3D evaluation of maxillary arches in unilateral cleft lip and palate patients treated with nasoalveolar moulding vs. Hotz's plate.

To compare the three-dimensional changes occurring in the maxillary arch during the use of modified pre-surgical nasoalveolar moulding (PNAM) and Hotz's plate. A clinical trial including 32 children with unilateral cleft lip and palate (UCLP), 16 treated with Hotz's plate and 16 with PNAM, was performed. Impressions of the maxillary arches were taken: A. prior to pre-surgical orthopaedics, B. before cheiloplasty and C. after cheiloplasty. Models were digitised using a stereophotogrammetric instrument, and geodesic distances were calculated: anterior, canine and posterior widths of the arch, an

Presurgical orthopedics has no effect on archform in unilateral cleft lip and palate.

OBJECTIVE: Evaluation of the effect of presurgical orthopedics on maxillary archform up to 6 months of age. DeSIGN: Retrospective, single-blinded, case-control study. PARTICIPANTS: Study model sets of 75 infants with consecutive, nonsyndromic, complete unilateral cleft lip and palate (excluding Simonart bands) from 1995 to 2005. INTERVENTIONS: All patients (PSO group, n  =  14; non-PSO group, n  =  61) received lip repair/vomer flap at 3 months and soft palate repair at 6 months by the same consultant surgeon. The two groups were comparable at birth (p > .01) in all archform variables. MAIN

Surgical management of recurrent nasopharyngeal carcinoma after radiation failure at the primary site.

Nine patients have undergone transpalatal resection of recurrent nasopharynx cancer 10 to 56 months following a full course of external beam irradiation. Seven patients were treated for cure, and two were treated palliatively because of nasal airway obstruction, recurrent epistaxis, and severe headaches. Of the patients treated for cure, five are living free of disease 6 to 48 months (mean, 22.2 months) after their surgery. Disease recurred in two patients at 5 and 7 months. Both underwent secondary procedures and are alive with disease at 25 months and 11 months, respectively. The mean hospit

Five-Year Success of Endodontics Plus Vital Pulp Therapy in a Symptomatic Maxillary Molar: A Case Report.

This case report details the integration of nonsurgical endodontic treatment (NSET) and vital pulp therapy (VPT) for symptomatic irreversible pulpitis (SIP) and symptomatic apical periodontitis (SAP) in a mature maxillary molar, with 5-year follow-up. A 23-year-old female presented with sensitivity in tooth #26 due to extensive distal caries. Initial pulpotomy with Five Mineral Oxides (5MO) failed after 1 month, revealing SIP/SAP. Selective NSET was performed on buccal canals (MB1, MB2, DB), while 5MO preserved vitality in the palatal canal. Buccal canals were obturated; the tooth was restored

Maxillary first molar with 8 root canals detected by CBCT scanning: a case report.

Maxillary molars are prone to exhibit some of the most complex root canal configurations in endodontics. These complexities can result in diagnostic dilemmas, as well as management challenges. This case report illustrates how a cone beam computed tomography (CBCT) scan confirmed an unusual maxillary first molar with 3 root canals each in the mesiobuccal and distobuccal roots and 2 canals in the palatal root. This article illustrates how CBCT scanning and a dental operating microscope can facilitate a better understanding of complex root canal anatomies, ultimately allowing the dentist to clean

Results of the Widmaier-Perko palatoplasty in clefts of secondary palate.

The Widmaier-Perko palatoplasty was evaluated in post alveolar clefts. The speech results of 42 Chinese patients revealed 78.5% as having acceptable speech. In post alveolar clefts of the soft palate only 80% had acceptable speech compared to 76% acceptable speech for clefts of the soft and hard palate. This compared favourably with other similar reported studies. No dental occlusal or growth disturbances were noted. There was significant middle ear disease in the patients examined. This type of palatoplasty does not disturb the palatine bone periosteum and may be helpful in preventing growth

Effect of placement of a speech appliance on levator veli palatini muscle activity during blowing.

