New investigations of a pulsatile impeller blood pump.

For circulatory assist devices and total artificial heart systems, impeller blood pumps with small total volumes would be fully implantable. One of the main obstacles, however, is generation of a pulsatile flow. The simplest way to overcome this problem is by changing the pump's revolutions per minute (rpm) periodically, but this often results in severe hemolysis. After theoretic analysis, two in vitro models of impeller blood pumps have been devised, producing pulsatile flow with constant rpm. In the first model, the impeller oscillates in an axial direction during constant rotation. The pump…

Pulsatile perfusion during cardiopulmonary bypass procedures in neonates, infants, and small children.

Multiple factors influence the outcome of cardiopulmonary bypass (CPB) procedures in pediatric patients with congenital heart defects. The benefit of pulsatile over nonpulsatile perfusion is one such factor that continues to be widely debated among researchers, perfusionists, and surgeons. However, by accurately measuring pulsatile flow in terms of energy equivalent pressure and surplus hemodynamic energy, pulsatile perfusion is clearly seen to replicate the physiologic heart in a manner unparalleled by nonpulsatile perfusion. Studied benefits of pulsatile perfusion in pediatric patients inclu…

Doppler sonography of adnexal masses: the predictive value of the pulsatility index in benign and malignant disease.

OBJECTIVE: The purpose of this study was to determine whether pulsed Doppler sonography can be used to distinguish between benign and malignant adnexal masses on the basis of pulsatility index. SUBJECTS AND METHODS: In an 18-month period, all patients in whom an adnexal mass was detected at sonography had further evaluation of the mass by color and pulsed Doppler sonography. Ninety-nine patients with 102 masses that were surgically removed were included in the study. The pulsatility indexes were calculated from the reproducible spectral waveforms generated from flow centrally or peripherally

Embolus Analog Trajectory Paths Under Physiological Flowrates Through Patient-Specific Aortic Arch Models.

Atrial fibrillation (AF) is the most common irregular heartbeat among the world's population and is a major contributor to cardiogenic embolisms and acute ischemic stroke (AIS). However, the role AF flow plays in the trajectory paths of cardiogenic emboli has not been experimentally investigated. A physiological simulation system was designed to analyze the trajectory patterns of bovine embolus analogs (EAs) (n = 720) through four patient-specific models, under three flow conditions: steady flow, normal pulsatile flow, and AF pulsatile flow. It was seen that EA trajectory paths were proportion…

Axisymmetric compact finite-difference lattice Boltzmann method for blood flow simulations.

An axisymmetric compact finite-difference lattice Boltzmann method is proposed to simulate both Newtonian and non-Newtonian flow of blood th … fied for (1) steady inflow of a Newtonian fluid through a stenosed lumen, (2) temporally developing pulsatile flow (Womersley flow) through a straight lumen with Newtonian fluid, and (3) steady inflow of a non-Newtonian fluid through a straight lumen. The solver is then applied to simulate the steady flow of a non-Newtonian fluid through a stenosed lumen, and it …

Tonsillar pulsatility before and after surgical decompression for children with Chiari malformation type 1: an application for true fast imaging with steady state precession.

INTRODUCTION: We hypothesize that surgical decompression for Chiari malformation type 1 (CM-1) is associated with statistically significant decrease in tonsillar pulsatility and that the degree of pulsatility can be reliably assessed regardless of the experience level of the reader. METHODS: An Institutional Review Board (IRB)-approved Health Insurance Portability and Accountability Act (HIPAA)-compliant retrospective study was performed on 22 children with CM-1 (8 males; mean age 11.4 years) who had cardiac-gated true-FISP sequence and phase-contrast cerebrospinal fluid (CSF) flow imaging as

Pulse-wave encephalopathy: a comparative study of the hydrodynamics of leukoaraiosis and normal-pressure hydrocephalus.

There is a strong association between the occurrence of leukoaraiosis and normal-pressure hydrocephalus (NPH). Venous compression secondary to alterations in craniospinal compliance is implicated in the pathogenesis of NPH, and venous pathology has also been implicated in leukoaraiosis. The purpose of this paper is to compare and contrast the blood-flow and fluid-pulsatility characteristics of these conditions. I initially studied 18 subjects without pathology, with MRI flow-quantification studies of the cerebral arteries and veins, to define the range of normality. The main study involved 10

Partial carotid occlusion as treatment for intracranial internal carotid aneurysms.

A study was undertaken to investigate the possibility of using partial carotid occlusion instead of complete carotid ligation for the treatment of intracranial internal carotid aneurysms with a view to avoiding such complications of the latter procedure as neurological deficit resulting from cerebral ischemia, and ascending thrombus formation. The beneficial effect of carotid ligation has been explained by the interruption of pulsatile flow which can cause rupture of an aneurysm by resonance phenomena. Studies on blood flow in the aorta in dogs, as well as in a human carotid artery in vivo and…

Phenotyping of Stable Left Ventricular Assist Device Patients Using Noninvasive Pump Flow Responses to Acute Loading Transients.

BACKGROUND: Although it has been established that continuous flow left ventricular assist devices are sensitive to loading conditions, the effect of acute load and postural changes on pump flow have not been explored systematically. METHODS AND RESULTS: Fifteen stable outpatients were studied. Patients sequentially transitioned from the seated position to supine, passive leg raise, and standing with transition effects documented. A modified Valsalva maneuver, consisting of a forced expiration with an open glottis, was performed in each position. A sustained, 2-handed handgrip was performed in

Posture-dependent cerebrospinal fluid flow measurement in the optic nerve subarachnoid space using multi-delay 2D interslice saturation MRI.

