[Early postoperative gustatory dysfunction after middle ear surgery].

Introduction: Preserving maximal quality of life is an important factor in middle ear surgery according to current standards. Taste disturbance is a common postoperative complication, which can be explained with the risk of injury due to the localisation of chorda tympani. The knowledge about this complication is mandatory for optimal surgical decision-making and for patient education as well. Objective: Investigation of early postoperative taste disturbances, to clarify the differences between the impact of different surgical interventions, and the impact of nerve manipulation. Methods: 15 st

Acquisition of basic ear surgery skills: a randomized comparison between endoscopic and microscopic techniques.

BACKGROUND: Endoscopic ear surgery is gaining increasing popularity and has an important impact on teaching middle ear anatomy and basic surgical skills among residents and fellows. Due to the wide-angled views offered, the approach significantly differs from the established microscopic technique. This randomized study compares the acquisition of basic ear-surgery skills using the endoscopic and microscopic technique under standardized conditions. We aim to investigate the required surgical times, attempts and accidental damages to surrounding structures (errors) in surgeons with different tra

Comparison of Middle Ear Visualization With Endoscopy and Microscopy.

INTRODUCTION: The primary goal of chronic ear surgery is the creation of a safe, clean dry ear. For cholesteatomas, complete removal of disease is dependent on visualization. Conventional microscopy is adequate for most dissection, but various subregions of the middle ear are better visualized with endoscopy. OBJECTIVE: The purpose of the present study was to quantitatively assess the improved visualization that endoscopes afford as compared with operating microscopes. METHODS: Microscopic and endoscopic views were simulated using a three-dimensional model developed from temporal bone scans.

[Conservative and surgical rehabilitation of hearing loss].

According to WHO 360 million people are hard of hearing. Hearing disorders are not only seen in elderly but also in children. Clinically we differentiate between hearing disorders concerning the transmission of sound from the outer ear to the cochlea (conductive hearing loss) and disorders with reduced sound perception concerning the inner ear an related structures (sensorineural hearing loss). This article summarizes common surgical and technical possibilities for rehabilitation. For each pathology we graphically illustrate the generic working principle of the rehabilitation including the ind

Thermal effects of endoscopy in a human temporal bone model: implications for endoscopic ear surgery.

OBJECTIVES/HYPOTHESIS: Although the theoretical risk of elevated temperatures during endoscopic ear surgery has been reported previously, neither temperature change nor heat distribution associated with the endoscope has been quantified. In this study, we measure temperature changes during rigid middle ear endoscopy in a human temporal bone model and investigate whether suction can act as a significant cooling mechanism. STUDY DESIGN: Human temporal bone model of endoscopic middle ear surgery. METHODS: Fresh human temporal bones were maintained at body temperature (∼ 36°C). Temperature fluct

[Sanation surgery for the treatment of chronic suppurative otitis media with cholesteatoma].

Results of the surgical treatment of 98 patients presenting with chronic suppurative otitis media (CSOM) and accompanying cholesteatoma. CSOM of large duration turned out to be associated with cholesteatoma in 31.3% of the patients regardless of localization of the tympanic defect. A variety of surgical interventions were undertaken depending on the extension of cholesteatoma including the intrameatal approach (n=75), endaural approach (n=16), postaural approach (n=6), and the combined approach (n=1). Forty two patients presenting with either central or peripheral peroration in the strained pa

Intracranial cholesteatoma: a case report and review.

A cholesteatoma can be a complication of ear infection, eustachian tube dysfunction, prior ear surgery, or tympanic membrane perforation. It is typically associated with otorrhea and conductive hearing loss; sensorineural hearing loss, dysequilibrium, facial nerve paralysis, and altered mental status signify advanced disease. The treatment is surgical and recurrences are common. A case of cholesteatoma is presented that was associated with extension into the posterior fossa and presented with only a headache and localized pain.

Abnormal facial nerve course associated with stapes fixation or oval window absence: report of two cases.

Congenital minor ear malformations are very uncommon. Their etiology can be explained by the embryological development of the middle ear structures. Their classification, diagnosis and treatment pose certain problems. We report on one case of congenital stapes fixation and one case of oval window absence, both associated with an abnormal facial nerve course. Good long-term results show that performing a platinotomy or vestibulotomy in cases of congenital stapes fixation or oval window absence can be a relatively safe procedure, even in presence of deviated facial nerve course. The literature a

Otoplasty.

