Arthroscopic repair of medial patellofemoral ligament avulsion in acute patellar dislocation in skeletally immature patient--a case report.

Acute patellar dislocation is a severe injury to the knee and usually manifests with symptoms such as giving way, a sensation of lateral displacement of the patella accompanied by hemarthrosis and tenderness over the medial epicondylar region to palpation. Spontaneous reduction of the patella makes the diagnosis more difficult in initial evaluation. Radiographs often show a small bony avulsion at the medial border of the patella, which may represent an injury to the medial restraints. Magnetic resonance imaging may show a bone bruise on the medial patella, and/or on the lateral femoral condyle

Techniques for reducing anterior knee symptoms after anterior cruciate ligament reconstruction using a bone-patellar tendon-bone autograft.

Seventy-five patients underwent unilateral anterior cruciate ligament reconstruction with an ipsilateral bone-patellar tendon-bone autograft at our institution. The graft was harvested using a two-transverse-incision technique, and patellar and tibial bony defects were repaired with cored bone grafts collected by reaming the femoral socket and the tibial socket or tunnel. We evaluated the incidence of anterior knee pain, donor site tenderness, and sensory disturbance after use of these procedures. We also analyzed the correlation between anterior knee pain and age, sex, bone plug length, range

Anterior cruciate ligament (ACL)-deficient knee with degenerative arthrosis: treatment with an isolated autogenous patellar tendon ACL reconstruction.

We evaluated 58 patients (mean age 30.4 years) who had undergone an isolated anterior cruciate ligament (ACL) reconstruction for chronic instability (mean time from injury to surgery, 8.2 +/- 5.2 years) and showed radiographic evidence of degenerative arthrosis. Objective evaluation at a mean of 4.1 years postoperatively included KT-1000 arthrometer stability, range of motion, and quadriceps muscle strength testing. Subjective analysis at a mean of 5.5 years postoperatively included rating of pain, stability, activity level, and a total score both preoperative and postoperative. Patients were

Distal Femoral Osteotomy and Medial Patellofemoral Ligament Reconstruction for Patellar Dislocation in Hereditary Multiple Exostoses: A Case Report.

CASE: A 44-year-old man with hereditary multiple exostoses (HME) presented with recurrent patellar dislocation of the left knee. Radiographs revealed valgus deformity and trochlear dysplasia. The patient underwent lateral opening-wedge distal femoral osteotomy with medial patellofemoral ligament reconstruction using FiberTape and knotless SwiveLock anchors. Postoperatively, patellar tracking normalized with no recurrence. CONCLUSIONS: This case highlights the need to address bony alignment and medial soft-tissue insufficiency concurrently in complex patellofemoral instability, particularly in

Total knee arthroplasty and medial patellofemoral ligament reconstruction for knee osteoarthritis with habitual patellar dislocation: a case report.

BACKGROUND: Knee osteoarthritis and habitual patellar dislocation are clinically common osteoarticular diseases. However, existing literature lacks reports on their co-occurrence and treatment approaches. Thus, we propose a surgical approach that integrates total knee arthroplasty with medial patellofemoral ligament reconstruction, addressing knee osteoarthritis alongside habitual patellar dislocation. CASE PRESENTATION: A 78-year-old female developed habitual lateral patellar dislocation of the left knee after an accidental fall over 20 years ago. One year prior, she experienced weight-beari

Isolated medial patellofemoral ligament reconstruction improves static bipedal balance control in young patients with recurrent lateral patellar instability.

BACKGROUND: Knee stability can be safely and reliably restored using medial patellofemoral ligament (MPFL) reconstruction, which is widely recognized in patients with recurrent lateral patellar instability. However, the literature regarding its influence on static balance control is limited. Thus, this study aimed to assess the impact of MPFL reconstruction on balance control and determine its functional significance. METHODS: The study comprised 26 patients with recurrent lateral patellar instability, scheduled for MPFL reconstruction, and 26 matched healthy controls who underwent double-leg

Editorial Commentary: Medial Patellofemoral Ligament Reconstruction for Recurrent Patellar Instability-Single or Double Bundle: Does It Really Matter?

