Seasonal variation in night blindness incidence among Union soldiers in the US Civil War.

BACKGROUND: During the US Civil War, medical officers typically attributed night blindness among soldiers to malingering. A dietary basis was not generally suspected or appreciated. DESIGN/METHODS: Incident cases of night blindness, scurvy, and diarrheal diseases, as well as mean troop strength among Union troops, were abstracted by month and race from tabulations of the US Surgeon General for the period from July 1861 through June 1866. Monthly incidence rates and annual incidence rates are presented as time series by race. RESULTS: Night blindness incidence was seasonal. Seasonal patterns …

Structural, energetic, and mechanical perturbations in rhodopsin mutant that causes congenital stationary night blindness.

Several point mutations in rhodopsin cause retinal diseases including congenital stationary night blindness and retinitis pigmentosa. The mechanism by which a single amino acid residue substitution leads to dysfunction is poorly understood at the molecular level. A G90D point mutation in rhodopsin causes constitutive activity and leads to congenital stationary night blindness. It is unclear which perturbations the mutation introduces and how they can cause the receptor to be constitutively active. To reveal insight into these mechanisms, we characterized the perturbations introduced into dark …

Congenital stationary night blindness is associated with the leopard complex in the Miniature Horse.

OBJECTIVE: To determine if congenital stationary night blindness (CSNB) exists in the Miniature Horse in association with leopard complex spotting patterns (LP), and to investigate if CSNB in the Miniature Horse is associated with three single nucleotide polymorphisms (SNPs) in the region of TRPM1 that are highly associated with CSNB and LP in Appaloosas. ANIMALS STUDIED: Three groups of Miniature Horses were studied based on coat patterns suggestive of LP/LP (n=3), LP/lp (n=4), and lp/lp genotype (n=4). PROCEDURES: Horses were categorized based on phenotype as well as pedigree analysis as L…

Negative electroretinogram in the differential diagnosis of malingering of night blindness in the military.

OBJECTIVE: We evaluated the value of unusual electroretinogram (ERG) findings (negative ERG, which is selective reduction in the amplitude of the b-wave) in the differential diagnosis of night blindness in the military. METHODS: We surveyed 3 years of data. Presumptive and final clinical diagnoses before and after negative ERG recordings were evaluated. RESULTS: A total of 495 ERGs were included. The mean age was 22 +/- 2.5 years (range, 20-33 years). Twenty-two patients (4.4%) had negative ERG. Final clinical diagnoses could be made for all except two patients (9%). The diagnoses were conge…

Night blindness with depolarizing pattern of ON/OFF response in electroretinogram: a case report.

PURPOSE: To present a patient who has night blindness with a depolarizing pattern of ON/OFF response by electroretinography (ERG). CASE: A 43-year-old woman had had night blindness and poorly corrected visual acuity since childhood. Parental consanguinity was noted. The patient had suffered from mental retardation, epilepsy, and mild cerebellar ataxia. Corrected visual acuity was 20/30 in the right eye and 20/25 in the left. Goldmann perimetry showed no scotoma but slight depression with internal isoptors. No evidence for rod activity was observed by Goldmann-Weekers adaptometry. The ocular f…

A dominant form of congenital stationary night blindness (adCSNB) in a large Chinese family.

A pedigree of congenital stationary night blindness (CSNB) is described in a large Chinese family. The clinical description, pedigree, dark adaptation and elctroretinogram (ERG) studies indicate that the patients have an autosomal dominant form (ad) of CSNB. The disorder has been transmitted through at least 12 generations with over 40 affected individuals identified. The ERG data reveal that affected persons have severely diminished b-wave responses to dim light, but normal a-wave and subnormal b-wave responses to maximum light stimuli. The dark adaptation curves of three patients show a mono…

ENU-induced late-onset night blindness associated with rod photoreceptor cell degeneration in zebrafish.

We describe a dominant mutation, night blindness d (nbd), that causes late-onset rod photoreceptor cell degeneration in zebrafish. The mutation was induced by treating male zebrafish with N-ethyl-N-nitrosourea (ENU). Visual sensitivity was tested using a behavioral assay based on a visually mediated escape response. At a young age, the heterozygous (nbd+/-) fish did not show any signs of night blindness or retinal degeneration. At 2 years, their behaviorally assessed visual sensitivity was decreased, albeit no alterations in the electroretinogram (ERG) were detected. Histology revealed that in…

Defects in the rhodopsin kinase gene in the Oguchi form of stationary night blindness.

Oguchi disease is a recessively inherited form of stationary night blindness due to malfunction of the rod photoreceptor mechanism. Patients with this disease show a distinctive golden-brown colour of the fundus that occurs as the retina adapts to light, called the Mizuo phenomenon. Recently a defect in arrestin, a member of the rod phototransduction pathway, was found to cause this disease in some Japanese patients. As rhodopsin kinase works with arrestin in shutting off rhodopsin after it has been activated by a photon of light, it is reasonable to propose that some cases of Oguchi disease m…

Environmental conditions, health behavior, and mortality in siblings as risk indicators of night blindness.

