Dedifferentiated endometrial cancer: an atypical case diagnosed from cerebellar and adrenal metastasis: case presentation and review of literature.

Dedifferentiated endometrial cancer (DEC) is microscopically characterized by the presence of high-grade areas emerging from low-grade tumour. DEC is an aggressive tumour even when the dedifferentiated component represents only 20% of the entire neoplasm. A proper histological diagnosis is essential to define the most appropriate therapeutic approach for these tumors, since they are characterized by a particularly aggressive trend and by an extremely poor prognosis. We report a single case of DEC associated with dedifferentiated and adrenal metastasis, for which the patient underwent both abdo

Alternative surgical approaches for aggressive angiomyxoma at different sites in the pelvic cavity.

Aggressive angiomyxoma, a rare soft tissue benign neoplasm, predominantly occurs in the female pelvic peritoneum and perineum region during reproductive age. It is slow growing, locally infiltrative, and has a high risk of local recurrence and the neoplastic character of blood vessels. The standard treatment is surgery. We report three unusual aggressive angiomyxoma cases. The first case was a pedunculated mass of the left labium major; the second, a left perineal mass that infiltrated into the paravesical area via the obturator foramen; and the third, a big mass in the retroperitoneal cavity,

Isolated sigmoid colon metastasis from a primary fallopian tube carcinoma: a case report.

Isolated metastasis of primary fallopian tube carcinoma (PFTC) is extremely rare. We describe a case of a 41-year-old asymptomatic woman who was referred three years after the initial treatment for PFTC due to elevated sertum CA-125 levels. The abdominal and pelvic CT scans revealed a pelvic mass near the top of the vaginal vault. On surgery, a sigmoid colon tumour was found and a sigmoidectomy was performed. On histopathology the tumour involved the bowel wall from serosa to submucosa, without involvement of the underlying mucosa. Immunohistochemical staining was positive for cytokeratin 7 an

Primary leiomyosarcoma of the fallopian tube.

Primary sarcoma of the fallopian tube is a very rare neoplasm. We report the case of a 69-year-old woman affected with leiomyosarcoma of the left fallopian tube. Her chief complaint was lower abdominal pain. The preoperative diagnosis was a left adnexal malignant tumor based on pelvic examination, abdominal computed tomography, and magnetic resonance imaging. Following a laparotomy, she was ultimately diagnosed with a FIGO IIc fallopian tube leiomyosarcoma. She was treated with total abdominal hysterectomy, bilateral salpingo-oophorectomy, pelvic lymph node dissection, partial omentectomy, and

Acute chlamydial salpingitis with ascites and adnexal mass simulating a malignant neoplasm.

Chlamydia trachomatis is a well-known cause of acute and chronic salpingitis, accounting for approximately half of all cases of pelvic inflammatory disease. Typically, patients with acute chlamydial salpingitis present with acute lower abdominal pain, tenderness on bimanual pelvic examination, or vaginal discharge. We describe a case of acute chlamydial salpingitis with marked ascites and an adnexal mass that simulated a malignant neoplasm. Microscopically, a severe lymphofollicular salpingitis and a marked lymphofollicular hyperplasia of the omentum and retroperitoneal lymph nodes were found.

Pathologic findings and clinical outcomes in women undergoing risk-reducing surgery to prevent ovarian and fallopian tube carcinoma: A large prospective single institution experience.

OBJECTIVES: Risk-reducing salpingo-oophorectomy (RRSO) is recommended for women at increased risk of ovarian, fallopian tube (FT), and peritoneal carcinoma (collectively OC). We describe rates of occult neoplasia in the largest single-institution prospective cohort of women undergoing RRSO, including those with mutations in non-BRCA homologous repair (HRR) genes. METHODS: Participants undergoing RRSO enrolled in a prospective tissue bank between 1999 and 2017. Ovaries and FTs were serially sectioned in all cases. Participants had OC susceptibility gene mutations or a family history suggesting

18 F-FDG PET/CT Findings in a Patient With Primary Sarcomatoid Carcinoma of the Uterus.

Primary sarcomatoid carcinoma of the uterus is a rare and highly malignant neoplasm with a poor prognosis. Herein, we reported a case of uterine sarcomatoid carcinoma in a 55-year-old woman with colporrhagia for 1 month. 18 F-FDG PET/CT showed intense 18 F-FDG uptake of the uterine tumor with hypermetabolic pelvic lymph node and pulmonary nodules. Histopathology and immunohistochemistry of the uterine lesion tissue confirmed the diagnosis of sarcomatoid carcinoma after needle biopsy.

Colorectal infiltrating deep endometriosis: Laparoscopic treatment. A case report.

Endometriosis is a disease affecting approximately 10% of women of fertile age. A particular presentation is deep infiltrating endometriosis of the rectosigmoid colon with symptoms that can mimic an intestinal obstruction or neoplasm. We report the case of a 39-year-old woman with pelvic pain during the menstrual cycle and significant anemia who presented an ectopic endometrial tissue in correspondence of the rectum. Because of the thickness of the lesion the patient underwent a segmental laparoscopic colorectal resection with end-to-end anastomosis.

Tension pneumoperitoneum: a report of 4 cases.

Four cases of tension pneumoperitoneum are described. In 3 patients this condition followed a perforation of a grossly distended caecum. In 2 of these patients there was an associated malignant neoplasm of the pelvic colon with obstruction. The third patient had a pseudo-obstruction of the transverse colon. The fourth patient had a tension penumoperitoneum with associated surgical emphysema in the neck and subcutaneous tissues of the abdomen and chest walls, following perforation of a duodenal ulcer. The aetiology, presentation and management, together with the mechanism of tension pneumoperit

Pelvic desmoid fibromatosis: a diagnostic and therapeutic challenge.