Medial patellofemoral ligament reconstruction is considered by most surgeons to be the standard of care for patients with recurrent lateral patellar instability, although the choice of how and when to address concomitant bony pathology (trochlear dysplasia, patella alta, or coronal-plane malalignment-elevated tibial tubercle-trochlear groove distance) remains unclear. Medial patellofemoral ligament reconstruction works to re-establish the primary static restraint to lateral translation of the patella and reduce the risk of recurrent dislocation. Regardless of graft choice or construct, this op

Long-term follow-up after anterior cruciate ligament reconstruction using a press-fit quadriceps tendon-patellar bone autograft.

BACKGROUND: The use of a quadriceps tendon-patellar bone (QTPB) autograft provides an alternative procedure in primary reconstruction of the anterior cruciate ligament (ACL). Using the press-fit technique for femoral fixation and knotting over a bone bridge as well as additional spongiosa filling for tibial fixation can completely eliminate the need for fixation implants. The objective of this study was to evaluate the long-term clinical, functional and radiological results of this operating method. METHODS: Sixty-nine patients (27 female-42 male) were included in this study. Fifty-seven pati

Medial Closing-Wedge Distal Femoral Osteotomy with Medial Patellofemoral Ligament Imbrication for Genu Valgum with Lateral Patellar Instability.

A varus-producing distal femoral osteotomy (DFO) is an effective technique for the treatment of lateral patellar instability (LPI) in patients with concomitant moderate to severe valgus malalignment. Patellar maltracking and subluxation are corrected via neutralization of some of the laterally directed forces on the patella due to the valgus deformity. This can be accomplished with a distal femoral lateral opening-wedge or medial closing-wedge osteotomy and medial soft tissue stabilization. A medial closing-wedge osteotomy offers the advantages of immediate weight bearing and a single incision

Patellar resurfacing in posterior cruciate ligament retaining total knee arthroplasty (PATRES): design of a randomized controlled clinical trial.

BACKGROUND: Anterior knee pain may occur after total knee arthroplasty (TKA). Patellar resurfacing, which is considered to lower the incidence of anterior knee pain after TKA, remains controversial. In the present study clinical and radiological outcomes after TKA performed on patients with clinical and radiological signs of femorotibial and patellofemoral osteoarthritis (OA) with and without patellar resurfacing will be compared. METHODS/DESIGN: Fifty patients will be included in a randomized controlled trial. Patients scheduled for TKA with clinical and radiological signs of femorotibial an

The transtibial versus the anteromedial portal technique in the arthroscopic bone-patellar tendon-bone anterior cruciate ligament reconstruction.

The transtibial (TT) drilling of the femoral tunnel in the bone-patellar tendon-bone (BPTB) anterior cruciate ligament (ACL) reconstruction was found to place the tunnel non-anatomically. The use of the anteromedial portal (AMP) for the femoral drilling would provide the surgeon with more freedom to anatomically place the tunnel in the real femoral ACL footprint. The purpose of this study was to compare the clinical outcomes of BPTB ACL reconstruction using the AMP or the TT technique for the femoral tunnel drilling. A Medline search was not able to identify any study directly comparing the cl

Two-strand posterior cruciate ligament reconstruction with a quadriceps tendon-patellar bone autograft: technical considerations and clinical results.

In patients with complete posterior cruciate ligament (PCL) ruptures in which 10 mm or more of increased posterior tibial translation is detected, associated knee disorders must be treated at the same time as the PCL injury. Varus malalignment and any abnormal medial or lateral ligament deficiency must be corrected before or with PCL reconstruction to avoid the risk of failure of the PCL graft. Gait retraining is recommended for patients with abnormal knee hyperextension patterns before ligament reconstruction is performed. Advances in PCL reconstruction include the use of a two-strand graft t
