Will a Conservative Case Selection Strategy Improve Hospital-Level TAVR Performance Metrics?

BACKGROUND: Current national performance metrics rank transcatheter aortic valve replacement (TAVR) centers based on risk-adjusted outcomes. This could make operators/centers less inclined to offer TAVR in high-risk cases. METHODS: We used simulation models based on registry data to explore whether avoiding high-risk TAVR cases would improve the hospitals' comparative risk-adjusted TAVR outcomes. This multicenter, retrospective cohort study included all adults (≥18 years) who underwent TAVR in the Society of Thoracic Surgeons/American College of Cardiology Transcatheter Valve Therapy registry

An intervention to support stroke survivors and their carers in the longer term (LoTS2Care): study protocol for the process evaluation of a cluster randomised controlled feasibility trial.

BACKGROUND: Whilst pathways relating to the early stages of stroke care have become well established, strategies for longer-term care are less developed and longer-term outcomes remain poor for many stroke survivors. New Start, a complex intervention that includes needs identification, exploration of social networks and components of problem-solving and self-management, was designed to improve stroke survivors' quality of life by addressing unmet needs and increasing participation. It is delivered approximately 6 months post-stroke by trained staff (facilitators). We are currently undertaking

A metaanalysis of treatment outcomes of early enteral versus early parenteral nutrition in hospitalized patients.

OBJECTIVE: Nutritional support as enteral or parenteral nutrition (PN) is used in hospitalized patients to reduce catabolism. This study compares outcomes of early enteral nutrition (EN) with early PN in hospitalized patients. DESIGN: The authors conducted a metaanalysis of randomized, controlled trials (RCT) comparing early EN with PN. Studies on immunonutrition were excluded. Studies were categorized as medical, surgical, or trauma. PATIENTS: RCTs of early EN/PN were identified by search of 1) MEDLINE (1966-2002), 2) published abstracts from scientific meetings, and 3) bibliographies of re

Selecting measures for balance and mobility to improve assessment and treatment of individuals after stroke.

Assessment of individuals with stroke using reliable and valid outcome measures is a key component of the treatment planning process. Health care professionals may have difficulty selecting balance and mobility measures given the large number of measures to choose from. This article utilizes a case-based approach to describe the benefits of using a common set of outcome measures and a process for selecting optimal measures across body structure/function, activity, and participation domains of the International Classification of Functioning, Disability and Health (ICF) model and stages of strok

Process improvement for follow-up radiology report recommendations of lung nodules.

UNLABELLED: In the modern healthcare system, there are still wide gaps of communication of imaging results to physician and patient stakeholders and tracking of whether follow-up has occurred. Patients are also unaware of the significance of findings in radiology reports. With the increase in use of cross-sectional imaging such as CT, patients are not only being diagnosed with primary urgent findings but also with incidental findings such as lung nodules; however, they are not being told of their imaging findings nor what actions to take to mitigate their risks. In addition, patients at high r

Effects of introducing an allied health assessment pro forma on the management of acute stroke patients.

PURPOSE: There is a small body of evidence that supports the use of care pathways and assessment pro formas for the management of acute stroke patients, however, such tools applied specifically to the allied health disciplines are not in widespread use. This study sought to evaluate the effects of introducing an assessment pro forma on the allied health management of acute stroke patients. METHODS: The allied health management of 40 consecutive stroke patients admitted after the introduction of the assessment pro forma was compared with that of a historical control group of the same size. The role of a critical time intervention on the experience of continuity of care among persons with severe mental illness after hospital discharge.

We investigated the impact of critical time intervention (CTI) on self-reported indicators of quality of continuity of care (COC) after discharge from inpatient psychiatric treatment with data from a randomized controlled trial that assessed the effectiveness of the intervention in reducing recurrent homelessness. Postdischarge COC outcome measures among previously homeless persons with severe mental illness randomly assigned to receive usual services only (n = 73) or 9 months of CTI in addition to usual services (n = 77) were compared. Those assigned to CTI had greater perceived access to car

Analysing health outcomes.

