Perichondrial Roll in Bridging Gap Between Long Process of Incus and Stapes Head in Ossiculoplasty: A New Technique.

BACKGROUND: The erosive effect of chronic otitis media impairs the middle ear's ossicles. Alloplastic materials or autologous incus can be used for ossicular repair. Perichondrial roll is simple to use and saves considerable operative time and cost, especially in developing countries where ossicular prosthesis and bone substitutes are not affordable for the majority of patients. OBJECTIVE: To evaluate the use of the perichondrial roll in the reconstruction of the long process of the incus. SUBJECTS AND METHODS: This study involved 62 patients diagnosed with chronic suppurative otitis media c

[Hyperacusis after tympanoplasty].

A case of hyperacusis after tympanoplasty is presented. A 62-year-old woman experienced unilateral conductive hearing loss for about 60 years after antrotomy in infancy. Neurological or otoneurological symptoms were not evident. Tympanotomy showed a missing incus with discontinuity of the chain. Reconstruction was performed by interposition of a partial ossicular replacement prosthesis (PORP). The postoperative audiogram revealed minimal perceptive hearing loss in the high-frequency field in the operated ear. Several weeks after middle ear surgery, the patient complained of hyperacusis on the

[Adult localized congenital cholesteatoma].

PURPOSE OF THE STUDY: Congenital cholesteatoma is a well-described anatomical and clinical entity. Adult forms are rare. We describe a posterosuperior encapsulated cholesteatoma and compare this case to other infantile and adult forms described in the literature. MATERIAL AND METHODS: A 25-year-old patient with no medical history consulted for left conductive hearing loss. A flat tympanogram was obtained. The temporal bone computed tomographic scan showed a soft tissue density lesion of the middle ear and anterior stapes erosion. A congenital cholesteatoma was discovered during surgical explo

Revision stapedectomy.

During the past decade, the evolution of surgical techniques and philosophy for revision surgery after stapedectomy has accelerated. Lasers, new surgical techniques, and new prostheses now permit the precise identification and reliable correction of the conductive problem, while reducing the risk of postoperative sensorineural hearing loss that plagued nonlaser revision techniques.

Iatrogenic carotid artery pseudoaneurysm presenting with conductive hearing loss.

Pseudoaneurysms of the intracranial internal carotid artery may occur in the setting of carotid artery dehiscence, due to trauma, invasive tumors, or as a complication of surgery. The rare surgical complication of carotid artery laceration in the petrous segment may occur during myringotomy procedures, middle ear operations, or petrous temporal bone surgery. We report a 28-year-old female patient, a case of internal carotid artery pseudoaneurysm as a complication of middle ear surgery. On her follow-up, computed tomography of the temporal bone demonstrated a dehiscent carotid artery, and a pse

Value of digital volume tomography in patients with conductive hearing loss.

Digital volume tomography (DVT) is an extension of panoramic tomography. With this diagnostic technique, characterized by high resolution, a narrow section width (0.125 mm) and three-dimensional display, small pathological processes can be well visualized. Twenty-five patients with the history of a progressive hearing loss were examined with DVT (Accu-I-tomo, Morita, Japan). The results were compared with pre- and intraoperative findings to evaluate the diagnostic value of DVT in cases of erosion of the ossicular chain. With high resolution and artifact-free demonstration of the middle ear and

Isolated Traumatic Dislocation of the Malleus into the External Auditory Canal.

We report a rare case of isolated malleus dislocation into the external auditory canal with lateralized intact tympanic membrane following a head trauma. The patient was a 63-year-old woman who presented at the outpatient department of our institute with hearing loss of 10 years' duration after a bicycle accident. During physical examination of the patient, total dislocation of the malleus-like bony structure into the external auditory canal on the right side was observed. In the computed tomography scan, an isolated malleus dislocation with intact incus-stapes articulation was identified. The

Acoustically functional 3D‑printed middle ear model for prosthesis development and otosurgical training.

BACKGROUND: Cadaver temporal bones are traditionally used in otosurgical training and research, but they have many limitations. The development of novel individualized middle ear prostheses requires easy and durable artificial middle ear models that are as realistic as possible anatomically, functionally, and acoustically. The aim of this study was to create an acoustically and mechanically functional, three-dimensionally (3D) printed middle ear model that overcomes the challenges of cadaver temporal bones while promoting prosthesis development. METHODS: Structures from microcomputed tomograp