The purpose of the study was to estimate the association of environmental factors (water and sanitation), health behavior (breast feeding practice and vaccination), and mortality among siblings with night blindness. A total of 5,420 children were enrolled in a cross-sectional study from 2,101 households in the northern part of Bangladesh, a high risk area for night blindness. One hundred twenty-four children were reported by parents to have night blindness. Our results indicated that environmental factors, health behavior, and the death of siblings were significantly associated with the occurr…

Duchenne muscular dystrophy: negative scotopic bright-flash electroretinogram but not congenital stationary night blindness.

Patients with Duchenne muscular dystrophy (DMD) have recently been reported to have an abnormal scotopic electroretinogram (ERG) showing weak rod-related responses along with a negative configuration of the bright-flash response, which has been described as being similar to the one in congenital stationary night blindness (CSNB). We compared qualitatively and quantitatively the ERGs of 6 subjects with DMD, 10 subjects with the complete form of CSNB (cCSNB), 13 subjects with the incomplete form of CSNB (iCSNB) and 1 subject with complex glycerol kinase deficiency (CGKD). The rod-related activit…

Large rod-like photopic signals in a possible new form of congenital night blindness.

A 10-year-old Persian girl has symptoms of congenital stationary night blindness and some drusen-like lesions in the region of the vascular arcades. Her electroretinogram shows no rod response to a weak stimulus, but a large (475 microV) slow scotopic response to a strong stimulus that is unchanged by photopic conditions (15 Fl background illumination). However, the response to flicker had the typical (smaller) amplitude of a cone signal. They may represent a new form of night blindness in which rod sensitivity is reduced so that there is no vision under dim conditions but rod function still p…

X-linked congenital stationary night blindness. Review and report of a family with hyperopia.

X-linked congenital stationary night blindness is almost always associated with myopia. We have reviewed all previously reported pedigrees and have found only two with patients without myopia. A recently proposed classification of night blindness includes a complete type associated with myopia and an incomplete type in which both hyperopia and myopia were found. Complete and incomplete types did not occur within the same pedigree. We report on a family in which three of the five affected members had hyperopia and could be classified as the incomplete type and in which a fourth member with myop…

Night-vision goggles for night-blind subjects: subjective evaluation after 2 years of use.

PURPOSE: To evaluate the usefulness of night-vision goggles (NVG) for night-blind subjects after 1 and 2 years of use. METHODS: Eleven night-blind subjects with retinitis pigmentosa used NVG for a 2-year period. At the end of each year, they were requested to fill-in two questionnaires regarding their use of the instrument. RESULTS: At both the 1- and 2-year evaluations, the subjects reported fewer problems with mobility in the dark when they used the goggles. At the 2-year evaluation, two individuals stated that they had stopped using the instrument, while seven used it at least twice a wee

Keratomalacia in a Neonate Born to a Mother with Night Blindness: A Case Report.

INTRODUCTION: Xerophthalmia, caused by vitamin A deficiency, is a significant public health problem that can lead to severe ocular surface damage, including keratomalacia and permanent blindness if not promptly treated. CASE: A 26-day-old male neonate presented with bilateral corneal clouding ten days after birth. Ocular examination revealed corneal xerosis in both eyes and a corneal perforation in the right eye, which was sealed with iris tissue. The mother reported a history of night blindness during the third trimester of pregnancy, suggestive of maternal vitamin A deficiency. OBSERVATION…

Clinical and genetic studies for a cohort of patients with congenital stationary night blindness.

BACKGROUND: Congenital stationary night blindness (CSNB) is an inherited retinal disorder. Most of patients have myopia. This study aims to describe the clinical and genetic characteristics of fifty-nine patients with CSNB and investigate myopic progression under genetic cause. RESULTS: Sixty-five variants were detected in the 59 CSNB patients, including 32 novel and 33 reported variants. The most frequently involved genes were NYX, CACNA1F, and TRPM1. Myopia (96.61%, 57/59) was the most common clinical finding, followed by nystagmus (62.71%, 37/59), strabismus (52.54%, 31/59), and nyctalopia…

Natural Course of Refractive Error in Congenital Stationary Night Blindness: Implications for Myopia Treatment.

PURPOSE: A range of pharmacological and optical therapies are being studied and implemented in children with myopia to reduce the progression of myopia. At present, the efficacy of these myopia reduction treatments in children with underlying inherited retinal disorders (IRDs) is largely unknown. To evaluate this efficacy, it is essential to first understand the natural progression of myopia within each distinct underlying IRD. We investigated the natural course of refractive error throughout childhood in patients with congenital stationary night blindness (CSNB) of the Schubert-Bornschein typ…

Cav1.4 congenital stationary night blindness is associated with an increased rate of proteasomal degradation.

Pathogenic, generally loss-of-function, variants in CACNA1F, encoding the Cav1.4α1 calcium channel, underlie congenital stationary night blindness type 2 (CSNB2), a rare inherited retinal disorder associated with visual disability. To establish the underlying pathomechanism, we investigated 10 clinically derived CACNA1F missense variants located across pore-forming domains, connecting loops, and the carboxy-tail domain of the Cav1.4α subunit. Homology modeling showed that all variants cause steric clashes; informatics analysis correctly predicted pathogenicity for 7/10 variants. In vitro analy…