Pelvic desmoid fibromatosis is a rare locally aggressive benign neoplasm, typically presenting in the reproductive age group with lower abdominal pain. Extra-abdominal occurrence being predominant, the pelvic origin of the tumour makes the clinical management a multidisciplinary challenge. Previous surgeries, current or previous pregnancies, and familial adenomatous polyposis have been documented as associated risk factors. Our patient was a young woman with three previous caesarean sections, admitted with a large infiltrative lesion extending into the left pelvic sidewall up to the pelvic bon

Oncocytic adrenocortical neoplasm arising from adrenal rest in the broad ligament of the uterus.

Oncocytic adrenocortical neoplasm is characterized by abundant eosinophilic cytoplasm containing mitochondria, occasional nuclear atypia and diffuse growth pattern. Oncocytic adrenocortical neoplasm arising in adrenal rest is, however, extremely rare. We report a case of oncocytic adrenocortical neoplasm arising in adrenal rest of the broad ligament with associated marked lipomatous metaplasia. A well circumscribed tumor was accidentally detected in the pelvic cavity of a 29 year old Japanese woman, adjacent to the broad ligament of the uterus. The tumor was composed of large eosinophilic cell

Multiseptated Cystic Mass in the Pelvic Cavity: A Case Report of a Leiomyoma with Cystic Degeneration Mimicking a Peritoneal Inclusion Cyst.

BACKGROUND: Uterine leiomyomas are common benign neoplasms, but atypical presentations such as extensive cystic degeneration are rare and can pose significant diagnostic challenges. When pedunculated and predominantly cystic, these tumors can closely mimic other pelvic pathologies, particularly Peritoneal Inclusion Cysts (PIC). CASE PRESENTATION: A 23-year-old nulliparous woman presented with lower abdominal pain. Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) revealed a large multiseptated cystic mass in the pelvic cavity, located posterolaterally to the uterus. An adjacent sm

Fallopian tube origin of supposed ovarian high-grade serous carcinomas.

INTRODUCTION: Serous carcinomas are the most frequent histologic type of ovarian and peritoneal cancers, and can also be detected in the endometrium and fallopian tubes. Serous carcinomas are usually high-grade neoplasms when diagnosed, yet the identification of an associated precursor lesion remains challenging. Pathological examination of specimens obtained from prophylactic bilateral salpingo-oophorectomies that were performed for patients harboring BRCA1/2 mutations suggests that high-grade serous carcinomas may arise in the fallopian tubes rather than in the ovaries. OBJECTIVE: To invest

Neurogenic tumors of the abdomen and pelvis: imaging features, differential diagnosis, and diagnostic pitfalls.

Neurogenic tumors of the abdomen and pelvis are a diverse group of neoplasms arising from paraganglia, peripheral nerves, and autonomic ganglia. Their imaging appearances range from benign incidental lesions to aggressive neoplasms with metastatic potential, creating diagnostic challenges because of overlapping imaging features and broad differential diagnoses. Cross-sectional imaging, supplemented by functional imaging, plays a central role in lesion characterization, staging, and surgical planning. This review summarizes the imaging features of paraganglion cell-derived, nerve sheath-derived

Ewing sarcoma of the cervix: an unusual site of presentation.

Ewing sarcoma is an exceedingly rare form of cancer that affects the cervix. It falls within the spectrum of neoplastic diseases known as Ewing's family of tumours, typically observed in osseous tissues. A woman in her 40s, experiencing symptoms of leucorrhoea and transvaginal bleeding that commenced 3 months before her consultation, was referred to our gynaecological oncology clinic with a preliminary diagnosis of ovarian teratoma. A colposcopy procedure was conducted unveiling a complete loss of cervical anatomy with friable and malodorous tissue. Pelvic ultrasound identified a lesion of unc

A rare disease with pregnancy: Castleman case report.

Castleman's disease was first described by Castleman et al. in 1956 as a non-lymphoproliferative disease.1 Castleman's disease (CD), or angiofollicular lymphoid hyperplasia, is a rare disease with unknown etiology that can be easily misdiagnosed as lymphoma, neoplasm, or infection. Very few cases of pelvic origin and observed in pregnancy have been reported in the literature and are usually asymptomatic. Preoperative diagnosis is very difficult due to nonspecific imaging findings and rarity; most cases are diagnosed based on postoperative pathological examination. In this paper, a case of a 36

[Mixed epithelial and stromal tumor growing with polypoid pattern in the renal pelvis].

A 68-year-old woman was found incidentally to have right hydronephrosis and a renal pelvic mass by abdominal ultrasonography. Radiographic examinations revealed a heterogeneous renal pelvic tumor, and right nephroureterectomy was performed. The tumor was well circumscribed yellow-whitish solid mass with scattered cysts. Histologically, the tumor was composed of both mesenchymal and epithelial components. The mesenchymal elements consisted of fibroblasts and smooth muscle cells, and the epithelial elements of cystic and tubular structures lined by cuboidal epithelium. Atypia and mitoses were no

Application of 99m Tc-FAPI-46 SPECT/CT in Evaluation of Idiopathic Retroperitoneal Fibrosis: Case Report of a Novel Promising Experience.

This case report describes a 59-year-old man with fatigue and bilateral lower limb edema, diagnosed with retroperitoneal fibrosis (RPF). Laboratory tests showed elevated serum creatinine, ESR, and CRP levels, while imaging revealed left hydroureteronephrosis and a pelvic mass encasing the iliac vessels. A biopsy indicated fibrotic changes without malignancy, confirming RPF. This experience of using 99m Tc-FAPI-46 as a novel agent for conventional imaging demonstrated a high target-to-background ratio in the RPF, suggesting its utility in diagnosis and assessing disease activity, also aiding in
