We studied lungs at autopsy from 40 patients with cystic fibrosis (CF) to determine the structural and clinicopathologic features of pneumonia associated with Pseudomonas cepacia respiratory tract colonization. Three clinical groups were identified: group A included 11 patients exhibiting a fulminant course following P cepacia colonization; group B included 20 patients who declined slowly following colonization; and group C included nine patients without P cepacia colonization. Acute pneumonia occurred in all groups but was most extensive and necrotizing in group A. Chronic lobular pneumonia i

OBJECTIVE: Panton-Valentine leukocidin-producing Staphylococcus aureus necrotizing pneumonia is an unusual cause of community-acquired pneumonia, although associated with a high case fatality. This infection mainly affects young individuals, without any history, and is most often preceded by flu-like symptoms.
METHOD: We focused on patients presenting with Staphylococcus aureus necrotizing pneumonia in Reunion (Indian Ocean) admitted to the emergency department. We performed a retrospective study based on data collected from laboratory registers and medical files of patients presenting with S

Rationale: Intrapleural enzyme therapy (IET) with tissue plasminogen activator (tPA) and DNase has been shown to reduce the need for surgical intervention for complicated parapneumonic effusion/empyema (CPPE/empyema). Failure of IET may lead to delayed care and increased length of stay. Objectives: The goal of this study was to identify risk factors for failure of IET. Methods: We performed a multicenter, retrospective study of patients who received IET for the treatment of CPPE/empyema. Clinical and radiological variables at the time of diagnosis were included. We compared four different mach

BACKGROUND: Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) frequently causes severe necrotizing pneumonia in young patients.
CASE: We present the case of a 24-year-old male, who was brought to the emergency department with persistent fevers, confusion, and severe cough. He was found to have necrotizing pneumonia, pneumomediastinum, and rhabdomyolysis with renal failure. Cultures were positive for influenza A and CA-MRSA. After a prolonged intensive care unit (ICU) stay, he made a complete recovery.
CONCLUSION: Community-acquired MRSA pneumonia is a growing health th

Staphylococcus aureus necrotizing pneumonia is a life-threatening disease that is frequently preceded by influenza infection. The S. aureus toxin Panton-Valentine leukocidin (PVL) is most likely causative for necrotizing diseases, but the precise pathogenic mechanisms of PVL and a possible contribution of influenza virus remain to be elucidated. In this study, we present a model that explains how influenza virus and PVL act together to cause necrotizing pneumonia: an influenza infection activates the lung epithelium to produce chemoattractants for neutrophils. Upon superinfection with PVL-expr

OBJECTIVE: To evaluate the CT features of complicated Klebsiella pneumonia.
MATERIALS AND METHODS: We retrospectively reviewed 15 CT examinations from 11 consecutive patients with complicated Klebsiella pneumonia with special attention to internal architecture of pneumonic consolidation and its changes on follow-up CT. On plain radiography, abscess or cavitation was suspected in all patients. The diagnosis was established by isolation of Klebsiella pneumoniae from blood in six patients, from sputum in two, and from both in three. Five patients underwent follow-up CT due to progression (one) o

Severe pneumonia caused by community-associated methicillin-resistant Staphylococcus aureus (MRSA) was reported in children soon after this pathogen emerged in the United States in the 1990s. Genes for Panton Valentine leukocidin, which are present in the majority of community-associated MRSA, are thought to enhance the ability of S aureus to cause necrotizing pneumonia. Despite the rapid spread throughout the United States of community-associated MRSA and related skin and soft-tissue infections, reports of severe pneumonia in adults have been rare. We describe a case of a healthy young adult

A 30-year-old apparently immunocompetent woman presented with acute respiratory failure (acute respiratory distress syndrome). No etiologic agent was found, and she died 2 weeks later despite antibiotic therapy. Postmortem examination of the lungs showed diffuse organizing alveolar damage with superimposed focal necrotizing peribronchiolar pneumonia. Cultures obtained from lung tissue were negative for virus, fungi, and bacteria. Histopathologic and electron microscopic studies showed that the necrotizing changes were consistent with herpesvirus infection. With the use of a new diagnostic tool

BACKGROUND: Pneumonia by Rhodococcus equi is infrequent and is associated with patients with important immunosuppression. To date 66 cases of pneumonia by Rhodococcus equi in patients with HIV infection have been published. The diagnosis, problems in determining diagnosis and treatment are discussed.
MATERIALS AND METHODS: Two new cases of pneumonia by Rhodococus equi in C3 stage patients with HIV infection are reported. Diagnosis was achieved by study of bronchoalveolar lavage samples with the Apy-Coryne method and gas chromatography.
RESULTS: The two patients presented pneumonia, one of wh

