BACKGROUND: Eye tics are among the most common manifested symptoms of tic disorders, including Gilles de la Tourette's syndrome (GTS).
METHODS: A review of medical literature or ocular signs and symptoms of tic disorders was conducted.
RESULTS: Tic disorders are characterized by repetitive and often disabling movements that primarily involve the eyes, face, neck, and voice apparatus. In addition, patients with tics may have eye problems related to medications used to control tics. Some eye signs are atypical in tic disorders and their presence may suggest alternate diagnoses.
CONCLUSION: Ey

PURPOSE: Acute onset comitant esotropia associated with spasm of accommodation in children and adults is a rare clinical condition. When occurring with pupillary miosis and restricted ocular motility, it is referred to as "spasm of near reflex" (SNR) and may require neurological investigation. The natural history of SNR depends on its etiology. There is little information in the literature regarding the long-term follow-up of SNR and the stability of visual signs and symptoms following cessation of treatment. We report a case of SNR in an otherwise healthy young male, and discuss the presentat

PURPOSE: The percentage of children who are symptomatic has been shown to increase with the number of signs of convergence insufficiency (CI). Our goal was to investigate whether there is a relationship between the severity of the clinical signs of CI and symptom level reported in children with a three-sign symptomatic CI.
METHODS: The Convergence Insufficiency Treatment Trial enrolled 221 children with symptomatic CI from ages 9 to 17 years. Inclusion criteria included the following three signs of CI: (1) exophoria at near at least 4Δ greater than at distance, (2) insufficient positive fusio

The article reviews the ophthalmic literature on spina bifida and hydrocephalus and presents the findings of a six-year survey of 322 children. Ophthalmic complications were found to occur very frequently. Forty-two per cent had a manifest squint, 29% lateral rectus palsy or musculoparetic nystagmus, 14% papilloedema and 17% optic atrophy. Only 27% definitely had normal visual function. Seventy per cent of proven episodes of shunt dysfunctions had positive ophthalmological evidence of raised intracranial pressure. The sudden appearance of a squint, other ocular motility disorder or papilloedem

CLINICAL RELEVANCE: Considering the significant relationship between opioid abuse and some accommodative and convergence disorders, opioid use should be considered in the differential diagnosis and will directly affect the management plan.
BACKGROUND: To determine the prevalence of accommodative and convergence anomalies and their related factors in a population of male young adults with opioid use disorder (OUD).
METHODS: This cross-sectional study was conducted using a convenience sampling method in 2019. The study sample included male young adults with OUD who had been referred to a speci

BACKGROUND: Several studies have suggested that accommodative and non-strabismic binocular dysfunctions are commonly encountered in optometric practice. This study aims to verify whether these findings apply to a Portuguese clinical population.
METHODS: This study included consecutive nonpresbyopic subjects that came to two Portuguese optometric clinics over a period of six months. A complete visual exam was conducted and included the measurement of visual acuity (VA), refraction, near point of convergence (NPC), distance and near phoria, near and distance fusional vergences, amplitude of acc

The visual system interacts synergistically with the vestibular system. A normally functioning vestibulo-ocular reflex is necessary but not sufficient for optimum visual acuity during head motion. Studies of dynamic visual acuity, the acuity achieved during relative motion of visual targets or of the observer, indicate that motion of images on the retina markedly compromises vision. The vestibulo-ocular reflex normally provides a substantial measure of stabilization of the retina during head movements, but purely vestibular compensatory eye movements are not sufficiently precise for optimal vi

Although ocular alterations alone rarely cause vertigo, the ophthalmologist can play an important role in the interdisciplinary context by testing visual function. Assessment of ocular motility is the most important individual examination in the diagnostic evaluation of vertigo. Methods that analyze specific visual functions, like the dynamic visual acuity test, may play an important role in the future. In addition to neuro-ophthalmologic disorders, ocular alterations are also receiving increasing attention. They are postulated to be a key element in the development of multifactorial age-relat

