The metal alloys which were investigated histopathologically in the first part of this study, were examined with respect to their allergic potentials using the patch test. Results from 60 subjects (aged 17-23) were evaluated following exposure to nickel sulphate, potassium dichromate, silver nitrate, cobalt nitrate, copper sulphate, palladium chloride, platinum chloride and gold chloride. Nickel sulphate produced the most vigorous allergic response whereas gold chloride showed the least of all. The remaining solutions were ranked in decreasing order of severity as follows: potassium dichromate

To study the effect of hypokalaemia in the regulation of aldosterone secretion, repeated injections of frusemide (3 mg/kg) plus saline with or without simultaneous infusion of potassium chloride (1 mEq/kg/h) were performed in 24 conscious female rabbits for 7 h. Without potassium supplementation, the plasma renin activity (PRA) remained elevated throughout the study, while an initial increase (1 h to 3 h) in plasma aldosterone (PA) gradually returned to normal with reduction of the serum potassium. In rabbits on potassium supplements to prevent the development of hypokalaemia, both PRA and PA …

A typical American diet contains amounts of sodium chloride far above evolutionary norms and potassium far below those norms. It also contains larger amounts of foods that are metabolized to noncarbonic acids than to organic bases. At baseline, in a steady state, diets that contain substantial sodium chloride and diets that are net acid producing each independently induce and sustain increased acidity of body fluid. With increasing age, the kidney's ability to excrete daily net acid loads declines, invoking homeostatically increased utilization of base stores (bone, skeletal muscle) on a daily

Anatomical and functional renal medullary abnormalities are common in patients with sickle cell disease (HbSS) and sickle cell trait (HbAS). Sickle cell disease is associated with impaired urinary potassium excretion, but renal potassium handling in patients with HbAS has not been reported. To address this question, we gave nine HbAS and eight black control (HbAA) volunteers an oral potassium chloride load (0.75 mEq/kg of body weight), collected urine for five hours, and measured changes in the serum and urinay potassium levels. Both prior to and after potassium chloride loading, there were no…

Fractional distal chloride reabsorption was measured in 8 patients with Bartter's syndrome, 8 patients with comparable degrees of hypokalemia of different etiologies and 7 normal subjects during maximal diuresis induced by an intravenous infusion of 5% dextrose in water. A low fractional distal chloride reabsorption (0.55 +/- 0.01) was found only in patients with Bartter's syndrome, whose serum potassium concentrations ranged between 1.2 and 3.1 mEq/l. Non-Bartter's syndrome hypokalemic patients had serum potassium concentrations between 2.4 and 3.2 mEq/l (p NS when compared to patients with B

The isolated rabbit caecum was studied in vitro. Under our experimental conditions, the rabbit caecum secreted potassium and chloride and absorbed sodium. To characterize the transport properties of the apical and the basolateral barriers, transepithelial electrical and flux (22Na, 36Cl and 86Rb) measurements and their sensitivity to transport inhibitors (furosemide, DIDS, ouabain and barium) are presented together with intracellular measurements with double-barrelled microelectrodes of intracellular electrical potentials and ionic activities. The fluxes of sodium and chloride were insensitive

Horses with hyperkalaemic periodic paralysis were challenged with an oral dose of potassium chloride, and the prophylactic efficacy of phenytoin, acetazolamide and hydrochlorothiazide was evaluated, with at least three weeks separating the trials of each drug. After the administration of potassium chloride without prophylactic medication the horses' clinical signs ranged from generalised fine muscle fasciculations to gross tremors, and weakness with occassional prolapse of the nictitating membrane; plasma potassium concentration increased significantly (P < 0.01) from 4.0 +/- 0.2 to 6.0 +/- 1.…

This baby boy was born after a pregnancy complicated by severe polyhydramnios at a gestational age of 28 weeks. Analysis of the amniotic fluid had shown a high chloride content, but normal concentrations of sodium, potassium, and calcium. After birth he displayed extreme polyuria, severe renal sodium and chloride loss, and marked hypercalciuria. Five weeks after birth, his sodium chloride loss turned into renal potassium loss, along with a marked decrease in urine output. All these features are characteristic of the neonatal variant of Bartter syndrome. He was discharged after 11 weeks with or

Existing genetic blood disorders are referred to as hereditary anemia. Such disease is especially high in the Mediterranean area. In Iraq, the prevalence of carriers was estimated between 4.5% -5% of the general population. The disease can be clinically presented by mild anemia up to severe manifestations that need transfusion and its complications can usually affect many different organ systems. During the past few years, focus was diverted to the cytokines and electrolyte status as potential disease severity markers. ELISA was used to identify serum cytokines. The analysis of serum sodium, c

