Acoustic neuroma observation associated with an increase in symptomatic tinnitus: results of the 2007-2008 Acoustic Neuroma Association survey.

OBJECT: Tinnitus is a known presenting symptom of acoustic neuromas, but little is known about the impact of observation or treatment on tinnitus. Most patients experience improvement with treatment, while others may worsen. Therefore, this study was designed to assess the overall impact of observation and treatment on tinnitus outcome in patients with acoustic tumors. METHODS: Data from the 2007-2008 Acoustic Neuroma Association survey were used. Tinnitus severity was graded both at presentation and at last follow-up for all patients questioned. This data set was analyzed using the Student t…

Rectal Traumatic Neuroma Mimicking a Malignant Mass.

Traumatic neuromas rarely occur in the digestive system, but the biliary tree is the most common site for neuromas in the post-cholecystectomy or liver transplantation setting. Herein, we report an extremely rare rectal neuroma in a 48-year-old male with familial adenomatous polyposis (FAP) and a history of subtotal colectomy for colonic adenocarcinoma. The surgical resection was performed for clinical, endoscopic, and radiological suspicion of recurrent malignancy. Surprisingly, the resected lesion was confirmed to be a traumatic neuroma at the ileorectal anastomosis.

Neuroma-to-Nerve Ratio: Does Size Matter?

BACKGROUND AND OBJECTIVES: Anatomic features of neuromas have been explored in imaging studies. However, there has been limited research into these features using resected, ex vivo human neuroma specimens. The aim of this study was to investigate the influence that time may have on neuroma growth and size, and the clinical significance of these parameters. METHODS: Patients who underwent neuroma excision between 2022 through 2023 were prospectively included in this study. Neuroma specimens were obtained after operative resection. Standardized neuroma size measurements, expressed as a neuroma-…

Paroxysmal Positional Nystagmus in Acoustic Neuroma Patients.

The association between acoustic neuroma and positional vertigo with paroxysmal positional nystagmus is relatively rare, but, when present, it certainly represents a challenge for the otoneurologist. There are few reports in the literature on this particular issue, and some questions are still unanswered, particularly regarding the characteristics of positional nystagmus that may distinguish between a true benign paroxysmal vertigo and a positional nystagmus associated with the tumor. We present the videonystagmographic patterns of seven patients with acoustic tumor who had paroxysmal position…

Acoustic neuroma presenting as a hypnic headache.

Hypnic headache (HH) is a rare, primary headache syndrome that invariably occurs during sleep and wakes the patient. Acoustic neuroma (AN) is a benign tumour that uncommonly presents with isolated headache. Here, we describe a patient with AN that presented with an HH-like syndrome. A 40-year-old woman presented with 4 months of generalised, throbbing, nocturnal headaches that woke her from sleep. Neurological examination was unremarkable. Retrospectively, she reported a 4-year history of mild, bilateral tinnitus. Neuroimaging demonstrated a large, left-sided AN in the cerebellopontine angle w…

Etiology and Treatment Outcomes for Sural Neuroma.

BACKGROUND: Neuroma results from disorganized regeneration following nerve injury and may be symptomatic. The aim of this study was to investigate the causes, treatment, and outcomes of operatively treated sural neuromas, and to describe the factors associated with persistent or unchanged postoperative pain symptoms. METHODS: Consecutive patients with surgically treated sural neuromas in a 14-year period were identified using Current Procedural Terminology (CPT) codes ( n = 49), followed by a chart review to collect patient and treatment characteristics. Postoperative pain symptoms were categ…

Tinnitus following treatment for sporadic Acoustic neuroma.

OBJECTIVES/HYPOTHESIS: To evaluate the impact of treatment modality, tumor size, time from therapy, and demographic features on tinnitus distress, as measured by the Tinnitus Functional Index (TFI) in patients treated for sporadic acoustic neuroma. STUDY DESIGN: Cross-sectional observation study. METHODS: A Web-based 44-question online survey was made available on the Acoustic Neuroma Association Web site for 3 months. Of 154 unique surveys that were completed in entirety, further screening netted 143 study participants. Questions included the TFI, treatment modality, tumor size, time from t…

Posttraumatic saphenous neuroma after open tibial fracture.