OBJECTIVE: We have observed clinically that some speakers wearing a speech appliance for correction of velopharyngeal incompetence can blow with variable intensity without nasal air escape. This clinical finding suggests that tightness of velopharyngeal closure may be regulated in accordance with oral air pressure during blowing. The purposes of this electromyographic study were (1) to examine whether levator vell palatini muscle activity can be changed in relation to oral air pressure during blowing when the speech appliance is removed, (2) to clarify whether or not the change is related to t

The impact of early infant jaw-orthopaedics on early speech production in toddlers with unilateral cleft lip and palate.

The purpose of study was to investigate the impact of pre-surgical Infant Orthopaedics (IO) on consonant production at 18 months of age in children with Unilateral Cleft Lip and Palate (UCLP) and to compare the consonant production to that of age-matched children without clefts. The first ten children in a consecutive series of 20 with UCLP received IO and the following ten did not. Both groups had soft palate repair at 6.3 months of age. The cleft in the hard palate was unrepaired. Ten normally developing children without clefts served as controls. Nine children in each group accomplished the

Post maxillectomy rehabilitation and amelioration of quality of life of post-COVID rhinocerebral mucormycosis patients using obturator: A case series.

Mucormycosis, a rare fungal infection seen in diabetes, is now very frequent owing to the deadly triad of COVID-19 infection, diabetes, and rampant use of corticosteroids. Early diagnosis and treatment can reduce the mortality and morbidity of this lethal fungal infection. Treatment principles may include antifungal agents along with surgical debridement or resection. The surgically removed palate can have a devastating effect on the appearance and speech of the patient. Obturators allow patients to eat and drink without any suspicion of food entering the oroantral cavities/pharynx during mast

Presurgical nasoalveolar molding (PNAM) for a unilateral cleft lip and palate: a clinical report.

Cleft lip and palate deformity is a congenital defect of the middle third of the face. Incidence varies from 1:500 to 1:2500 live births. Etiology depends upon hereditary and environmental factors. Restoration of these defects is important not only for functional and esthetic reasons, but also because there may be a positive psychological impact for the patient and parents. The goal of primary closure of the lip for unilateral cleft lip is to ensure a normal and symmetrical lip and nose. Presurgical infant orthopedics has been employed since the 1950s as an adjunctive neonatal therapy for the

Overdentures in partial anodontia: simple solutions for complex problems.

Congenital defects in children and adolescents can present complex and challenging dental problems, including arch discrepancies, palatal defects, and malposed or otherwise disrupted dentition. Traditional treatment is often time-consuming, expensive, and in extensive defects, frequently compromised at best. The overdenture prosthesis, often overlooked, is a relatively simple and expedient solution to many of these severe problems, and the prosthetic techniques are familiar and readily employed by dentists without specialized training. This article reviews the general principles employed in th

An Intraoral Appliance to Retract the Protrusive Premaxilla in Bilateral Cleft Lip Patients Presenting Late for Primary Lip Repair.

The protruding and deviated premaxilla in bilateral complete cleft lip and palate (BCL/P) patients causes a perplexing problem in achieving a tension-free primary lip closure. An innovative noninvasive active intraoral Fixed Pre-Surgical Cleft Orthopedic (FPSCO) appliance is presented that has enabled to successfully reposition the premaxillary segment to a more favorable position prior to the surgical repair in 4 BCL/P patients, who reported late for treatment with a mean age of 18 ± 3 months. All the patients had at least 1 deciduous tooth erupted in 3 cleft segments, 1 premaxilla (PM), and

Modified technique of presurgical infant maxillary orthopedics for complete bilateral cleft lip and palate.

This article introduces technical modifications to the conventional presurgical infant maxillary orthopedics device for newborns with complete bilateral cleft lip and palate, providing procedural simplicity and efficiency as well as therapeutic efficacy. The modifications incorporate a wax block-out on the stone model prior to device fabrication in a manner that the need for periodic acrylic addition and removal is not required, and thus eliminates the risk of natural maxillary growth restriction during infant maxillary orthopedics treatment. The premaxilla is completely excluded from the acry
