Challenges in care of the transgender patient seeking facial feminization surgery.

Facial feminization surgery (FFS) is the general term for a group of specific procedures that are used in an attempt to alter the perceived gender of an individual's face. These procedures include many commonly offered by facial plastic surgeons, such as rhytidectomy, brow lift, cheek implantation, and lip augmentation. Several procedures performed less often by most facial plastic surgeons also are involved, including scalp advancement, frontal cranioplasty, and reduction mandibuloplasty. Often, a challenge greater than the technical is the interpersonal interactions with patients. This artic

Acute Pain Management Following Facial Plastic Surgery.

Facial plastic surgery, including septorhinoplasty, aging face procedures, otoplasty, and oculoplastic procedures, has varying levels of evidence for the management of acute pain after surgery. This article discusses the available evidence in these procedures and discusses the authors' recommendations for the treatment of postoperative pain, with a focus on decreasing the reliance on opioid pain medication.

Basics of Trauma-Informed Care for the Facial Plastic Surgeon.

Patients seek facial plastic surgery to feel better, not only look better. As surgery is an inherently traumatic event, regardless of the result, patients seeking secondary facial plastic surgery procedures should be evaluated for possible traumatic response to prior surgery in initial consultation. Herein, we cover the history of trauma and trauma-informed counseling along with common approaches employed by counselors certified in this therapeutic approach. If signs of traumatic response are identified, trauma-informed counseling offers a valuable intervention to improve care.

Plastic surgeons' opinions of facial surgery for individuals with Down syndrome.

Plastic surgeons from Los Angeles and New York (N = 182) were surveyed on their opinions of facial plastic surgery for individuals with Down syndrome. Of the 100 respondents, 88 were familiar with the procedures, and 24 had performed the surgery. The surgeons indicated the circumstances for recommending the surgery, who would be required to give consent, the degree to which they thought individuals with Down syndrome could understand the nature of the surgery, and their opinion of the degree of discomfort associated with the surgery.

The Plantaris Tendon in Surgery: A Scoping Review of Reconstructive Roles Beyond Orthopedics.

INTRODUCTION: The plantaris tendon, a slender and often vestigial structure of the posterior leg, has long been recognized in orthopedic surgery for its favorable properties as an autograft. However, its role beyond musculoskeletal applications remains under characterized. This scoping review synthesizes the existing literature on the surgical use of the plantaris tendon in nonorthopedic procedures, highlighting patterns, outcomes, and evidence gaps across clinical specialties. METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Revi

Reduction otoplasty: correction of the large or asymmetric ear.

Reduction otoplasty is an uncommon procedure performed for macrotia and ear asymmetry. Techniques described in the literature for this procedure are few. The authors present their ear reduction approach that not only achieves the desired reduction effectively and accurately, but also addresses and creates the natural anatomic proportions of the ear, leaving a scar well hidden within the fold of the helix.

[Surgical treatment of micro-ophthalmic syndromes].

The complex embryology of the oculo-orbito-palpebral region is responsible for a number of heterogeneous clinicopathological situations, associating variable proportions of the three components of the malformation: micro-anophthalmia, microblepharism, micro-orbitism. Application of the double principle of skin expansion for the eyelids and distraction of the callus for the orbit, as early as possible (first year of life), is possible by means of a device which consists of a combination of an intraorbital expansion balloon, an antireflux valve to avoid effective pressure losses in the balloon,

How many people work in your operating room? An assessment of factors associated with instrument recounts within plastic surgery.

BACKGROUND: Intraoperative instrument recounts are performed to avoid retained foreign surgical items. These additional counts, however, beget risks of their own, including prolonged operative times, exposure to radiation, and increased cost. Our study aimed to identify factors that increase the likelihood of instrument recounts during plastic surgery procedures, and use our findings to guide potential solutions for preventing unnecessary recounts across all surgical fields. STUDY DESIGN: This is a retrospective review of all plastic surgical cases in the main operating setting at New York Un…

Single-specialty management and reconstruction of necrotizing fasciitis of the upper extremities: clinical and economic benefits from a case series.

PURPOSE: Patients with necrotizing fasciitis are managed with multiple prompt, radical surgical debridements and critical care support. Debridement and reconstruction are often provided by different surgical teams. Anecdotally, single-specialty management seemed to be a more efficient management strategy. This study aimed to investigate and compare the outcomes of management by plastic surgery versus multiple disciplines through a retrospective economic and clinical analysis of patients with necrotizing fasciitis treated over 8 years. We also present 3 index cases for which our service functio

The Impact of Plastic Surgery Volume on Inpatient Burn Outcomes.

BACKGROUND: Acute burn care involves multiple types of physicians. Plastic surgery offers the full spectrum of acute burn care and reconstructive surgery. The authors hypothesize that access to plastic surgery will be associated with improved inpatient outcomes in the treatment of acute burns. METHODS: Acute burn encounters with known percentage total body surface area were extracted from the National Inpatient Sample from 2012 to 2014 based on International Classification of Diseases, Ninth Edition, codes. Plastic surgery volume per facility was determined based on procedure codes for flaps,

Techniques for reconstruction of perialar and perialar-nasal ala combined defects.

BACKGROUND: The nasal ala and perialar areas involve junctions with the nose, cheeks, and lips. Following Mohs surgery, defects in this area often extend across one or more creases or folds, thus requiring repair of more than one reconstructive subunit. OBJECTIVE: Our goal is to present various reconstructive techniques required to obtain aesthetic results. METHODS: Defects and reconstructions will be illustrated to demonstrate techniques and results. Emphasis will be on selection of techniques and documentation of results. RESULTS: Examples include various flap procedures (advancement, rot

Perioperative Management of Antithrombotic Therapy in Patients Undergoing Plastic and Reconstructive Surgery: A Practical Tool Based on Current Guidelines.

Introduction: Given the aging population, plastic surgeons are increasingly faced with the challenge of balancing procedural bleeding risk with thromboembolic risk in patients receiving antithrombotic therapies undergoing elective procedures. Guideline recommendations remain unclear in this population, which contributes to heterogeneity in clinical practices. We present a practical approach that summarizes guideline recommendations to facilitate the perioperative management of patients requiring surgery who are already on antithrombotic agents. Methods: Due to the scarcity of plastic surgery-s
