Holoprosencephaly: a case presentation.

Holoprosencephaly is a rare, potentially catastrophic genetic defect involving the midfacial region and brain. The most severe expressions of the disorder are fatal. Approximately 20 percent of individuals with holoprosencephaly have normal brains and mild facial deformities. As adults, these individuals have the potential of producing severely affected offspring. Therefore, accurate health histories for purposes of case finding and referral for genetic counseling are important components of nursing care for any obstetric patient. NICU nurses should be aware of this rare disorder. When it pres

Improving Diaper Performance for Extremely Low-Birth-Weight Infants.

BACKGROUND: Extremely low-birth-weight (ELBW) infants face significant diapering challenges compared with their full-term peers, due to immature musculature, nervous system, and skin development. Advances in medical care have increased an ELBW infant's rate of survival, which creates a growing need for diapers to better serve these infants. Aim of research. The objective of this study was to identify and confirm the requirements for optimal diaper performance from the neonatal intensive care unit nurses' perspective, as well as to assess in-hospital performance to determine if new features imp

Meconium Peritonitis: A Case Study.

Meconium peritonitis is a sterile chemical peritonitis preceded by bowel perforation, resulting in meconium leakage and subsequent inflammatory cascade within the peritoneal cavity. The presentation can range from simple failure of the neonate to pass meconium to complications such as persistent pulmonary hypertension, lung hypoplasia, and systemic inflammatory syndrome. The purpose of this article is to review a case of meconium peritonitis while considering its etiology, diagnosis, management, and multidisciplinary team care.

Traumatic perinatal events and educational needs of labor and delivery room nurses in Korea: a cross-sectional survey.

PURPOSE: The present study investigated experiences of traumatic perinatal events, the provision of related education, and educational needs of nurses working in the labor and delivery room (LDR). METHODS: Nurses working in the LDRs of six institutions and two nurse portal sites were invited to participate in the survey, delivered on paper or online. The data were collected from October 1 to November 25, 2022. Data from 129 nurses were analyzed using frequency, the chi-square test, the Fisher exact test, the t-test, and analysis of variance. RESULTS: Virtually all participants (98.6%) report

Extending "Helping Babies Breathe" to an Academic Setting in Fiji: A Project Involving American Nursing Faculty and Graduate and Undergraduate Nursing Students.

Approximately 2.5 million neonates died worldwide in 2018. Over one quarter of neonatal deaths are caused by birth asphyxia. Helping Babies Breathe (HBB) was created to teach basic neonatal resuscitation steps in limited-resource settings. Fifteen Fijian faculty members attended a master teacher class. Nine undergraduate nursing students from the western United States assisted in teaching two HBB classes for Fijian nursing students. Fijian faculty and student knowledge increased significantly posteducation. Educational settings provide ideal locations for future nurses to learn and practice ev

A data-driven approach to retinopathy of prematurity prevention leads to dramatic change.

Retinopathy of prematurity (ROP) is a common disease of premature infants. It is characterized by an abnormal proliferation of blood vessels in the developing retina and is the leading cause of childhood blindness in developed countries. This article describes a systematic approach to prospectively reduce the incidence of ROP in a newborn intensive care unit (NICU). We realized the need for improvement when we reviewed comparative data indicating that our rates of ROP were higher than those in NICUs in our local area, across the United States, and around the world. After informal efforts appea

Hypertensive disorders of pregnancy.

Hypertensive disorders occur in 6% to 8% of all pregnancies, are the second leading cause of maternal death, and contribute to significant neonatal morbidity and mortality. This is a problem not only in inpatient settings, as ambulatory and home-care nurses are increasingly being called upon to monitor women who are at high risk and may have hypertensive disorders. To prevent hypertension-induced problems in pregnant women, nurses must have strong assessment, advocacy, and counseling skills. Nurses also must provide care based on the latest national standards as described in this article.

Early postnatal assessment of the newborn in the developing country: Malawi.

The high rate of infant mortality, defined as the number of deaths in the first year of life for every 1000 babies born alive (WHO, 1981), is recognized as an extremely serious problem in the developing world. Worldwide decreases in the number of infant deaths are occurring; however, the gap continues to increase in countries with high socioeconomic baselines and those with low per capita income. This article discusses ways in which relevant health care policies and the use of newborn assessment tools can decrease infant mortality in Malawi Central Africa. The description of the tool, Newborn

Neonatal abdominal X-rays: indications, procedure and interpretation.

Abdominal X-rays (AXRs) are one of the most common imaging modalities used to investigate suspected abdominal conditions in neonates. They are frequently requested for a variety of suspected conditions including necrotising enterocolitis, congenital diaphragmatic hernia, duodenal atresia and Hirschsprung's disease. Nurses play an important role in preparing and supporting neonates who are undergoing an AXR, and in some centres advanced neonatal nurse practitioners may be involved in requesting and interpreting AXRs as part of their enhanced role in patient management. It is therefore important

Infants in Drug Withdrawal: A National Description of Nurse Workload, Infant Acuity, and Parental Needs.

Infants in drug withdrawal have complex physiological and behavioral states, requiring intensive nursing care. The study objectives were to describe acuity, parental needs, and nurse workload of infants in drug withdrawal compared with other infants. The design was cross-sectional and involved secondary nurse survey data from 6045 staff nurses from a national sample of 104 neonatal intensive care units. Nurses reported the care of 15 233 infants, 361 (2.4%) of whom were in drug withdrawal. Three-fourths of hospitals had at least 1 infant in drug withdrawal. In these hospitals, the mean number

Neonatal intensive care: decision making in the face of prognostic uncertainty.

This article discusses ethical decisions faced by parents and physicians in evaluating neonates. The role of the neonatal nurse is to be caretaker of the most vulnerable. The nurse assumes the role of safeguarding the infant's well-being, necessitating both parental engagement and work within an interdisciplinary team.

Comparing N-PASS and NIPS: Improving Pain Measurement in the Neonate.

BACKGROUND: Proper assessment of pain is essential to allow for safe and compassionate care of infants in the neonatal intensive care unit (NICU). The Neonatal Infant Pain Scale (NIPS) used in an urban level IV NICU addresses acute pain but may not adequately measure chronic neonatal pain. PURPOSE: The purpose of this quality improvement study was to improve acute and chronic pain measurements for neonates in an NICU through implementation of the Neonatal Pain, Agitation, and Sedation Scale (N-PASS). METHODS/SEARCH STRATEGY: An evidence search for a comprehensive tool to assess neonatal pain

Infantile Hypertrophic Pyloric Stenosis: An Epidemiological Review.

Infantile hypertrophic pyloric stenosis (IHPS) is the most common gastrointestinal disease among infants. IHPS occurs as an isolated condition or together with other congenital anomalies. Association with genetic and environmental factors, bottle feeding, younger maternal age, maternal smoking, and erythromycin administration in the first two weeks of life has been shown. Familial aggregation has been described and a family history is seen in 47.9 percent of siblings. Infants typically present with projectile vomiting associated with symptoms of failure to thrive. An olive-like mass palpable i
