PURPOSE: Peyronie 's disease is an acquired connective tissue disorder of the penile tunica albuginea with fibrosis and inflammation. The patient was injured when he forcibly bent his erect penis and heard a dull sound different from the"usual click sound"in the morning. Priapism is a pathological and mostly painful permanent penile erection. Typically only both corpora cavernosa and not the corpus spongiosum urethrae and the glans penis are involved. The vascular and nervous preconditions for a normal erection are discussed and a new concept presented. On this basis the conditions for a priapism are enumerated and the principles of therapy are discussed.

We injected 20 micrograms of prostaglandin E1 (PGE1) intracavernously in 32 patients with impotence, we then examined penile blood flow using a Xe-133 washout technique. In addition, we also performed a cavernosography. After PGE1 injection, 18 patients (56%) showed complete erection, 10 (31%) tumescence, and 4 (13%) no response. Penile fracture is a rare urologic emergency. The main finding is a partial disruption of one or both cavernosal bodies due to blunt trauma of the penis during an erection. Complete or partial injury of the urethra may accompany the penile fracture but complete urethral rupture is rarely encountered. In this study, we present the management of a penile fracture case with disruption of both corpus cavernosum with total urethral rupture.