Recent studies have indicated that CSF dynamics within the optic nerve subarachnoid space (ONSAS) may play an important role in the progression of optic diseases. Yet, as common optical tests are limited to the ocular structures and cannot probe beyond the optic nerve head, direct evaluation of CSF dynamics in ONSAS remains challenging due to limited imaging methods. In this study, we propose a non-invasive MRI method, inversion-recovery alternate ascending/descending directional navigation (IR-ALADDIN), that utilizes interslice saturation and variable delay times (TD) to enable quantification

The association between the pulse pressure gradient at the cranio-cervical junction derived from phase-contrast magnetic resonance imaging and invasively measured pulsatile intracranial pressure in symptomatic patients with Chiari malformation type 1.

BACKGROUND: In symptomatic Chiari malformation type 1 (CMI), impaired intracranial compliance (ICC) is associated with an increased cranio-spinal pulsatile pressure gradient. Phase-contrast magnetic resonance imaging (MRI) represents a non-invasive modality for the assessment of the pulse pressure gradient at the cranio-cervical junction (CCJ). We wished to explore how the MRI-derived pulse pressure gradient (MRI-dP) compares with invasively measured pulsatile intracranial pressure (ICP) in CMI, and with healthy controls. METHODS: From phase-contrast MRI of CMI patients and healthy controls,

Pulsatility in ventricular assistance devices: A translational review focused on applied haemodynamics.

Heart failure affects more than 30 million people worldwide and its prevalence is constantly rising. In 2020, heart transplantation is the only curative treatment, but left ventricular assistance devices (LVADs) are fully integrated into the decision algorithm for management of patients with advanced heart failure, with more than 20,000 devices implanted worldwide in the last decade. Intended to support cardiac output, LVADs remove the blood from the left ventricle and eject it into the proximal aorta. Whereas first-generation LVADs were pulsatile, second- and third-generation LVADs are more r

Numerical analysis of blood flow through an elliptic stenosis using large eddy simulation.

The presence of a stenosis caused by the abnormal narrowing of the lumen in the artery tree can cause significant variations in flow parameters of blood. The original flow, which is believed to be laminar in most situations, may turn out to turbulent by the geometric perturbation created by the stenosis. Flow may evolve to fully turbulent or it may relaminarise back according to the intensity of the perturbation. This article reports the numerical simulation of flow through an eccentrically located asymmetric stenosis having elliptical cross section using computational fluid dynamics. Large ed

Rotary pumps and diminished pulsatility: do we need a pulse?

Ventricular assist devices (VADs) have been successfully used as a bridge to heart transplant and destination therapy (DT) for congestive heart failure (HF) patients. Recently, continuous flow VAD (CVAD) has emerged as an attractive clinical option for long-term mechanical support of HF patients, with bridge-to-transplant outcomes comparable with pulsatile flow VAD (PVAD). Continuous flow VADs are smaller, more reliable, and less complex than the first-generation PVAD. Despite the widespread clinical use, CVAD support has been associated with gastrointestinal bleeding, hemorrhagic strokes, and…

Research on the Characteristics of Heat Transfer and Mass Transfer of Imitation Heart Pulse Membrane Distillation.

Membrane distillation, a separation technology, has drawn wide attention in industries such as seawater desalination and high-salinity wastewater treatment. However, conventional steady-flow operation suffers from flux decline and membrane fouling, while standard square-wave and sine-wave pulsations provide limited enhancement and stability. To address these issues, this study introduces heartbeat-mimicking pulsatile flow. Its effects on membrane distillation performance were evaluated through comparative (steady vs. pulsatile) experiments, a three-level orthogonal experiment, and tests with v…

Carbon dioxide angiography: effect of injection parameters on bolus configuration.

PURPOSE: Predict the intravascular distribution of carbon dioxide during angiography. MATERIALS AND METHODS: Mathematical modeling was used to predict the flow pattern of CO2 in a pulsatile system as a function of the CO2 flow rate. Findings were validated in an in vitro pulsatile circuit. RESULTS: The annular flow pattern with filling of nearly the entire lumen with CO2 is the most desirable, followed by intermittent bubble flow (provided individual bubbles are large). Stratified flow relates to a continuous floating CO2 bubble. Configuration of the CO2 bolus depends on fluid properties, fl

Effect of Continuous-Flow Mechanical Circulatory Support on Microvasculature Remodeling in the Failing Heart.

Left ventricle (LV) unloading caused by a left ventricular assist device (LVAD) has been shown to enhance reverse LV remodeling in end-stage cardiomyopathy. Several reports consistently suggest that a pulsatile-flow LVAD has more profound effects compared to continuous-flow LVAD, though the responsible mechanisms are not fully understood. We hypothesized that arterial pulsatility, being affected by the type of LVAD, may affect microvasculature and functional/pathological LV remodeling in end-stage cardiomyopathy. The study included 18 patients with chronic heart failure who underwent LVAD impl

High frame rate in vivo two-photon microscopy to quantify murine cerebrospinal fluid flow heterogeneity.

Cerebrospinal fluid (CSF) flows through perivascular spaces (PVSs) surrounding brain vasculature, and impaired flow has been linked to neurodegenerative diseases such as Alzheimer's. However, the mechanisms driving CSF flow and its oscillatory dynamics remain poorly understood. Using high frame rate two-photon imaging (up to 113 Hz), we show that CSF flow is spatially heterogeneous and highly pulsatile, quantified using a regional CSF pulsatility index (PI). Reduced-order simulations with realistic domain length and Windkessel boundary conditions show that arterial pulsations generate peak CSF
