients showed that the pattern of malignancies that occurred in pediatric recipients was very different from the general pediatric population and from adult recipients. Tumors (527) occurred in 512 pediatric patients (aged 18 years or less), and 9639 adults developed 10286 neoplasms. Post-transplant lymphoproliferative disease (PTLD) was the predominant neoplasm in pediatric recipients and comprised 52% of all tumors compared with 15% in adult recipients. Eighty-four percent of PTLD in the former patients presented du

Renal epithelial hyperplastic and neoplastic proliferation in autosomal dominant polycystic kidney disease.

The early v late occurrence of tubular epithelial hyperplasia and the frequency and malignant potential of renal neoplasms in autosomal dominant polycystic kidney disease (ADPKD) are controversial. The kidneys from 87 patients with documented or presumed ADPKD, removed at autopsy (n = 49) or prior to transplantation (n = 38), were thoroughly sectioned and examined. Hyperplastic polyps were found in 90.8% of the patients, even in the absence of renal insufficiency or marked renal enlargement. However, their number was significantly higher in the patients with advanced stages of the disease, and

Seronegative Herpes simplex Associated Esophagogastric Ulcer after Liver Transplantation.

Herpes simplex infection is characterized by acute or subacute infection, often followed by a chronic carrier state. Consecutive recurrences may flare up if immunocompromise occurs. Herpes simplex associated esophagitis or duodenal ulcer have been reported in immunocompromised patients due to neoplasm, HIV/AIDS or therapeutically induced immune deficiency. Here we report the case of an HSV-DNA seronegative patient who developed grade III dysphagia 13 days after allogeneic liver transplantation. Endoscopy revealed an esophageal-gastric ulcer, and biopsy histopathology showed a distinct fibropla
