A case of disseminated perforating granuloma annulare is presented. Concomitant diseases and exogeneous factors previously described as eliciting mechanisms were not found. Treatment with corticosteroids or cloroquin was not successful. All lesions were cleared with photochemotherapy within eight weeks.

BACKGROUND: Erythroderma could be the first sign of a cutaneous T-cell lymphoma (CTCL), such as Sézary syndrome. Causes of erythroderma include inflammatory dermatosis, toxicoderma, paraneoplastic erytroderma, and CTCL. Hence, diagnosing Sézary syndrome can be difficult. Fifteen out of twenty-three female patients (65%) receiving PUVA therapy showed signs of moderate to severe hypertrichosis. Only two out of fourteen patients (14%) on UV-A therapy without systemic psoralens developed hypertrichosis. We conclude that hypertrichosis may develop in many patients treated with photochemotherapy.

Precipitation of generalized pustular psoriasis was manifested in a patient with previously stable psoriasis vulgaris following lithium carbonate therapy for manic-depressive psychosis. Methotrexate toxicity was observed during the acute stage of the illness, and photochemotherapy was the successful treatment. Further pustulation occurred when lithium carbonate therapy was reintroduced.

Lesions typical of subacute cutaneous lupus erythematosus developed in an elderly woman after 6 months of PUVA (8-methoxypsoralen and longwave ultraviolet light) therapy for psoriasis. Pancytopenia, antibodies to double-stranded DNA, and hypocomplementemia developed concurrently with the appearance of the cutaneous lesions. With discontinuation of photochemotherapy, the cutaneous lesions disappeared and the pancytopenia improved.

Reactivation of a sunburn has been reported after the administration of methotrexate for cancer chemotherapy. A similar reaction is described in a patient with psoriasis who was receiving both coal tar with ultraviolet radiation for cutaneous lesions and methotrexate for arthritis. Knowledge of this poorly understood side effect of methotrexate is particularly important for physicians administering phototherapy and, perhaps, photochemotherapy.

A 55-year-old woman with psoriasis vulgaris was treated with oral 5-methoxypsoralen and UVA photochemotherapy. After 40 treatments over 3 months she became unwell with hepatitis attributable to the psoralen. Six years earlier she developed cholestatic hepatitis to flucloxacillin. A previous history of drug-induced reactions should be sought before prescribing further drugs with similar adverse effects.

Mycosis fungoides (MF) treatment must be chosen only after staging extent of lymphoma. Stage I and II diseases respond best to either topical nitrogen mustard, photochemotherapy (PUVA), or electron beam. Chemotherapy of stage III and IV MF has only limited remission rates. Combinations of therapies and experimental approaches, such as monoclonal antibodies, may give longer survivals.

Eight of 216 psoriatic patients treated at The London Hospital with photochemotherapy (PUVA) have developed a total of twenty-five skin carcinomata. The type and site of tumour are discussed with special reference to the importance of PUVA compared with other risk factors. We suggest that a history of previous skin carcinoma, arsenic therapy or radiotherapy are relative contra-indications to PUVA therapy for psoriasis.

A patient with psoriasis vulgaris developed verruciform xanthoma (VX) on the scrotum during psoralen photochemotherapy (PUVA). Although it is uncertain whether VX was induced by PUVA therapy, we report the first case of VX which appeared in a patient with psoriasis during PUVA therapy. We speculate that UV light may be one of the etiologic factors triggering VX in this case.

Phototherapeutic modalities, including narrowband-UVB, broadband-UVB, PUVA photochemotherapy, and excimer laser therapy are valuable tools that can be used for photoresponsive dermatoses in children. As a systematically safer alternative compared with internal agents, including the prebiologic and biological therapies, phototherapy should be considered a possible treatment option for children with diseases including psoriasis, atopic dermatitis, pityriasis lichenoides chronica, and vitiligo.

The development of unusual melanocytic lesions after exposure to UVA tanning beds has been reported recently. We studied one such case with the use of light and electron microscopy and found the appearance similar to the lentigines that may occur after psoralen photochemotherapy. Histologically, some melanocytes showed mild nuclear atypia. Ultrastructural examination showed features similar to those of other forms of lentigo.
