The placing reaction in adult neurology.

The first description of the placing reaction (PR) in 12 human adult cases is given. The optimum position for eliciting the placing reaction (PR) is the dangling leg posture, i.e. the same as for the forward groping of the foot. There are three forms of PR quite similar to those noticed in animal physiology, i.e. the visual-PR, the dorsum-PR and the sole-PR. The term contact placing is preferred for the last two forms. The PR is encountered only in patients displaying a forward and medial groping of the foot as well as a groping of the hand on the same side. The PR is usually ipsilateral to th

The neurology of metastatic chorion carcinoma.

Although chorion carcinoma of the uterus is the seventh commonest in the comparative frequency of malignant tumours seen at the Cancer Registry of the University College Hospital, Ibadan, Nigeria (being exceeded in order of commoness by the reticuloendothelial tumours, carcinoma of the cervix, liver, breast, stomach and ovary), it is the most frequent source of tumour deposit of the brain in this hospital. Between 1960 and 1969, 197 Nigerians with chorion cancer of the uterus were admitted to UCH; in twenty-five of them the nervous system was involved during the course of their disease. The ne

Neuroglial markers of damage in autoimmune neurology.

The advancements in assays for the detection of neuroglial markers of damage, such as neurofilament light chain and glial fibrillary acid protein, have led to a growing interest in their investigation in neurological disorders. In this review, we aim to summarize evidence regarding the role of biomarkers of neuroglial damage in autoimmune neurological disorders of the central nervous system, including myelin oligodendrocyte glycoprotein antibody-associated disease, neuromyelitis optica spectrum disorder, autoimmune encephalitis, and paraneoplastic neurological syndromes. We will discuss how th

Immunoglobulin use in neurology: a practical approach.

Human immunoglobulin, delivered either intravenously (IVIg) or subcutaneously, is used to treat a range of immune-mediated neurological disorders. It has a role in acute or subacute inflammatory disease control and as a maintenance therapy in chronic disease management. This review considers mechanisms of IVIg action and the evidence for IVIg in neurological conditions. We use Guillain-Barré syndrome and chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) as frameworks to demonstrate an approach to IVIg use in acute and chronic dysimmune neurological conditions across two differen

Functional Neurological Disorder Among Neurology In-Patients.

BACKGROUND: The high prevalence of functional neurological disorders (FND) in specialized neurological services such as epilepsy centers or dizziness clinics is well established. Few studies exist that investigate the frequency of FND among neurology in-patients. METHODS: In-hospital patients with functional neurological symptoms as part of the presenting complaint were identified by combining prospective and retrospective case ascertainment over a period of 2 years. Demographic and clinical features as well as reimbursements were analyzed. RESULTS: Across a total of 4648 in-hospital patient…

Sturge-Weber syndrome: updates in translational neurology.

Sturge-Weber syndrome (SWS) is a rare congenital neurovascular disorder that initially presents with a facial port-wine birthmark (PWB) and most commonly associated with a R183Q somatic mosaic mutation in the gene GNAQ. This mutation is enriched in endothelial cells. Contrast-enhanced magnetic resonance imaging (MRI) diagnoses brain abnormalities including leptomeningeal vascular malformation, an enlarged choroid plexus, and abnormal cortical and subcortical blood vessels. Mouse SWS models identify dysregulated proteins important for abnormal vasculogenesis and blood brain barrier permeability

Emerging Trends: Neurofilament Biomarkers in Precision Neurology.

Neurofilaments are structural proteins found in the cytoplasm of neurons, particularly in axons, providing structural support and stability to the axon. They consist of multiple subunits, including NF-H, NF-M, and NF-L, which form long filaments along the axon's length. Neurofilaments are crucial for maintaining the shape and integrity of neurons, promoting axonal transport, and regulating neuronal function. They are part of the intermediate filament (IF) family, which has approximately 70 tissue-specific genes. This diversity allows for a customizable cytoplasmic meshwork, adapting to the uni

[Temporary and persistent visual phenomena in neurology].

Temporary and persistent visual phenomena are a frequent reason for a neurological presentation. The diagnosis can usually be made with the help of a structured anamnesis with determination of the time of onset, the course and symptoms as well as the monocular vs. binocular manifestation. The visual aura in migraine is certainly the most frequent entity to be differentiated. In particular, persistent visual phenomena such as visual snow syndrome, hallucinogen persisting perception disorder and the Charles Bonnet syndrome (CBS) seem to be underdiagnosed in clinical practice for various reasons

Orphan Drugs in Neurology-A Narrative Review.

BACKGROUND AND AIMS: Orphan diseases, or rare diseases, are defined in Europe as diseases that affect less than 5 out of every 10,000 citizens. Given the small number of cases and the lack of profit potential, pharmaceutical companies have not invested much in the development of possible treatments. However, over the last few years, new therapies for rare diseases have emerged, giving physicians a chance to offer personalized treatment. With this paper, we aim to present some of the orphan neurological diseases for which new drugs have been developed lately. METHODS: We have conducted a liter

Neonatal Fc Receptor-Targeted Therapies in Neurology.

Autoantibodies are increasingly recognized for their pathogenic potential in a growing number of neurological diseases. While myasthenia gravis represents the prototypic antibody (Ab)-mediated neurological disease, many more disorders characterized by Abs targeting neuronal or glial antigens have been identified over the past two decades. Depletion of humoral immune components including immunoglobulin G (IgG) through plasma exchange or immunoadsorption is a successful therapeutic strategy in most of these disease conditions. The neonatal Fc receptor (FcRn), primarily expressed by endothelial a

The ethics of crowdfunding in paediatric neurology.

In the last decade, there has been a dramatic increase in the number of families resorting to internet-based public appeals to fund access to novel, highly expensive, or experimental therapies for rare disorders. Medical crowdfunding may provide a means to fund treatments or interventions, but it raises individual and societal ethical questions. In this review, we consider the ethical challenges crowdfunding poses in paediatric neurology, drawing on the example of gene therapy for spinal muscular atrophy. We discuss physician responsibilities, and how neurologists should respond to crowdfundin…

Faradization for insomnia: a sleep neurology history.

Insomnia is highly prevalent and challenging to treat. We typically regard insomnia as a disorder of the modern world, but physicians and patients have been struggling with this malady for millennia. Here we present the curious historical practice of using electrization or faradization to treat insomnia. We present methods of application, hypotheses regarding mechanism of action, and historical case reports and case series to better understand this phenomenon. We put faradization for insomnia in the context of the modern use of electrical therapies to support and facilitate human health in mul

Shell Shock: The Response of UK Neurology.

The neurological and psychological manifestations of trauma, confinement, and terror became apparent throughout Europe as soldiers were evacuated from the trenches of the Western Front. The response in the UK evolved as a result of the experience of medical staff embedded with the troops in base hospitals and the philosophy of those treating returned soldiers in specialist establishment. There were widely disparate approaches to the management encompassing simple supportive care, a psychanalytic approach and radical electric shock therapy. The latter was partially driven by the Queen Square ex
