Evaluation of a Natural Language Processing Model to Identify and Characterize Patients in the United States With High-Risk Non-Muscle-Invasive Bladder Cancer.

PURPOSE: Treatment of non-muscle-invasive bladder cancer (NMIBC) is guided by risk stratification using clinical and pathologic criteria. This study aimed to develop a natural language processing (NLP) model for identifying patients with high-risk NMIBC retrospectively from unstructured electronic medical records (EMRs) and to apply the model to describe patient and tumor characteristics. METHODS: We used three independent EMR-derived data sets including adult patients with a bladder cancer diagnosis in 2011-2020 for NLP model development and training (n = 140), validation (n = 697), and appl

New synthetic nano-immunotherapy (OncoTherad®) for non-muscle invasive bladder cancer: Its synthesis, characterization and anticancer property.

Bacillus Calmette-Guérin (BCG)-based intravesical immunotherapy has been applied as gold standard treatment for high-risk non-muscle invasive bladder cancer (NMIBC) for almost half a century. However, several patients with high-risk disease experience relapse, including those whose condition has worsened and who failed to respond to BCG. Non-significant therapeutic options have been developed for these at-risk patients, for many years. Immunotherapies have shown promising outcomes for bladder cancer treatment. Accordingly, our research group developed the OncoTherad® (MRB-CFI-1) immunotherapy,

Bioengineering strategies to enhance BCG immunotherapy against bladder cancer.

Bacillus Calmette-Guérin (BCG) remains the frontline immunotherapy for non-muscle-invasive bladder cancer (NMIBC), yet recurrence and non-response occur in over half of patients, highlighting the need for improved therapeutic strategies. Recent advances in bioengineering including genetic modification, synthetic biology, biomaterials, and delivery systems are expanding BCG from a traditional vaccine into a more versatile immunotherapeutic platform. In this review, we summarize the mechanisms of BCG-induced antitumor immunity and discuss current bioengineering strategies aimed at enhancing tumo

Mechanisms of microbiome-immune interactions in bladder cancer and targeted regulatory strategies.

Bladder cancer (BC), a highly prevalent malignancy of the urinary tract, imposes a considerable clinical burden because of its high recurrence rates and a substantial risk of disease progression. Accumulating evidence suggests that microbiome dysbiosis is associated with the initiation and progression of BC through potentially bidirectional interactions. Recent advances in high-throughput sequencing technologies have facilitated a more comprehensive characterization of the urinary microbiome, thereby revealing its potential involvement in the pathogenesis of BC. This review summarizes the prop

Enhancing PDT efficacy in NMIBC: Efflux inhibitor mediated improvement of PpIX levels and efficacy of the combination of PpIX-PDT and SO-cleavable prodrugs.

Protoporphyrin IX (PpIX)-based photodynamic therapy (PDT) has shown limited efficacy in nonmuscle-invasive bladder cancer (NMIBC). To improve PDT efficacy, we developed singlet oxygen-cleavable prodrugs. These prodrugs, when combined with PpIX-PDT, induce cancer cell death through both PDT and drug release mechanisms. Inhibition of PpIX efflux was reported to be an effective strategy to improve PpIX-PDT in certain cancer cells. Our main goal was to investigate whether adding an efflux inhibitor to the combination of PpIX and prodrugs can improve the PpIX levels in bladder cancer cells and the

Oncological Outcomes of Nonmuscle-Invasive Bladder Cancer in Patients Aged 45 Years and Younger: A Retrospective Matched Cohort Study.

PURPOSE: Most bladder cancers are diagnosed as nonmuscle invasive (NMIBC), with 90% after the age of 55 years. Although the incidence of several malignancies is increasing among younger adults in whom more aggressive behavior is observed, our knowledge about NMIBC in young patients remains limited. We compared oncological outcomes in patients with NMIBC aged 45 years and younger with a cohort of older patients. MATERIALS AND METHODS: This is a retrospective study of NMIBC diagnosed at age 45 years or younger (2005-2022) from 4 Canadian centers. End points included cancer-specific mortality, r

Patient-Centered Approaches to Non-Muscle-Invasive Bladder Cancer Care.

Approximately 75% to 80% of cases are nonmuscle-invasive bladder cancer (NMIBC), an often chronic condition with high recurrence rates necessitating lifelong surveillance and repeated interventions. Despite contemporary favorable 5-year cancer-specific survival ranging from 71% to 96%, patients experience substantial physical, psychological, and financial burdens due to the significant and invasive follow-up monitoring and treatment of recurrences. TURBT and adjuvant intravesical therapy remain the cornerstone of treatment of NMIBC, and we discuss some innovations within this space, with novel

Minimal Residual Disease Detection with Urine-derived DNA Is Prognostic for Recurrence-free Survival in Bacillus Calmette-Guérin-unresponsive Non-muscle-invasive Bladder Cancer Treated with Nadofaragene Firadenovec.

BACKGROUND AND OBJECTIVE: Urinary tumor DNA (utDNA) profiling identifies mutations associated with urothelial carcinoma and can be used to detect minimal residual disease (MRD). We evaluate the utility of utDNA profiling to predict treatment failure in bacillus Calmette-Guérin-unresponsive high-grade (HG) non-muscle-invasive bladder cancer (NMIBC) treated with nadofaragene firadenovec. METHODS: Urine was collected from participants prior to induction (n = 32) and at their 3-mo evaluation (n = 18) in the parallel-arm, phase 2 study (NCT01687244) of nadofaragene firadenovec. Analysis of the Application of the Micrometric Substaging System to Predict Recurrence and Progression in a Cohort of Patients with pT1 High-Grade Non-Muscle Invasive Bladder Cancer.

