Therapy talk: analyzing therapeutic discourse.

Therapeutic discourse is the talk-in-interaction that represents the social practice between clinician and client. This article invites speech-language pathologists to apply their knowledge of language to analyzing therapy talk and to learn how talking practices shape clinical roles and identities. A range of qualitative research approaches, including ethnography of communication, conversation analysis, and frame theory, provides a background for the case presentation of a 13-year-old girl who stutters. Asymmetry is a feature of the therapeutic discourse presented , with evidence of recognitio

Holism in occupational therapy: elusive fiction and ambivalent struggle.

The profession of occupational therapy is said to have underpinnings of holistic, humanistic, and client-centered values. How does this claim translate into practice? This article reports on a qualitative study in which the practice experiences of 12 occupational therapists in the United Kingdom were explored. Through phenomenological analysis of interviews and participant observation data, the findings revealed that although holism is indeed valued, considerable uncertainty exists about what it actually means. The therapists studied seemed to understand holism and enact it in different, somet

Patient Autonomy in Physical Therapist Practice: Defining the Limits.

Patient autonomy has long been considered a foundational principle in biomedical ethics and has been defined, in its most basic form, as a right to self-governance or self-determination. The emphasis that is placed on patient autonomy results in a necessary attention to a patient's values, expressed wishes, and decision making in a medical context. However, in an era of healthcare consumerism, where healthcare may be treated like any other good, the line between patient-centered care and patient-directed care becomes blurred, potentially resulting in a mindset that the patient (as a consumer)

Mental health consumers and providers dialogue in an institutional setting: a participatory approach to promoting recovery-oriented care.

OBJECTIVE: This brief report presents the preliminary findings of a participatory project, to answer a question raised by stakeholders in mental health services: How can providers and patients create a process for knowledge exchange to support recovery-oriented care? METHOD: Participatory action research (PAR) and narrative phenomenological methodology guided the selection of methods, which consisted of an iterative process between telling stories and dialoguing about personal values related to recovery. The sample consisted of three occupational therapists, a psychiatrist, an academic-clinic

Therapeutic factors affecting the cognitive behavioral treatment of bulimia nervosa via telemedicine versus face-to-face delivery.

OBJECTIVE: Recently, Mitchell et al. (Behav Res Ther, 46, 581-592, 2008) conducted a randomized controlled trial of an empirically supported treatment for bulimia nervosa (BN) delivered face-to-face (FTF-CBT) or via telemedicine (TV-CBT). Results suggested that the TV-CBT and FTF-CBT were generally equivalent in effectiveness. The objective of the current study was to examine ratings of therapeutic alliance factors in TV-CBT and FTF-CBT. METHOD: Data obtained from 116 adults who met criteria for BN or eating disorder-not otherwise specified with binge eating or purging weekly and six doctoral

Poor health literacy as a barrier to patient care.

Only 12% of adults have proficient health literacy, according to the National Assessment of Adult Literacy. In other words, nearly 9 out of 10 adults may lack the skills needed to manage their health and prevent disease. The elderly patient is at especially high risk for having low health literacy. To provide optimal care for patients, pharmacists and other health care practitioners must understand the problems of health literacy and incorporate strategies and tools to improve the effectiveness of their communication with patients.

Communication between health care professionals and deaf patients.

A review of the literature about communication between health care professionals and deaf people demonstrates that a continuum of accuracy in perceptions of deaf people and their communication problems exists in health care settings. The author provides information on deaf culture, history, population size, sign language, and communication needs that is used to evaluate professional journal articles. Examples of research on this topic are presented and the need for further studies and the role of social workers are discussed.

Associations between interpersonal synchrony and clients' perception of session quality are moderated by depression severity.

OBJECTIVE: Previous research has identified a complex relationship between client-therapist synchrony and therapy outcomes. We investigated the ways in which depression severity influences this relationship. METHOD: The sample was comprised of 30 clients undergoing a 16-session supportive-expressive dynamic therapy for depression. Client-therapist interpersonal synchrony was assessed using the Client-Therapist interpersonal Synchrony (CIB) coding system during five pre-selected sessions. After each session, the clients evaluated their satisfaction with the session. We employed multi-level mod

Trust in the patient-provider relationship: an arts-based research film exploring trust in adolescents and young adults with HIV.

Arts-based research offers an innovative method for exploring complex emotions such as trust by capturing subjective experiences that traditional methods may overlook. Adolescents and young adults with HIV (AYA-WH) are less likely than older adults to achieve viral suppression and remain engaged in care, with only 40-60% retained one year post-transition to adult services. Patient-provider trust plays a critical role in care engagement and adherence, yet little is known about how AYA-WH experience and interpret trust. Using a cross-sectional mixed-methods approach guided by Caring Science, Car

Should Clinicians Challenge Faith-Based Institutional Values Conflicting with Their Own?

Catholic health care organizations generally prohibit their employees from prescribing contraceptives for the purpose of birth control. This restriction might go against a clinician's own beliefs and the explicit wishes of a patient. In this case, Dr. N is being asked by a patient, Ms. K, to code oral contraception as treatment for acne, a noncontraceptive benefit of birth control pills, although both parties know Ms. K's primary desire is to prevent pregnancy. We examine the legal and moral arguments surrounding contraceptive provision in this case and offer guidance for how Dr. N and Ms. K m

Client perceptions of the ideal addictions counselor.

Addicted persons in a residential treatment center rated the traits which they felt were the most positive and negative in a counselor. Lists of traits were developed by having one group of clients make a list, in their own words, of positive and negative traits. These traits were compiled into lists from which other groups of clients rated the top 10 positive and the top 10 negative counselor traits. Profiles were developed for eight subgroups (Males, Females, Black Clients, White Clients, Alcoholics, Cocaine Addicts, Younger Clients: 18-23 years old, and Older Clients: 43+ years). Significan

Promoting resiliency among palliative care clinicians: stressors, coping strategies, and training needs.

BACKGROUND: Palliative care clinicians (PCCs) are susceptible to burnout, as they regularly witness immense patient and family suffering; however, little is known about their specific challenges and training needs to enhance their long-term sustainability. OBJECTIVE: The purpose of this qualitative study was to explore common stressors, coping strategies, and training needs among PCCs in efforts to inform the development of a targeted Resiliency Program. METHODS: Utilizing a semistructured interview guide, we conducted a series of in-depth interviews with 15 PCCs at the Massachusetts General

The treatment of craniosynostosis: an ethical perspective.

The parents of children with craniofacial deformity have expectations that cannot always be reasonably met in a world of clinical uncertainty. In order to bridge the "reality gap," the members of the cranofacial team must be open and honest in discussing the known harms and benefits of a proposed treatment, so that a relationship of trust evolves between the health care professional and the patient/parent. It is only through this trust that a truthful implementation of consent, within its moral framework, can be achieved. This, in turn, requires an analysis of the outcomes of the various optio

Open Visitation: Enabling Family Presence, Centered Care, and Engagement in Intensive Care Unit.

Family-centered care (FCC) emphasizes collaboration, dignity, respect, and shared decision-making between families and health care teams. In the ICU, FCC relies on family presence at the bedside, facilitated by open visitation policies. However, the COVID-19 pandemic disrupted this model, as restrictive visitation policies eliminated family presence, leading to adverse outcomes such as loneliness and delirium in patients, distress and grief among families, and moral injury and burnout in staff. As health systems recover, there is a need to reestablish FCC by prioritizing open visitation while
