In three male patients with Alport's disease, two of them successfully treated by kidney transplantation, ophthalmoscopy revealed bilaterally densely packed whitish-yellow lesions around, and to some extent in, the macular area. These spots, superficially located in the retina, have not been previosly desribed in Alport's disease.

INTRODUCTION: Most infants cry with lid speculum, scleral depression and indirect ophthalmoscopy. This simple observation, "did the infant cry?" during the initial 31-week ROP screening exam was prospectively studied.
METHODS: From Fall 1989 through Summer 2003, all Alaskan infants with birthweight < or = 1500 grams were examined by RWA. After 1992, at the 31-week initial ROP screening, we recorded whether the infant was able to cry (cry), did not cry (quiet), or was intubated (vent) during indirect ophthalmoscopy with lid speculum and scleral depression.
RESULTS: ROP was classified as to th…

PURPOSE: To assess retinal dysfunction when ophthalmoscopy reveals normal features.
METHODS: Ganzfeld electroretinography of the rod and of the cone systems was used to detect congenital or acquired retinal dysfunction. In infants under 5 years of age ERG was performed using inhalational anaesthesia. In addition, multifocal ERG techniques detect discrete local retinal dysfunctions. The ERG results are evaluated in comparison to normative data and to ophthalmoscopic as well as perimetric results. Patient data were selected to demonstrate typical ERG changes and the corresponding differential d…

We report 2 patients with unilateral concurrent optic nerve pit and coloboma complicated by subretinal fluid. Further examination revealed bilateral incomplete retinal peripheral vascularization in 1 case. Laser indirect ophthalmoscopy and the use of carbonic anhydrase inhibitors stabilized fluid levels and prevented worsening visual symptoms in both cases.

Examination of the ocular fundus is a critical aspect of the neurologic examination. For example, in patients with headache the ocular fundus examination is needed to uncover "red flags" suggestive of secondary etiologies. However, ocular fundus examination is infrequently and poorly performed in clinical practice. Nonmydriatic ocular fundus photography provides an alternative to direct ophthalmoscopy that has been studied as part of the Fundus Photography versus Ophthalmoscopy Trial Outcomes in the Emergency Department (FOTO-ED) Study. Herein, the results of the FOTO-ED study are reviewed wit…

When performed by a trained examiner, ultrasonography (US) of the eye is a useful tool in diagnosing conditions of the ocular globe, especially when combined with ophthalmoscopy. Pathologic conditions of the ocular globe include several usual and unusual entities, most of which may be properly identified at US. For instance, the ocular globe may have an abnormal size or unusual morphologic characteristics. Lesions of the anterior chamber (eg, hyphema), lens (eg, cataract, luxation), and iris or ciliary bodies (eg, cysts) are usually seen at ophthalmoscopy but may also be depicted at US. Vitreo…

OBJECTIVE: Although hypopigmented macules are an important manifestation of tuberous sclerosis (TS), the probability of TS in healthy individuals who have hypopigmented macules is unknown. The purpose of this study was to establish the prevalence of hypopigmented macules among a cross section of the general white population.
STUDY DESIGN: The skin of 423 white individuals younger than 45 years of age was screened for hypopigmented macules with ambient incandescent and fluorescent light and a Wood lamp. Indirect ophthalmoscopy was performed in patients with unexplained hypopigmentation to scre…

Clinical evaluation of the disease activity of retinopathy of prematurity (ROP) is routinely performed via indirect ophthalmoscopy and/or widefield fundus photography. Herein we report an infant with advanced ROP imaged with an investigational handheld 400kHz optical coherence tomography angiography system that visualized microvascular details not seen in fundus exam or photography.

Ophthalmoscopy in a somnolent girl of 12 years revealed papilloedema of R = 3D and L = 4D. In the right eye there was an additional retinochoroiditis ad papillam. Seroreactions for toxoplasmosis were highly positive. By treatment with Daraprim, Durenat and Leucovorin, the left eye recovered nearly completely, but the right eye developed a severe optic atrophy.