The effects of i.v. and local anaesthetics on guineapig tracheal smooth muscle have been investigated in vitro. Diazepam, propanidid and ketamine induced relaxation and antagonized histamine-, acetylcholine-and potassium chloride-induced contractures in the tracheal smooth muscle. The relaxing effects of the anaesthetics were not affected by propranolol. Thiopentone, at concentrations which would be used clinically, caused contracture of the tracheal smooth muscle. Morphine induced relaxation and antagonized histamine-induced contracture, but did not antagonize acetylcholine-or potassium chlor…

The aim of the present study was to test whether intravenous neutral phosphate supplementation, recently shown in our laboratory to acutely stimulate proton secretion in the distal nephron, was able to induce a sustained metabolic alkalosis. Neutral Na and K phosphate supplementation for seven days, with equivalent reduction in chloride supply and unchanged intake of sodium and potassium, in ADX rats receiving fixed physiological doses of aldosterone and dexamethasone (group 1, N = 7), was responsible for a severe metabolic alkalosis (MA; delta [HCO3] 11 +/- 1.3 mM, and delta pH 0.11 +/- 0.06

BACKGROUND: Bromide is a halide ion of the element bromine usually administered in the form of potassium salt as monotherapy or add-on treatment in epileptic dogs. It is excreted unchanged in the urine and undergoes tubular reabsorption in competition with chloride. Thus, dietary chloride content affects serum bromide concentrations. This is the first published clinical report of bromide toxicosis secondary to a dietary modification of chloride content in an epileptic dog treated with potassium bromide.

Background: Electrolyte disturbances are common in critically ill patients with acute kidney injury (AKI) and often require urgent intervention. Arterial blood gas (ABG) analyzers provide rapid point-of-care electrolyte measurements, but their agreement with central laboratory analyzers in critically ill patients with AKI remains uncertain. This study evaluated the agreement between ABG and central laboratory measurements of sodium (Na+), potassium (K+), and chloride (Cl-). Methods: We conducted a retrospective observational study of adult critically ill patients with AKI admitted to a tertiar

Recent studies of hereditary renal tubular disorders have facilitated the identification and roles of chloride channels and cotransporters in the regulation of the most abundant anion, Cl-, in the ECF. Thus, mutations that result in a loss of function of the voltage-gated chloride channel, CLC-5, are associated with Dent's disease, which is characterized by low-molecular weight proteinuria, hypercalciuria, nephrolithiasis, and renal failure. Mutations of another voltage-gated chloride channel, CLC-Kb, are associated with a form of Bartter's syndrome, whereas other forms of Bartter's syndrome a

Since beta-adrenergic stimulation enhances extrarenal potassium uptake, we postulated an opposite effect of the alpha-adrenergic nervous system. Seven healthy subjects were given intravenous potassium chloride (0.5 mmol per kilogram of body weight), in the presence and absence of the alpha-agonist phenylephrine. After potassium chloride alone, the potassium level rose to 0.64 +/- 0.03 mmol (mean +/- S.E.M.); phenylephrine augmented the rise (0.93 +/- 0.09 mmol, P less than 0.025) and prolonged it, without changing urinary potassium excretion. Subsequent administration of potassium and phenylep…

Renal salt loss in Bartter's syndrome is caused by impaired transepithelial transport in the loop of Henle. Sodium chloride is taken up apically by the combined activity of NKCC2 (Na+-K--2Cl- cotransporters) and ROMK potassium channels. Chloride ions exit from the cell through basolateral ClC-Kb chloride channels. Mutations in the three corresponding genes have been identified that correspond to Bartter's syndrome types 1-3. The gene encoding the integral membrane protein barttin is mutated in a form of Bartter's syndrome that is associated with congenital deafness and renal failure. Here we s

OBJECTIVE: To compare the effectiveness of dynamic stratified potassium supplementation at high concentrations with enteral potassium supplementation in older patients with chronic heart failure and moderate to severe hypokalaemia.
METHODS: We performed a single-centre, short-term, randomised, controlled, open-labelled, clinical trial, and patients were randomly allocated to the control or intervention group. The intervention group received intermittent infusions of 30 mmol/100 mL potassium chloride. In the control group, 10% potassium chloride was administered orally in a bolus dose. Short-t…

BACKGROUND: Heterotopic abdominal pregnancies with coexisting intrauterine pregnancies pose unique therapeutic challenges, and management options, particularly nonsurgical approaches, are limited.
CASE: We present a case in which selective reduction of a heterotopic abdominal pregnancy during the second trimester using fetal intracardiac injection with potassium chloride enabled subsequent vaginal delivery of the intrauterine pregnancy at term. In addition, we summarize nine cases of nonsurgical management of heterotopic abdominal pregnancies, four of which involve potassium chloride selectiv…