We report a 47-year-old woman who manifested ocular motility disorder, bilateral pyramidal signs, and severe parkinsonism after a ventriculo-peritoneal shunt for non communicating hydrocephalus secondary to idiopathic aqueduct stenosis. The ocular motility disorder consisted of severe vertical gaze palsy and convergence retraction nystagmus. Parkinsonism included not only bradykinesia but also resting tremor and cogwheel rigidity. On the other hand, striatal uptake did not decrease in (18)F-dihydroxyphenylalanine positron emission tomography, and anti-Parkinsonian drugs were not effective. 99m

BACKGROUND: Mild traumatic brain injury (mTBI) can cause persistent symptoms suggestive of oculomotor deficits. This research synthesized evidence on restitutive interventions for reducing oculomotor deficits in adults with mTBI to understand if these interventions have clinical utility for improving recovery.
METHODS: Medline, EMBASE, CINHAL, PsychInfo, and Scopus, databases were searched for experimental studies published in English. We rated risk of bias (RoB) using recommended tools, and the certainty of the evidence according to GRADE guidelines. We conducted meta-analyses for similar ou

Oblique saccades often display component stretching, in which the shorter vector in one cardinal direction is slowed so that its duration matches that of the longer vector in the orthogonal direction, resulting in a straighter trajectory. In internuclear ophthalmoplegia, adducting saccades are typically slowed while vertical saccades are unaffected. It is not known whether these slowed adducting movements are accompanied by adaptive component stretching of the vertical vector during oblique saccades. This was a cross-sectional study. We recorded the saccadic eye movement in 5 patients with rig

BACKGROUND: Video-oculography (VOG) is used to quantify functional deficits in internuclear ophthalmoplegia (INO), whereas MRI can detect the corresponding structural lesions in the medial longitudinal fasciculus (MLF). This study investigates the diagnostic agreement of MRI compared to VOG measurements.
METHODS: We prospectively compared structural MRI findings and functional VOG measures of 63 MS patients to assess their diagnostic agreement for INO.
RESULTS: MRI detected 12 true-positive and 92 true-negative MLF lesions for INO compared to VOG (12 true-positive and 38 true-negative patien

Vertical pendular nystagmus (PN) rarely occurs with acute pontine lesions. To hypothesize a pathophysiology for acute vertical PN, we analyzed the clinical characteristics and quantitative eye-movement recordings of one new case with acute vertical PN and an additional 11 patients from the literature. Most patients had extensive pontine lesions causing either the locked-in syndrome or unresponsiveness, but two conscious patients had focal lesions restricted to the paramedian caudal pontine tegmentum. All patients presented a complete or partial horizontal gaze palsy, and about half showed ocul

Neurometabolic diseases are complex group of rare neurogenetic disorders, which are difficult to diagnose. Patients may have toxic metabolite accumulation, inadequate energy supply, or neurotransmitter deficiency, resulting in a variety of clinical manifestations and severity with enzyme activity or transporter function defects. Multiple organ involvement is frequently seen, among which neurological symptoms and signs are one of the most encountered problems. Ocular motor problems deserve special attention for it occurs in some inborn error of metabolism. Furthermore, some are early signs or c

OBJECTIVES: Oculogyric crisis is a dystonic movement disorder characterized by continuous contraction of the ocular muscles and spasmodic movements of the pupils in a fixed position, usually upward.
METHODS: In this case report, an early-stage acute oculogyric crisis due to low-dose olanzapine in a 15-year-old adolescent boy is presented.
RESULTS: To the best of our knowledge, this is the first report showing that acute oculogyric crisis can develop with low-dose olanzapine administration.
CONCLUSIONS: Even though second-generation antipsychotics are safer than conventional antipsychotics i

A 78-year-old man was evaluated for altered mentation in the setting of significant uremia. On examination, he was found to be encephalopathic with generalized myoclonus and spontaneous opsoclonus. He had no known risk factors for the development of opsoclonus and upon undergoing hemodialysis, experienced near resolution of his eye movement abnormalities, thus highlighting a possible link between the uremic state and opsoclonus.
