[The unsightly chest: Analysis and anomaly. Curative or palliative approach?].

Anterior chest wall deformity are mostly represented by pectus excavatum, which is a depression of the chondrosternal plastron from the 3rd to the 7th pairs of the costal cartilages, then by pectus carinatum which conversely represents a protrusion of this plastron. The major esthetic and psychosocial impact is not to be demonstrated anymore whereas the cardiopulmonary functional impact remains still highly debated. Regarding the management, curative surgical techniques such as Wurtz's sub-perichondrial simplified sternochondroplasty or Nuss' minimaly invasive technique are opposed to palliati…

Chest wall deformities in children.

Pectus excavatum and pectus carinatum deformities are not uncommon. Pectus excavatum is associated with definite demonstrable adverse physiologic effects. The psychologic impact of these deformities on the patient can be truly devastating. The clinician must realize the impact and importance of the advice given to patients and their families. Although correction of these deformities is a major surgical undertaking, it is well tolerated, safe, and usually produces a satisfactory result.

Repair of the pectus deformity: results of the Ravitch approach in the current era.

BACKGROUND: Recent publications have advocated a minimally invasive approach to repair of the pectus deformity. Efforts to evaluate this new … action score was 4 on a scale of 1 to 5, at an average of 4.75 years after repair. The subset of 25 pectus carinatum patients had a mean LOS of 2.4 days and a median patient satisfaction score of 5. CONCLUSIONS: The modified Ravitch procedure yields excellent results with low morbidity, hospital LOS, and cost, combined with high patient satisfaction. These curren …

Minimal cosmetic revision required after minimally invasive pectus repair.

BACKGROUND: Despite surgical correction procedures for pectus deformities, remaining cosmetic asymmetry may have significant psychological effects. We sought to evaluate factors associated with plastic surgery (PS) consultation and procedures for these deformities at an academic institution. METHODS: We reviewed patients aged 0-21 diagnosed with a pectus excavatum or carinatum deformity at our institution between January 2001 and October 2016. Pectus diagnoses were identified by ICD-9/ICD-10 codes and surgical repair by CPT codes; patients receiving PS consultation were identified by clinical

Anterior chest wall deformities and congenital heart disease.

Pectus excavatum and pectus carinatum usually exist as isolated abnormalities. Only 19 cases of associated congenital heart defects have been reported. Significant complications related to uncorrected pectus excavatum have been described either during or after cardiac operations. Therefore we reviewed our experience with these coexisting lesions to assess the risk of surgical repair of chest wall deformities before and after correction of congenital cardiac anomalies. Among 20,860 infants and children with congenital heart disease seen at our institution, 36 (0.17%) had associated anterior tho…

The pectus care guidelines: best practice consensus guidelines from the joint specialist societies SCTS/MF/CWIG/BOA/BAPS for the treatment of patients with pectus abnormalities.

Pectus defects are a group of congenital conditions found in approximately 1 in 250 people, where the sternum is depressed back towards the spine (excavatum), protrudes forwards (carinatum) or more rarely is a mixture of both (arcuatum or mixed defects). For the majority of patients, it is well tolerated, but some patients are affected psychologically, physiologically or both. The deformity becomes apparent at a young age due to the growth of the ribs and the cartilage that links them to the sternum. The majority of defects are mild and are well tolerated, i.e. they do not affect activity and

A case of ruptured descending thoracic aortic aneurysm into the right pleural cavity: importance of preoperative drainage of the right pleural cavity.

We present an unusual case of a ruptured descending thoracic aortic aneurysm into the right pleural cavity of a patient with pectus carinatum. The presence of pectus carinatum played an important role in the development of the aneurysm at the atypical site and the rupture into the right pleural cavity. A small amount of right pleural bleeding on admission can increase and develop to massive hemothorax until emergency operation. Massive bleeding in the right pleural cavity where the dependent lung is located causes atelectasis and increased shunt fraction under one lung ventilation. Therefore, …

Reconstruction of congenital chest wall deformities using solid silicone onlay prostheses.

Congenital chest wall deformities include five types: pectus excavatum (funnel chest), pectus carinatum (pigeon breast), Poland's syndrome, defects of sternal fusion, and miscellaneous dysplasias and skeletal disorders. Of these five types, two, pectus excavatum and Poland's syndrome, are defects of the skeletal chest wall. These two specific anomalies comprise the vast majority of congenital defects of the chest wall and, as depression deformities, are readily amenable to surgical correction.

Pectus deformities in children: a pictorial review.

Pediatric chest wall deformities, such as pectus excavatum (PE), carinatum (PC), and arcuatum (PA), may result in psychosocial and cardiorespiratory morbidity. Pediatric radiologists play a key role in imaging-based characterization, preoperative assessment, and the evaluation of postoperative complications. This pictorial essay highlights key imaging characteristics, indices for surgical planning, and common postoperative findings for pectus deformities.

Congenital Chest Wall Deformities in Children: A Narrative Review.

Congenital chest wall deformities encompass a broad spectrum of anomalies, from the common pectus excavatum and pectus carinatum to rare, life-threatening conditions such as sternal clefts, ectopia cordis, pentalogy of Cantrell, and asphyxiating thoracic dystrophy (Jeune syndrome). This narrative review synthesizes current evidence on their epidemiology and pathogenesis, genetics and syndromic associations, diagnostic assessment, and the full range of conservative and surgical management, with particular attention to developments of the past decade. Pectus deformities affect roughly 1% of chil…

Physiotherapy as an adjuvant to the surgical treatment of anterior chest wall deformities: a necessity? A prospective descriptive study in 21 patients.

PURPOSE: The authors postulated that physiotherapy as an adjuvant to the surgical treatment of anterior chest wall deformities is only indicated if specific abnormalities can be found that could be corrected by physiotherapy. The purpose of this study is to investigate whether such abnormalities can be found and to evaluate their course during a postoperative period of 18 months. METHODS: Twenty-one patients, 16 with pectus excavatum and 5 with pectus carinatum, were evaluated 6 weeks before and 6 weeks, 6 months, and 18 months after surgical correction. Postural impairments, spinal mobility …

Preliminary results of orthotic treatment of pectus deformities in children and adolescents.

Fifty-five children and adolescents with different types of pectus deformities and a minimum follow-up of 1 year were assessed to determine the effectiveness of nonoperative treatment with a dynamic chest compressor orthosis. Thirty-seven patients were treated and compared with 18 untreated patients. The treated patients had a better outcome. Reshaping of the contour of the chest wall was observed in 21 patients with inferior and lateral types of pectus carinatum. The dynamic chest compressor has minimal morbidity and is a valuable treatment option of pectus deformities in the growing period.

[Surgical treatment of thoracic wall malformations].

Anterior chest wall deformities in children are the expression of some congenital malformation such as Poland's syndrome and pectus excavatum. The surgical treatment of these malformations affecting children is aimed at correcting large wall defects and stabilizing the chest wall, assuring at the same time chest harmonious growth and postoperative functional assessment. We present the results of the treatment of 2 cases of Poland's syndrome, 20 cases of pectus excavatum and 2 cases of pectus carinatum, with a follow-up of 6 months--9 years.

Pectus deformities of the anterior chest wall.

Pectus deformities affect about 1% of the population. Despite a subjective reduction in exercise tolerance, objective physiological disturbance is very uncommon. However, there is no doubt that cultural changes have significantly altered perception and tolerance of these deformities and increasing numbers of children are actively seeking surgery. Pectus excavatum is readily amenable to correction using minimally invasive techniques and pectus carinatum can be corrected using safe established conventional techniques. Good or excellent results can be expected in over 90% of cases, and children s…
