A skeletal extension bar (SEB) through the skin may be involved with many treatments of orthopedic diseases. A new locked percutaneous device (LPD), LPD-III, was designed for the SEB not only to allow the skin to bind onto the SEB, similarly to the LPD, but also to allow the skin binding to shift up or down the SEB while the body changes weight. Recent supermicrosurgical techniques have made it possible to anastomose lymph vessels to the subdermal venular system. However, because the lymphatic fluid is clear and the vessel is translucent, soft, and fragile, with a thinner wall than blood vessels, supermicrosurgical lymphaticovenular anastomosis is a demanding technique. We studied the complications resulting from using running Mersilene suture in 14 patients who underwent penetrating keratoplasty for keratoconus. 11-0 Mersilene running suture and 12 interrupted 10-0 nylon stitches were used in each procedure and the patients were followed for 22 to 48 months. None of the Mersilene sutures broke, but four were removed in two patients: in one, due to astigmatism or loosening of the suture as a result of sterile ulceration, and in the other, due to herpetic keratitis. We report on 183 patients with urinary stress incontinence treated with bladder-neck suspension plasty according to Zoedler from 1972-1982 using the conventional nylon sling. 86.1% of the patients were continent postoperatively. Long term follow-up to 10 years however showed a continuous decrease of success rate in the first 2 postoperative years (74.9% in the 1st, 67.2% in the 2nd year). One hundred forty-five consecutive frontalis suspension procedures performed during the years 1976 to 1982 at the Wills Eye Hospital for the repair of congenital ptosis were reviewed. PURPOSE: Purpose of the study was to evaluate single versus double running sutures in penetrating keratoplasty for keratoconus with respect to suture loosening.
METHODS: Eighty-eight patients were consecutively operated for keratoconus by the same surgeon with the same surgical technique. For the first 45 patients, a single running 10-0 nylon suture was used. For the remaining 43 patients, double running 10-0 nylon sutures were taken.

A major disadvantage of nylon sutures is the need to remove them post-operatively to prevent suture fracture and irritation. Mersilene (polyester) sutures do not hydrolyse or disintegrate and are in theory superior to nylon. Fifty-two consecutive patients were examined an average of 3 years after uncomplicated extracapsular cataract extraction with corneal sections sutured with interrupted 11/0 polyester. In a controlled, randomized trial in 45 cataract operations a new synthetic absorbable suture (8-0 monofilament Vicryl, made by Ethicon), was compared with monofilament nylon or virgin silk normally used. There was a higher incidence of early complications-hyphaema, shallow anterior chamber, and choroidal detachment (15-4 per cent v. 0 per cent)-with the absorbable suture.