BACKGROUND: Auricular deformities, specifically, prominent ears, are relatively frequent. Although the physiologic consequences are negligible, the aesthetic and psychological effects on the patient can be substantial. METHODS: Otoplasty techniques are used to correct many auricular deformities, including the prominent ear, the constricted ear, Stahl's deformity, and cryptotia. Various treatments and techniques have been developed for the correction of these deformities, including methods that excise, bend, suture, scratch, or reposition the auricular cartilage. RESULTS: The multitude of dif

Total ossiculoplasty with footplate removal.

OBJECTIVE: We reviewed the results and management of cases in which total ossiculoplasty requires footplate removal. STUDY DESIGN AND SETTING: We conducted a retrospective review of 10 patients who underwent total ossiculoplasty with footplate removal between 1999 and 2002. These 10 patients represented 5.4% of those undergoing stapedectomy (n = 91) and ossiculoplasty (n = 93) during these years at a tertiary otologic referral center. RESULTS: A total of 10 patients were evaluated, with a mean 17-month follow-up. Closure of air-bone gap (ABG) to within 20 dB was achieved in 6 of 10 patients

Stapedius muscle myoclonus.

Myoclonus of the middle ear is a very rare condition. We present the case of a 20-year-old soldier who was exposed to intense artillery noise during a fierce battle and immediately afterward complained of an incapacitating tinnitus in both ears. Microscopic examination of the ears demonstrated rapid rhythmic movements of the tympanic membrane coinciding with the tinnitus, which was also easily heard by the examiner. There was no evidence of palatal myoclonus, and thus the diagnosis of middle ear myoclonus was made. Exploratory tympanotomy confirmed the diagnosis of stapedial muscle myoclonus.

Transpetrosal surgery techniques.

The complex anatomy of the region necessitates thorough study if transpetrosal surgery is to be performed with minimal patient morbidity. Furthermore, by using flexibility, a team approach, and preoperative planning, the surgical procedure can be tailored to the patient, rather than fitting the patient to a fixed surgical procedure. Finally, minimizing patient morbidity is equivalent to preservation of cranial nerves and crucial vascular structures. Close surveillance and judicious postoperative radiotherapy following cytoreductive surgery provides improved patient outcomes compared with radic

Modified retrolabyrinthine approach with partial labyrinthectomy: anatomic study.

This study was undertaken to evaluate the feasability of the modified retrolabyrinthine approach (traditional retrolabyrinthine approach plus resection of the posterior semicircular canal) to expose the entire fundus of the internal auditory canal (IAC). This approach is advocated by its proponents to manage acoustic neuromas reaching the lateral IAC and with the preservation of hearing as the goal. Little anatomic data directly estimate the limitations of this exposure. Measurements were recorded from 25 cadaver temporal bones dissected with this modified approach. The distances were taken be

[Minimally invasive otoplasty].

OBJECTIVE: We have further developed a method, first published in 1967 by Kaye but not widely used later, to correct protruding ears. To our knowledge, clinical experience with a closed otoplasty technique based on a large number of patients has not yet been published. SUBJECTS AND METHODS: Of 442 ears in 4.5 years, 385 (200 patients, 15 one sided) were corrected using the minimally invasive technique. The key part of this technique to such otoplasty is the creation of a naturally appearing antihelix by combining permanent buried sutures and a conservative cartilage-breaking technique, both a

Microsurgical Robotic System for Myringoplasty: Feasibility Study.

OBJECTIVE: Otological surgery requires exceptional precision and advanced surgical skills, particularly for procedures involving the tympanic membrane and middle ear structures. Microsurgical robotic systems have shown promise in enhancing surgical accuracy by minimizing tissue trauma and enabling remote operations. We aimed to evaluate the feasibility and effectiveness of a microsurgical telerobotic system for performing myringoplasty using an animal model. METHODS: This feasibility study was conducted with five rabbits (10 ears). Two surgeons performed myringoplasty using a microsurgical ro