If we cross-classify the absolutist-consequentialist distinction with an intuitive-analytical one we can see that economists probably attract the hostility of those in the other three cells as a result of being analytical consequentialists, as much as because of their concern with "costs". Suggesting that some sources of utility (either "outcome" or "process" in origin) are to be regarded as rights cannot, says the analytical consequentialist, overcome the fact that fulfilling and respecting rights is a resource-consuming activity, one that will inevitably have consequences, in resource-constr

Nursing assessment of patients with non-acute wounds.

Accurate and thorough assessment of patients with wounds is essential in order to provide correct treatment. In relation to patients with open, non-acute wounds, the areas that need assessing include general health and specific underlying disease process, care being given and local wound environment. This article outlines specific details of the assessment process. It includes a breakdown of factors affecting healing, a discussion of the importance of diagnosis of cause, an outline of factors within care provision that may have a negative effect on healing, and areas relevant to the wound itse

[A process audit of stillbirths in a rural family medical practice].

A process audit was undertaken of 5 stillbirths recorded in a rural family medical practice between 1985-1987. The stillbirth rate for this practice was 81/1000, as opposed to a national rate of only 4.5/1000 (p less than 0.015). The clinic records of the women, both before and during the affected pregnancies were reviewed in detail. In each case the family and hospital physicians involved were interviewed, as were 3 of the 5 patients. In 2 cases fetal death was recorded when the women were admitted to hospital after fetal movements had already been absent for more than 48 hours. The other 3 w

The General Practice Assessment Questionnaire (GPAQ) - development and psychometric characteristics.

BACKGROUND: Continual quality improvement in primary care is an international priority. In the United Kingdom, the major initiative for improving quality of care is the Quality and Outcomes Framework (QoF) of the 2004 GP contract. Although the primary focus of the QoF is on clinical care, it is acknowledged that a comprehensive assessment of quality also requires valid and reliable measurement of the patient perspective, so financial incentives are included in the contract for general practices to survey patients' views. One questionnaire specified for use in the QoF is the General Practice As

The readability of psychosocial wellness patient resources: improving surgical outcomes.

BACKGROUND: Patient education is increasingly accessed with online resources and is essential for patient satisfaction and clinical outcomes. The average American adult reads at a seventh grade level, and the National Institute of Health (NIH) and the American Medical Association (AMA) recommend that information be written at a sixth-grade reading level. Health literacy plays an important role in the disease course and outcomes of all patients, including those with depression and likely other psychiatric disorders, although this is an area in need of further study. The purpose of this study wa

Health Promotion in an Opioid Treatment Program: An Evidence-Based Nursing Quality Improvement Project.

Community assessment and review of the literature indicate that individuals supported in opioid treatment programs are at a significant disadvantage for access to preventative and primary healthcare. In addition, this population faces increased comorbidities and chronic disease. Finally, access to housing, nutritious food, and other social determinants of health is also a challenge for these individuals. This project, aimed at addressing healthcare disparities and improving health outcomes for the opioid treatment program client, was undertaken at a large, private, not-for-profit, community me

Statistical benchmarks for process measures of quality of care for mental and substance use disorders.

OBJECTIVE: Benchmarks, representing the level of performance achieved by the best-performing providers, can be used to set achievable goals for improving care, but they have not heretofore been available for mental health care. This article describes the application of a method for developing statistical benchmarks for 12 process measures of quality of care for mental and substance use disorders. METHODS: Twelve quality measures--taken from a core measure set selected by a multistakeholder panel through a formal consensus process--were constructed from 1994-1995 administrative data on care re

Implementation outcomes of national decentralization of integrated outpatient services for severe non-communicable diseases to district hospitals in Rwanda.

OBJECTIVES: Effective coverage of non-communicable disease (NCD) care in sub-Saharan Africa remains low, with the majority of services still largely restricted to central referral centres. Between 2015 and 2017, the Rwandan Ministry of Health implemented a strategy to decentralise outpatient care for severe chronic NCDs, including type 1 diabetes, heart failure and severe hypertension, to rural first-level hospitals. This study describes the facility-level implementation outcomes of this strategy. METHODS: In 2014, the Ministry of Health trained two nurses in each of the country's 42 first-le