We greatly appreciate the comments of Rakovich and Kurz regarding our recently published article. In our correspondence, we clarify several issues raised by the authors regarding the definition, pathophysiology and management of necrotizing lung infections (NLI) in patients requiring VV-ECMO support for severe respiratory failure. We emphasize that there is currently no universally accepted radiological definition or volumetric cut-off for necrotizing pneumonia. Our study intentionally used a broader CT-based definition encompassing necrotizing pneumonia, lung abscess and necrotizing cavitatio

Panton-Valentine leukocidin (PVL) is a toxine which forms a pore-forming heptamer on neutrophil membrandes leading to neutrophil lysis. Invasive community-onset PVL producing staphylococcus aureus (S. aureus) induced diseases have emerged worldwide, but incidence vary and is strongly attributed to strain types and lineages. Unfortunately diagnostic microbiology laboratories do not routinely test for PVL. These strains have been reported to exceed in virulence hospital-acquired MRSA (methicillin-resistant S. aureus) causing sepsis, necrotizing fasciitis but in rare cases cause severe necrotizin

Hemorrhagic necrotizing pneumonia caused by cytotoxic necrotizing factor-1-positive extraintestinal pathogenic Escherichia coli (ExPEC) is a rapidly fatal disease in dogs. Lung tissues from 6 client-owned dogs with culture-confirmed ExPEC pneumonia were analyzed using immunohistochemistry (IHC) and fluorescence in situ hybridization (FISH). Both assays localized E. coli within pneumocytes, alveolar macrophages, and alveolar spaces. Notably, one case exhibited numerous filamentous E. coli, representing a stress-induced morphologic adaptation previously unreported in canine ExPEC pneumonia. Reco

This study presents a case of nasal aspergillosis in a 17-days old calf (German Fleckvieh): it had been admitted moribund to the Clinic for Ruminants of the University of Munich, and died after a short time. Pathologically, the calf was diagnosed with purulent-necrotizing rhinitis, necrotizing pneumonia, and diphtheroid-necrotizing abomasitis. Histologically, fungal elements were found in all the localizations mentioned before, and mycologically, Aspergillus fumigatus was cultured from nasal cavity. Pathogenesis is discussed.

Panton-Valentine leukocidin (PVL) is an exotoxin that is produced by many strains of Staphylococcus aureus, and an important virulence factor. A PVL-positive S. aureus infection leads to rapid and severe infections of soft tissue and necrotizing pneumonia in healthy adolescents, and has a high mortality. This case report included a 12-year-old male patient who admitted for fever, respiratory distress and hip pain and was identified with necrotizing pneumonia with septic pulmonary embolism, psoas abscess, cellulitis and osteomyelitis. The PVL positive methicillin-sensitive S. aureus (MSSA) was

The majority of CA-MRSA infections present as skin and soft tissue infections such as abscesses or cellulitis. However, CA-MRSA can cause invasive infections such as joint infections, necrotizing pneumonia or septicemia. Here we describe five cases with CA-MRSA bacteremia complicated with osteoarticular infection, necrotizing pneumonia, and infective endocarditis. We report these case series to outline the spectrum of invasive CA-MRSA diseases and to demonstrate clinical outcome. Early proper intervention with regular revisiting the empirical treatment based on local susceptibility data is cru

Staphylococcus aureus is a major human pathogen responsible for a variety of toxin-mediated and suppurative diseases. About 50 staphylococcal virulence factors have been described to date. In this review, we examine the clinical implications of key staphylococcal virulence factors in toxin-mediated diseases, septic shock, and severe focal infections such as arthritis, infective endocarditis, pneumonia acquired during mechanical ventilation, and necrotizing pneumonia. Staphylococcal pathogenicity is sometimes due principally to a single virulence factor, as in toxic shock syndrome and necrotizi

Methicillin-resistant Staphylococcus aureus (MRSA) has recently emerged as a cause of community-acquired infections among individuals without risk factors. Community-associated MRSA (CA-MRSA) appears to be more virulent, causing superficial mild skin and soft tissue infections to severe necrotizing fasciitis, and in rare cases, pneumonia. Community-associated MRSA was first reported in Australia in the early 80s, after almost two decades in the USA, and then in several countries in Europe, Asia and South America. No data exists in Portugal. We report the first case of CA-MRSA infection in Port

We present the histopathologic features of fatal Burkholderia cepacia pneumonia in three adults (one man [age 44 years] and two women [aged 40 and 43 years]). In all patients, the pulmonary infiltrates initially were localized (right middle lobe, left upper lobe, and right middle lobe) but rapidly progressed. Two open-lung biopsies and one pneumonectomy specimen showed necrotizing granulomatous inflammation merging with areas of more conventional necrotizing bronchopneumonia In one patient, a mediastinal lymph node also showed stellate necrotizing granulomas. Vasculitis was absent. B. cepacia
