BACKGROUND: Infantile esotropia inaugurates a series of preternatural eye movements that include dissociated vertical divergence (DVD), primary oblique muscle overaction, and latent nystagmus. Congenital binocular visual loss leads to infantile nystagmus, a seemingly unrelated condition. These disorders seem to defy clinical explanation or nosologic classification. So where do these conditions come from and why are their neurologic pathways so elusive?
METHODS: Analysis of these pediatric ocular motor disorders from the perspective of evolutionary biology and its applicability to humans.

Comorbidity of vestibular and anxiety disorders was suggested by epidemiological studies and, recently, new insights into potential neural circuits which subserve both balance control and emotions, appear to support this hypothesis. In particular, disorienting visual surroundings, such as those generated by full-field moving scenes, equally disrupt postural control of patients with vestibular or panic disorders. In the present study, behaviour of body sway was assessed in response to an optokinetic stimulation by means of static posturography in 20 patients with vestibular neuritis (10 patient

Rhythmic activity of various biological systems and functions have been investigated extensively. However, information about diurnal variations of the vestibular system is scarce. In the present preliminary report, diurnal variations of the spontaneous and induced nystagmus in healthy subjects and in patients with peripheral vestibular disorders have been assessed. Circadian rhythms of the vestibular responses were demonstrated. In some variables significant differences have been found between health and sick examinees. These differences may have practical clinical implications on diagnosis an

OBJECTIVE: This study investigated the influence of pathological eye movements on the recordings of ocular vestibulo-evoked myogenic potential (oVEMP).
METHODS: Ten patients with pathological eye movements of non-vestibular origin (nine congenital nystagmus and one opsoclonus) who had negative MRI result were assigned to Group A. Another 20 vestibular neuritis (VN) patients with spontaneous nystagmus were assigned to Group B. Both groups underwent audiometry, and caloric, oVEMP and cervical VEMP (cVEMP) tests.
RESULTS: In Group A, the caloric, oVEMP and cVEMP tests showed 40%, 55% and 50% ab

A patient with a postoperative fistula of the left posterior semicircular canal is presented. Negative pressure in the external ear canal produced upbeat-torsional nystagmus, which was recorded in three dimensions using binocular scleral search coils. The nystagmus was conjugate, without skew deviation, and its trajectory corresponded to the anatomic axis of the left posterior canal. The current study helps validate Ewald's first law in humans: the axis of nystagmus should match the anatomic axis of the semicircular canal that generated it. This law is clinically useful in diagnosing pathology

Extensive vestibular examination was done in fifty patients with unilateral Menière's disease in the interval between the attacks of vertigo. Horizontal and vertical eye-movements were recorded by electro-oculography. In 80% of the significant difference was found in the slow phase velocity of the nystagmus due to caloric stimulation between the normal and pathological ear (mean difference 41% +/- 24,2). A spontaneous nystagmus was found with closed eyes in 58% of the patients with a mean value of 2,5 degrees/sec +/- 1,6 and a positional nystagmus in 76% with a mean value of 2,3 degrees/sec +/

Acquired periodic alternating nystagmus (PAN) describes a horizontal jerk nystagmus that reverses its direction with a predictable cycle, and is thought to arise from lesions involving the brainstem and cerebellum. We report a 20-year-old patient with PAN who presented with an acute vertiginous episode and developed an involuntary head movement that initially masked the PAN. The involuntary head movements were abolished with a subtherapeutic dose of botulinum toxin to the neck muscles. We propose that the head movements initially developed as a compensatory movement to the nystagmus, to mainta

In order to localize the site of lesion of peripheral vertigo, 3D analysis of nystagmus during peripheral vertiginous attacks was carried out. In comparison between the three components, the horizontal component had the largest ratio in each disease. Spontaneous nystagmus was directed toward the affected side in 3 cases and to the opposite side in 15. In patients with Menière's disease (MD), all subjects had horizontal and torsional components and had almost the same slow phase velocity in these two components. In patients with vestibular neuritis (VN) and Hunt's syndrome (HS), nystagmus was d