Retinal pigment epithelium detachment (PED) is a pathological condition in which retinal pigment epithelium is separated from the underlying … rial is contained between the RPE and Bruch's membrane. It was first described by Casswell in 1985. Ophthalmoscopy of the fundus shows a yellow-white cellular structure in the subretinal space, sometimes containing a patch of pigment, with an uneven surface, clear boundaries and a wavy edge. Serous PED is larger in area and prominence than drusenoid, characteriz …

HEALTH POLITICAL BACKGROUND: About 950,000 people are affected by glaucoma in Germany, about 50% of which are undiagnosed. The German Ophthalmological Society and the German Association of Ophthalmologists recommend a screening for glaucoma according to their guidelines. The Federal Joint Committee disapproved a glaucoma-screening program on expense of the compulsory health insurance in 2004.
SCIENTIFIC BACKGROUND: Primary open angle glaucoma is diagnosed by evaluation of the optic disc, the retinal fibre layer and the visual field. The main examinations are ophthalmoscopy, scanning laser pol…

CASE REPORT: Optical coherence tomography features of a sub-internal limiting membrane (ILM) premacular hemorrhage and dome-shaped preretinal membrane secondary to Valsalva retinopathy are described.
COMMENTS: The origin of the membrane (internal limiting membrane or posterior hyaloid) and the location of the blood (sub-ILM or subhyaloid) are difficult to define by ophthalmoscopy.

In two infants hospitalized at the ages of three and one-half months and 11 months because of epileptic seizures, the diagnosis of tuberous sclerosis was reached upon the basis of retinal astrocytomas found in the fundus examination. It is recommended that indirect ophthalmoscopy be performed in all infants and young children referred with signs possibly related to this disease.

Almost two-thirds of patients with giant cell arteritis (GCA) develop ocular symptoms and up to 30% suffer permanent visual loss. We review the three most common mechanisms for visual loss in GCA, describing the relevant ophthalmic arterial anatomy and emphasising how ophthalmoscopy holds the key to a rapid diagnosis. The short posterior ciliary arteries supply the optic nerve head, while the central retinal artery and its branches supply the inner retina. GCA has a predilection to affect branches of posterior ciliary arteries. The most common mechanism of visual loss in GCA is anterior arteri…

Eye irritation assessment is compulsory to anticipate health risks in military personnel exposed to riot control agents such as capsaicin, the principal constituent of oleoresin capsicum, or pepper sprays. The present work investigates certain fundamental yet unaddressed pharmacological manifestations on ocular exposure to capsaicin. Ocular pharmacology of capsaicin was studied using acute eye irritation (AEI), bovine corneal opacity and permeability (BCOP) assay, corneal fluorescein staining and indirect ophthalmoscopy studies, transcorneal permeation, Schirmer tear secretion test, nerve cond…

BACKGROUND: Acute syphilitic posterior placoid chorioretinitis (ASPPC) is a rare manifestation of ocular syphilis. We report on multimodal imaging including ophthalmoscopy, spectral-domain optical coherence tomography (SD-OCT), fluorescein angiography (FA) and indocyanine green angiography (IA) of a case diagnosed with ASPPC.
CASE: A 45-year-old man who was positive for human immunodeficiency virus presented with a 2-week history of visual loss in the right eye.
CLINICAL FINDINGS: Ophthalmoscopy showed a unilateral yellowish lesion involving the macula. SD-OCT revealed absence of the photore…

The following case report describes a 1-year-old female cheetah (Acinonyx jubatus) with bilateral blindness and unresponsive pupils. For comparison, a second healthy 2.5-year-old male cheetah without visual deficits was also examined. Clinical examination of both animals included biomicroscopy, indirect ophthalmoscopy, tonometry, and electroretinography. The young female cheetah showed no menace response, no direct or indirect pupillary light reflex, and no dazzle reflex in either eye. Fundus lesions, as detected by indirect ophthalmoscopy, are described for the female animal. In both eyes, th…

Choroidal coloboma and posterior staphyloma are two clinically distinct entities, with choroidal excavation being a unifying feature. They are associated with early onset cataract which can make ophthalmoscopy difficult. This report studies the transition between the normal and ectatic area in these cases with ultrasound. We evaluate "posterior hump" as a sign of differentiation between these two conditions.