Neuralgia and neuroma secondary to iatrogenic saphenous nerve injury have been described in the setting of orthopedic surgical interventions. However, postoperative neuropathic pain caused by saphenous neuroma in the setting of orthopedic trauma has not been reported. We present a case of a 43-year-old woman with a symptomatic saphenous neuroma after an open, laterally angulated tibial fracture. The diagnosis was confirmed in the clinical setting, and treatment consisted of neurolysis and partial neurectomy, resulting in complete pain relief and functional recovery. This case demonstrates the …

Amputation neuroma growing intravascularly into a thrombus.

An adult man underwent arm amputation for a sarcoma. Pain and three masses observed radiologically prompted surgical exploration five years later. Microscopically, the masses represented amputation neuromas. One of them was located in the lumen of an artery, in a remote organized thrombus. Intravascular growth of an amputation neuroma has not been described previously.

Ultrasound guided alcohol ablation of Morton's neuroma.

BACKGROUND: Morton's neuroma is a common cause of metatarsalgia. This study evaluated the efficacy of ultrasound guided alcohol injection as a treatment for this condition. METHOD: Data from 87 treatment courses were included in this study with a mean follow of 14.3 months. RESULTS: Technical success was 100%. One patient developed symptoms consistent with an allergic reaction to the injection and one patient declined further injection because of periprocedural pain. Partial or total treatment response was achieved in 66%, with 32% of patients having complete resolution of pain. The median v…

Morton's neuroma: A clinical versus radiological diagnosis.

BACKGROUND: The aim of our study was to compare the clinical versus radiological diagnosis of patients suffering from a Morton's neuroma. METHODS: Clinical assessments and pre-operative radiological imaging of patients who had excision of a Morton's neuromas were retrospectively compared. RESULTS: 43 neuromas were excised from 36 patients over 68 months. The commonest clinical symptoms were tenderness on direct palpation (100%), pain on weight bearing (91%) which was relieved by rest (81%). The most sensitive clinical sign was a Mulder's click. Clinical assessment had a sensitivity of 98% (4…

Occupational exposures and risk of acoustic neuroma.

OBJECTIVES: Acoustic neuroma is a benign tumour accounting for approximately 6-10% of all intracranial tumours and occurs mainly in patients aged ≥50 years. Our aim was to investigate a wide range of occupational exposures, individual occupational titles and socioeconomic status (SES) as potential risk factors for acoustic neuroma. METHODS: We conducted a population-based case-control study of 793 acoustic neuroma cases identified through the Swedish Cancer Registry and 101,762 randomly selected controls. Information on SES and occupation was obtained from censuses and linked to job-exposure …

Magnetic resonance imaging screening in acoustic neuroma.

OBJECTIVES: Magnetic resonance imaging (MRI) is the definitive investigation for detection of an acoustic neuroma. It is however an expensive resource, and pick-up rate of a tumor can be as low as 1% of all patients scanned. This study aims to examine referral patterns for MRI screening for patients presenting with asymmetrical sensorineural hearing loss (ASHL). A second aim was to suggest appropriate screening criteria. METHOD: All 132 MRI scans performed for ASHL in the year 2005 were reviewed retrospectively along with their case records and audiograms. In addition, MRI scans and case reco…

Gait instability in patients with acoustic neuroma.

OBJECTIVE: To investigate pathological gait in patients with unilateral acoustic neuroma using tactile sensors placed under both feet. MATERIAL AND METHODS: Forty-three patients were enrolled in the study. They were categorized into two groups: the small tumor group had tumors < 2 cm from the porus acousticus without any brainstem compression and the large tumor group had tumors > 2 cm from the porus acousticus with brainstem compression. Eighteen healthy subjects served as controls. Subjects were asked to walk freely with eyes open or closed for a distance of nearly 8 m. The coefficients of …