Acoustic neurinomas comprise 8% of all primary intracranial neoplasms. The authors reviewed 100 cases of pathologically confirmed acoustic neurinomas and compared tumor size, based on observations at operation, with findings on history and physical examination. Of the 100 patients, 36 had a decreased corneal reflex, 36 had nystagmus, and 8 had papilledema. Ninety-two percent of the patients with nystagmus had brainstem compression from tumor, and unilateral nystagmus when present was predominantly on gaze toward the side of the tumor. A positive correlation was found between tumor size and the

BACKGROUND: Nystagmus in infancy may occur as part of an ocular condition, a neurological disorder or be idiopathic. The objective of our study was to evaluate the main causes for nystagmus in infants aged 0-1 year, the accompanying clinical characteristics and the effectiveness of different diagnostic tests used for reaching definite diagnosis.
METHODS: A retrospective study was conducted on medical records of infants who were seen at a tertiary hospital between 2016 and 2021. Clinical data were obtained including age at presentation, presence of strabismus, refractive errors, auxiliary test

The common denominator of the different meanings of dizziness is a disturbance of spatio-postural orientation, which indicates an imminent danger of fall. Höhenschwindel (fear of hights) with grasping and holding in the primate is instinctive behavior caused by a visual cliff or sudden dizziness. Because of the many possible meanings of dizziness, the case history is important for differential diagnosis. Vertiginous and non-vertiginous dizziness, black out and unsteady gait without vertigo should be distinguished. In addition, the duration and time course of the attack, releasing mechanisms an

PURPOSE: Acquired nystagmus, a highly symptomatic consequence of damage to the substrates of oculomotor control, often is resistant to pharmacotherapy. Although heterogeneous in its neural cause, its expression is unified at the effector-the eye muscles themselves-where physical damping of the oscillation offers an alternative approach. Because direct surgical fixation would immobilize the globe, action at a distance is required to damp the oscillation at the point of fixation, allowing unhindered gaze shifts at other times. Implementing this idea magnetically, herein we describe the successfu

Vertigo is a type of dizziness characterised by the subjective feeling of movement despite being stationary. One in four individuals in the community experience symptoms of dizziness at any given time, and it can be challenging for clinicians to diagnose the underlying cause. When dizziness is the result of a malfunction in the inner-ear, the eyes flicker and this is called nystagmus. In this article we describe the first use of Deep Neural Network architectures applied to detecting nystagmus. The data used in these experiments was gathered during a clinical investigation of a novel medical de

Infantile nystagmus is a debilitating involuntary eye movement disorder often associated with retinal diseases such as congenital stationary night blindness (CSNB). The oscillating eye movements of infantile nystagmus come with reduced visual acuity and strongly impair quality of life. No cure exists for this condition. Previously, we demonstrated that nystagmus in the CSNB mouse model Nyxnob has a retinal cause. Specifically, we found that synchronized oscillations of retinal ganglion cells (RGCs) are transmitted to the accessory optic system, triggering compensatory eye movements. The RGC os

Hyperventilation-induced nystagmus (HVIN) has previously been shown by the senior author to be common in patients with both acoustic neuromas and following resection. The recurrent study's aim was to examine if HVIN was specific for retrocochlear pathology. To test this, the incidence of HVIN in 24 patients with confirmed acoustic neuroma was compared with its incidence in 38 patients with end-organ vestibular disease (defined as a greater than 25% reduction in caloric testing). Hyperventilation was carried out for 90 seconds. The results showed that 58% of the acoustic neuroma group were posi

Vestibular neuritis is one of the most common reasons that someone may experience an episode of acute spontaneous vertigo, with the majority cases impacting only one ear. Cases of bilateral vestibular neuritis are rare and are thought to account for less than 10% of all cases of vestibular neuritis. Skull vibration testing is an efficient means to screen for asymmetry in vestibular function but is still in its infancy in clinical use. The ideal assessment methods and the typical patterns of skull vibration-induced nystagmus are relatively well understood; however, the presentation of skull vib

The head tilt suppression test has been known to be a diagnostic tool to distinguish between centrally originating dizziness and peripheral vertigo, as the latter enhances the natural spontaneous firing rate of the velocity storage system and decreases the time constant; however, the otolith organ is now considered to be a cause of head tilting effect. Therefore we questioned whether the head tilt suppression test is still worth differential diagnostic tool. We measured the time constant and initial eye velocity of pre- and post-rotatory nystagmus in a velocity step test in nine healthy adults
