UNLABELLED: Therapeutic plasma exchange (TPE) is used to eliminate toxins, hormones or antibodies from the blood and replace the lost volume with fresh frozen plasma, albumin or crystalloids. In this article, recent advances in the usage for TPE for four different critical disease entities are explored: Septic shock, acute liver failure, catastrophic antiphospholipid syndrome (CAPS) and thyrotoxic storm.

The authors reviewed a series of 10 consecutive patients treated with plasma exchange (PE) for acute, severe optic neuritis (ON) largely unresponsive to previous high-dose IV glucocorticosteroids. PE was associated with an improvement of visual acuity according to the study criteria in 7 of 10 patients. On follow-up, three of these patients continued to improve, two remained stable, and two had worsened again. PE may be beneficial as an escalating treatment in a subset of patients with severe ON. A controlled trial is warranted.

Two patients with thrombotic thrombocytopenic purpura (TTP) were treated with daily plasma exchange with fresh frozen plasma of the plasma volume of the body (40 mg/kg . b.w.), in combination with red blood cell transfusion. The clinical symptoms resolved immediately and the parameters of laboratory examination improved promptly after the treatment, and follow-up of the 2 patients revealed no recurrent TTP episodes. This result indicates that plasma exchange is obviously superior to traditional treatment modalities and has significantly changed the course of TTP.

Focal segmental glomerulosclerosis (FSGS) recurs in 30% of renal allograft transplants with graft loss in half of the cases. A humoral factor may be implicated. We report on the use of therapeutic plasma exchange (TPE) in 11 patients with recurrent FSGS post transplantation. To assess the effects of plasma exchange on the hemodynamic performance of septic patients, we studied 12 septic patients (11 males and 1 female, age range 19-64, mean 39 +/- 17 years). A case of severe cyclosporin-induced hypertriglyceridemia that prompted plasma exchange therapy is reported. Hyperlipemic retinitis, headache, stupor, and peripheral paresthesias became apparent when the level of triglycerides exceeded the 1,500 mg/dL level. Two plasma exchanges were required to halt disease progression. This is the second time that plasma exchange was used in a bone marrow transplanted patient on cyclosporin-A therapy. In both cases hypertriglyceridemia became apparent and symptomatic after approximately 7 months of therapy with cyclosporin.

This report describes one case of rapidly progressive glomerulonephritis associated with amyloidosis in a 53-year-old woman with rheumatoid arthritis, successfully treated with intensive plasma exchange and immunosuppression. Amyloid deposits were present in all of 20 glomeruli in the kidney biopsy specimen and eight out of nine nonfibrosed glomeruli contained crescents. Plasma exchange was performed weekly during 6 months in two patients with symptomatic primary biliary cirrhosis. At the end of the treatment period improvement of liver demethylating capacity was noted, along with reduced C3 activation and a decrease in serum concentrations of IgM and procollagen III aminopeptide. Serum concentrations of aminotransferases, alkaline phosphatases, and anti-mitochondrial antibodies were not affected. These data suggest that long-term plasma exchange may improve liver function and possibly retard the progression of fibrosis in primary biliary cirrhosis.
