A review of neonatal peripherally inserted central venous catheters in extremely or very low birthweight infants based on a 3-year clinical practice: Complication incidences and risk factors.

BACKGROUND: The application of peripherally inserted central venous catheters (PICCs) in neonates has proven effective in avoiding repetitiv … verall incidence of 12.13%. The order of incidences of different complications from high to low was phlebitis (7.71%), malposition (3.66%), leakage (1.35%), pleural effusion (1.15%), central line-associated bloodstream infection (0.58%, 0.25/1,000d), and accidental removal (0.38%). Multivariate analysis revealed that the inserted vessel was an independent r …

Peripheral migration of pacemaker electrodes.

A 55-year-old male was admitted with right ileo-femoral phlebitis caused by an unused pacemaker electrode fragment which had migrated from the right subclavian vein to the right iliac vein. Vena cava plication was followed by removal of the electrode fragment and complete resolution of the phlebitis. The exposed metal coil may have contributed to the severe inflammatory reaction.

Intermittent flushing with heparin versus saline for maintenance of peripheral intravenous catheters in a medical department: a pragmatic cluster-randomized controlled study.

BACKGROUND: Three meta-analyses conducted in the 1990s concluded that the effect of intermittent flushing with heparin at low concentration (10 U/mL) was equivalent to that of 0.9% sodium chloride flushes in preventing occlusion or superficial phlebitis. No firm conclusion was reached on the safety and efficacy of heparin concentrations of 100 U/mL used as an intermittent flush. PURPOSE: To determine whether flushing peripheral intravenous catheters with 3 mL of a 100 U heparin/mL solution instead of saline improves the outcome of infusion devices. METHODS: Cluster-randomized, controlled, tw…

Implementation of Evidence-Based Practice in Peripheral Intravenous Catheter Care.

BACKGROUND: Inserting a peripheral intravenous (PIV) catheter is a common health care procedure; however, risks include phlebitis, extravasation, and accidental dislodgement. Using evidence-based practices (EBPs) can reduce these risks. PURPOSE: The purpose of this study was to implement an evidence-based PIV catheter care bundle and a decision-making algorithm. METHODS: A quasi-experimental study design was used. A care bundle and an evidence-based decision-making algorithm were implemented on a medical unit. Outcomes included length of PIV catheter dwell time, phlebitis and other complicat…

Cerebellar tuberculoma presenting as haematoma--a case report and pathophysiological consideration.

Intracranial tuberculoma represents the most common mass lesion in neurotuberculosis. We report an unusual case of tuberculoma with haemorrhage presenting clinically as a cerebellar haemorrhage. Cranial CT scan revealed a peripherally enhancing cerebellar mass with haemorrhage and obstructive hydrocephalus, requiring urgent surgical evacuation of the haematoma. Histopathological examination revealed tuberculoma with features of phlebitis. In view of the pathological evidence of phlebitis in the lesion, it could represent an immune-mediated acute haemorrhagic leucoencephalitis in response to tu…

[Thromboembolism of the lung and lower extremities in patients with chronic global congestive heart failure].

The results of a prospective study on the incidence of pulmonary thromboembolism and deep leg veins phlebitis in patients with congestive heart failure are presented. Pulmonary thromboembolism was diagnosed by means of pulmonary isotopic scanning and angiography; deep leg veins were studied using conventional and isotopic phlebography. A total incident of 60% for pulmonary thromboembolism and 92% for deep leg veins phlebitis was found, being of little significance the results suggested by clinical signs. ECG, chest X rays and laboratory tests such as arterial PO2, transaminase and lactic dehid…

[Severe thromboembolic complications following lumbar spinal phlebography].

From a series of 400 cases of lumbar spine phlebography by selective catheterisation, 4 serious thrombo-embolic episodes were observed, including 2 cases of severe pulmonary embolism which responded favourably to medical treatment. In 3 cases, risk factors were found (prolonged immobilisation in bed past history of phlebitis). One case developed severe phlebitis of the lower limbs despite prophylactic treatment with low dose calcium heparin. In the other cases, no anticoagulant therapy was prescribed before or during the examination.

Hepatic vein lesions in alcoholic liver disease: retrospective biopsy and necropsy study.

Obliteration of the terminal hepatic venules with perivenular fibrosis (phlebosclerosis) is a well recognised feature in alcoholic liver disease. Veno-occlusive lesions with intimal obliteration of hepatic veins and a lymphocytic phlebitis of hepatic veins may also be present. We looked for these lesions in 256 liver biopsies and 50 livers obtained at necropsy from patients with alcoholic liver disease. Phlebosclerosis was a universal finding in alcoholic hepatitis and cirrhosis and showed increasing severity with progressive liver injury. Veno-occlusive lesions, however, were found in only 25…

[Pseudophlebitic forms of popliteal cysts].

We report five observations of popliteal cysts responsible for clinical features of phlebitis of the calf. In each case, thrombosis of deep veins was outruled by phlebography. Clinical manifestations, which did not respond to efficiently dosed anticoagulant therapy, consistently resolved quickly after treatment of the cyst. Rheumatoid arthritis was the most frequent etiology. These observations induce us to look for a popliteal cyst in patients with signs of phlebitis. The negative phlebography rectifies the clinical diagnosis, thus avoiding prolonged anticoagulant therapy.

Impact of clinical profile at admission on the outcomes in patients hospitalized for acute pulmonary embolism: data from the IPER Registry.

Acute pulmonary embolism (PE) is characterized by a large heterogeneity of clinical presentation and disease course. We investigate whether different symptom PE phenotypes in hemodynamically stable PE could be associated with 30-day mortality risk. Hemodynamically stable patients from the multicentre, prospective Italian Pulmonary Embolism Registry (IPER) (September 2006-August 2010) presenting the most common four clinical phenotypes (< 24 h onset dyspnoea, chest pain, pleuritic pain and phlebitis) at admission were included and compared to those who were asymptomatic at admission. Overall, 1…

Peripheral intravenous catheter complications in critically ill children: a prospective study.

Six hundred fifty-four peripheral Teflon catheters in 303 pediatric intensive care unit patients were examined to determine complication rates and associated risk factors. Phlebitis, extravasation, and bacterial colonization occurred at rates of 13%, 28%, and 11%, respectively. Logistic regression of factors that increased phlebitis risk revealed infusion of hyperalimentation (odds ratio 2.9) or lorazepam (odds ratio 2.2) and catheter location (odds ratio 2.9) as the most important determinants of phlebitis risk. Age (less than or equal to 1 year, odds ratio 2.0), catheter time in situ (less t…
