The co-occurrence of alcoholism with other psychiatric disorders in the general population and its impact on treatment.

It is apparent from previous studies in clinical populations that there is a high comorbidity rate between alcoholism and other psychiatric diagnoses. However, this may simply be an expression of Berkson's bias (i.e., an increased tendency for persons with multiple diagnoses to seek and receive treatment and thus fall into study populations drawn from treatment sources). In this article, we use data from the Epidemiologic Catchment Area survey to examine the comorbidity between alcohol abuse and dependence, other substances of abuse and nonsubstance psychiatric disorders in a sample of approxi

[Forensic psychiatric examinations of the mentally retarded].

3,343 forensic psychiatric examinations were recorded in Norway from 1980 to 1996. 294 patients were diagnosed as mentally retarded. The annual number of mentally retarded persons increased during the observation period, but the relative percentage remained stable. The majority of the retarded persons being discussed had been charged with sexual felony, and some with arson. About half had previous convictions. Few had problems with alcohol or other intoxicants. Every fourth retarded person examined was considered not only to be mentally retarded, but also to suffer from other psychiatric disor

Passageway: A Novel Approach to Success of Conditional Release - Principles and Constructs of the Model Residential Program for the Forensic Mentally III Patient.

With the advent of psychotropic medications and with the deinstitutionalization of psychiatry starting in 1968, patients were prematurely discharged from forensic state hospitals. Due to lack of resources, psychiatric forensic patients ended up in the correctional system or homeless with the reduction of psychiatric beds in forensic and civil state hospitals. Lacking proper training and medication management, the recidivism rate of this population was close to 10% for rearrest and about 35% for revocation of conditional release (CR; Manguno-Mire et al., ). A new treatment modality was created

Nicotine dependence, major depression, and anxiety in young adults.

To determine whether nicotine dependence, classified by level of severity, was associated with other substance dependence, major depression, and anxiety disorders, we studied a random sample of 1007 young adults in the Detroit (Mich) area using the National Institute of Mental Health Diagnostic Interview Schedule, revised according to DSM-III-R. The systematic coverage of DSM-III-R criteria of nicotine dependence provides an unprecedented opportunity to separate persons with nicotine dependence from the larger class of persons with a history of smoking and to examine the prevalence of psychiat

Elemental psychopathy assessment's nomological net: A meta-analytic review.

Psychopathy is a longstanding construct of great clinical interest, marked by traits such as Callousness, manipulativeness, and impulsivity. The Elemental Psychopathy Assessment (EPA; Lynam et al., 2011) was developed to anchor the measurement of psychopathy within the five-factor model of personality. This preregistered study presents a meta-analytic review of the EPA's construct validity, examining its relations at the domain and total levels with other psychopathy measures, personality traits, and psychopathological outcomes. Drawing from 50 studies across 38 articles, over 3,500 effect siz

Long-term treatment of chronically suicidal patients.

Chronically suicidal persons with chronic psychiatric disorders (dysthymia, recurrent depression, alcoholism/addiction, schizophrenia, personality disorders) challenge the experience and resourcefulness of psychiatrists. The author reviews his 30 years of experience with these patients and makes six recommendations for long-term treatment: (1) a team approach using consultants and ancillary therapists, (2) flexible therapeutic plans combining medication with psychotherapy, (3) care in monitoring transference and countertransference, (4) brief hospitalization at turning points in the patient's

Psychiatric and substance use comorbidity among treatment-seeking opioid abusers.

BACKGROUND: Major studies of psychiatric comorbidity in opioid abusers reported rates of comorbidity that far exceeded general population estimates. These studies were published more than a decade ago and reported on few women and few substance use diagnoses. METHODS: Psychiatric and substance use comorbidity was assessed in 716 opioid abusers seeking methadone maintenance. Diagnostic and Statistical Manual of Mental Disorders, Revised Third Edition diagnostic assessment was conducted 1 month after admission. Rates of psychiatric and substance use disorder were compared by gender, and associa

[Failure to draw up medical certificates increase judiciary releases].

OBJECTIVES: In France, a systematic control of compulsory psychiatric admissions has existed since the enactment of the law of July 5th 2011. The Court of Cassation clarified that the liberty and custody judges (JLD) cannot supersede the medical opinion described in the medical certificates. In 2015, the JLD ordered the release of 8.4 % of all compulsory psychiatric admissions. The goal was to compare the quality of medical certificates derived from judicial release based on medical grounds with non-released witnesses from the cohort of compulsory psychiatric admissions ordered in the Groupe H

[New mask of hysteria: three case reports].

BACKGROUND: Hysteria is a polymorphous manifestation pathology in which unconscious psychic conflicts convert symbolically into somatic and psychic symptoms. THE AIM: When patients used to express their symptoms mostly in motor register, we are assisting progressively to the emergence of a new expressive mode of this disease. In fact, passional attitudes of the "Great hysteria" of Charcot and Richer have become exceptional, replaced by rich and sometimes confusing psychic symptoms. METHODS: We report three cases of patients with hysteria hospitalized in the department of psychiatry E at the

Psychiatric comorbidity reduces quality of life in chronic methadone maintained patients.

Despite the efficacy of methadone maintenance treatment (MMT), opioid dependence still involves severe impairment of functioning and low quality of life. This study examines the influence of the psychiatric comorbidity of MMT patients on their quality of life. A total of 193 middle-aged patients in long-term MMT were assessed for current and lifetime Axis I psychiatric disorders, substance use disorders, and personality disorders using the MINI, the CIDI-SAM, and the SIDP-IV. Quality of life (Qol) was assessed using the EQ-5D. Psychiatric comorbidity was documented in 78% of the patients. Mood

Reduced resting-state thalamostriatal functional connectivity is associated with excessive daytime sleepiness in persons with and without depressive disorders.

BACKGROUND: Excessive daytime sleepiness (EDS) is a common and significant problem encountered in affective illness, however, the biological underpinnings of EDS in persons with psychiatric disorders are not clear. This study evaluated the associations between thalamic connectivity with cortical and subcortical brain regions with EDS in persons with and without depressive disorders (DD). METHODS: Resting-state functional connectivity magnetic resonance imaging scans from 67 unmedicated young to middle-aged women with current DD (n = 30), remitted DD (n = 13), and healthy controls (n = 24) wer…

Personality disorder features as predictors of symptoms five years post-treatment.

Personality disorders are associated with dysfunction in a variety of areas. Recent longitudinal research has shown that personality disorders are also predictive of problems later in life, as well as of poor response to treatment of depression and anxiety. This study assessed whether personality disorder features were associated with psychiatric symptoms in a cohort of women treated for substance abuse in Sweden. Patients were diagnosed with personality disorders using the Structured Clinical Interview for DSM-IV (SCID-II) personality questionnaire and SCID-II interview, and were then adminis
