A 62-year-old female patient presented with no improvement in vision 10 days after undergoing cataract extraction in the right eye. The unaided visual acuity in the right eye was 0.1, and examination with a slit lamp revealed the presence of the intraocular lens with an increased gap between the intraocular lens and the posterior capsule. Anterior segment optical coherence tomography showed a distance of 3.236 mm between the posterior capsule and the posterior surface of the intraocular lens. Based on the medical history, ocular examination, and auxiliary examinations, a diagnosis of right eye

In 144 eyes in 144 patients with senile cataract the rate of posterior capsular opacification requiring YAG capsulotomy up to 36 months following intercapsular cataract surgery with lens epithelial cell removal using ultrasound and aspiration was evaluated and compared to the rate for 471 senile cataractous eyes in patients who had had posterior chamber lens implantation following phacoemulsification and extracapsular cataract extraction without lens epithelial cell removal. Posterior capsular opacification occurred in 3.7% of patients who had lens epithelial cell removal, significantly less (

The cellular response in lens remnants was studied after ECCE in the rabbit eye. Following capsulotomy, the lens content was gently exprimated. No aspiration/irrigation was performed. Degenerative changes were found in the lens fibers and lens epithelium at the equator. Proliferation of lens epithelium started early. Lens fibers break-down almost ceased when lens fibers had been enclosed by proliferating epithelium, that also formed new foci for lens fiber formation. In the opening between the anterior and posterior capsule fibroblastlike cells emerged proliferating on to the posterior capsule

In a retrospective study dealing with 52 eyes, we compare the features and prognosis of retinal detachment following intracapsular cataract extraction, non complicated extracapsular cataract extraction, extracapsular cataract extraction followed by YAG laser posterior capsulotomy, extracapsular cataract extraction complicated by peroperative rupture of the posterior capsule with phakic retinal detachment. Our results show several common features between retinal detachment after non complicated cataract extraction and phakic retinal detachment. Cataract extraction, no matter the technique, is n…

Extracapsular cataract extraction (ECCE) and posterior chamber intraocular lens (PC-IOL) implantation requires that the posterior capsule and the zonules remain sufficiently intact. To assess the surgical trauma on the lens suspensory apparatus of the enucleated rabbit eye, we developed a new fine pressure gauge that could quantitate its strength. The cornea and the iris of enucleated eyes were removed to allow clear visualization of the anterior surface of the lens to the equator. Afterwards several types of anterior capsulotomy were performed for ECCE and PC-IOL implantation. And then the le

We conducted a prospective, randomized clinical trial of the use of 1% sodium hyaluronate (Healon) in the anterior chamber during anterior capsulotomy and nuclear expression in extracapsular cataract extraction with posterior chamber lens implantation. Two months postoperatively, the mean (+/- standard deviation) central endothelial cell loss was 6.1% +/- 11.4% in 92 eyes in which sodium hyaluronate was used for the capsulotomy and nuclear expression and 7.6% +/- 14.7% in 89 eyes in which it was not used (P greater than .6). Sodium hyaluronate was also used later in the procedure in all eyes t

Between 8/81 and 9/89 2658 patients underwent retinal detachment surgery at the university eye hospital Erlangen/Nürnberg. From this number there were 30 (= 1.1%) pseudophakic retinal detachments consisting of 14 males and 16 females. Minimal age was 48, maximal 89 years, average age was 70.9 years. There were 4479 extracapsular cataract extractions with implantation of a posterior chamber lens performed in the same interval at our eye hospital. Out of this 4479 extracapsular cataract extractions we found 9 patients (= 0.2%) with pseudophakic retinal detachment. In our investigated group we fo

PURPOSE: Spontaneous fracture of an intraocular lens (IOL) haptic is a rare complication of cataract surgery. The authors report a case of spontaneous fracture of an implanted posterior chamber IOL.
CASE: Five years ago, a 12-year-old patient underwent linear lens extraction, posterior capsulotomy, and anterior vitrectomy due to traumatic cataract and received a polymethyl methacrylate (PMMA) biconvex posterior chamber IOL implanted in ciliary sulcus. Five years later, IOL optic was found in anterior chamber with its haptics broken from the optic-haptic junction.
DISCUSSION: The broken hapti…

A 56-year-old Asian woman with nanophthalmos and cataract in the right eye underwent extracapsular cataract extraction with primary posterior chamber intraocular lens implantation of +31.0 D. Intraoperatively, there was severe vitreous upthrust; however, there was no vitreous loss and the lens could be placed in the posterior chamber. The patient's postoperative course was uneventful except for a moderate degree of coagulum over the lens. Her best-corrected visual acuity was 20/80. The patient required YAG capsulotomy one year later. Her intraocular pressure was under control without any medic