BACKGROUND: The World Health Organization (WHO) classification system for bladder cancer (BC) advocates for the substaging of pT1 disease, which may improve the prediction of cancer recurrence and progression. This study aims to evaluate the application and prognostic significance of a micrometric substaging system, utilising a 1 mm cut-off depth of invasion in patients with pT1 BC. METHODS: We retrospectively reviewed all patients diagnosed with pT1 High-Grade Non-Muscle Invasive Bladder Cancer (NMIBC) at our institution. Impact of concomitant carcinoma in situ distribution on non-muscle-invasive bladder cancer progression risk.

OBJECTIVE: To assess whether the distribution of concomitant carcinoma in situ (CIS; unifocal or multifocal) with papillary non-muscle-invasive bladder cancer (NMIBC) impacts the risk of progression, as concomitant CIS is an established risk factor for progression in papillary NMIBC and commonly used calculators do not make this distinction. PATIENTS AND METHODS: In this multi-institutional retrospective cohort study from both academic and community hospitals, clinicopathological data were collected from patients with pTa/pT1 NMIBC treated from 2005 to 2022. Unifocal concomitant CIS was defin

Real-World Experience with a Commercial Circulating Tumor DNA Assay in Non-muscle-invasive Bladder Cancer.

Circulating tumor DNA (ctDNA) is an emerging biomarker in advanced bladder cancer, but its role in non-muscle-invasive bladder cancer (NMIBC) remains undefined. We conducted a retrospective study of 23 patients with NMIBC who underwent serial ctDNA monitoring using the commercially available Signatera assay at a single institution. ctDNA was detected in 35% of patients, with two illustrative cases highlighting its clinical utility. In one case, baseline ctDNA positivity prompted earlier reimaging that revealed locally advanced disease, leading to initiation of systemic therapy followed by plan

Assessing oncological outcomes of bladder-sparing treatments in patients with BCG-unresponsive and BCG-exposed non-muscle-invasive bladder cancer.

BACKGROUND: A subset of non-muscle invasive bladder cancer (NMIBC) patients falls between BCG-naïve and BCG-unresponsive, leading to the introduction of the 'BCG-exposed' category. We assessed oncological outcomes in BCG-unresponsive and BCG-exposed patients undergoing bladder-sparing treatments (BST). METHODS: This retrospective study included BCG-unresponsive and BCG-exposed patients who underwent BST with BCG or a shift to different intravesical chemotherapy. Patients undergoing early RC after BCG failure (N = 14) were excluded. Kaplan-Meier (KM) curves estimated HG recurrence-free surviva

An exploratory study investigating the impact of the bladder tumor microbiome on Bacillus Calmette Guerin (BCG) response in non-muscle invasive bladder cancer.

PURPOSE: Intravesical Bacillus Calmette-Guerin (BCG) is standard of care for intermediate- and high-risk non-muscle invasive bladder cancer (NMIBC). The effect of the bladder microbiome on response to BCG is unclear. We sought to characterize the microbiome of bladder tumors in BCG-responders and non-responders and identify potential mechanisms that drive treatment response. MATERIALS AND METHODS: Patients with archival pre-treatment biopsy samples (2012-2018) were identified retrospectively. Prospectively, urine and fresh tumor samples were collected from individuals with high-risk NMIBC (20

Bladder Epicheck Test: A Novel Tool to Support Urothelial Carcinoma Diagnosis in Urine Samples.

Bladder cancer and upper urothelial tract carcinoma are common diseases with a high risk of recurrence, thus necessitating follow-up after initial treatment. The management of non-muscle invasive bladder carcinoma (NMIBC) after transurethral resection involves surveillance, intravesical therapy, and cytology with cystoscopy. Urinary cytology, cystoscopy, and radiological evaluation of the upper urinary tract are recommended during follow-up in the international urological guidelines. Cystoscopy is the standard examination for the first assessment and follow-up of NMIBC, and urine cytology is a

Traditional and next-generation bacillus Calmette-Guérin based treatment strategies for bacillus Calmette-Guérin unresponsive non-muscle-invasive bladder cancer in the era of emerging therapies.

PURPOSE OF REVIEW: Bacillus Calmette-Guérin (BCG) remains the standard of care for high-risk non-muscle-invasive bladder cancer (NMIBC), yet up to 40-50% of patients experience treatment failure, leaving limited alternatives to avoid radical cystectomy. This narrative review critically examines both traditional and emerging BCG-based strategies - including repeat induction and modern combination regimens - for patients with BCG-unresponsive NMIBC. RECENT FINDINGS: BCG monotherapy after BCG failure has shown limited effectiveness, with recent studies reporting 12-month disease-free survival (D

RESECT: A Randomised Controlled Trial of Audit and Feedback in Non-muscle-invasive Bladder Cancer Surgery.

BACKGROUND AND OBJECTIVE: We aimed to determine whether audit, feedback, and education improves surgical performance after transurethral resection of bladder tumour surgery for non-muscle-invasive bladder cancer and as a secondary aim if it reduced recurrence rates. METHODS: This cluster randomised controlled trial compared audit and feedback plus peer comparison and education, with audit alone for four coprimary outcomes: (1) Single-instillation chemotherapy, (2) detrusor muscle sampling, (3) documentation of tumour features, and (4) resection completeness. Early recurrence was a secondary o
