Recurrent Infantile Hypertrophic Pyloric Stenosis in the Emergency Department: A Case Report.

INTRODUCTION: Infantile hypertrophic pyloric stenosis (IHPS) is a common cause of infant vomiting. Emergency department (ED) diagnosis is usually made by pyloric ultrasound and treated by pyloromyotomy. CASE REPORT: An eight-week-old boy with a history of IHPS about six weeks status post pyloromyotomy presented to the ED with vomiting and failure to thrive, and a critically narrowed pylorus was identified by ultrasound. An upper gastrointestinal series confirmed recurrent pyloric stenosis, necessitating another pyloromyotomy. CONCLUSION: Prolonged vomiting after pyloromyotomy should be conce…

Intraoperative Endoluminal Pyloromyotomy Versus Stretching of the Pylorus for the Reduction of Delayed Gastric Emptying After Pylorus-Preserving Partial Pancreatoduodenectomy: A Blinded Randomized Controlled Trial (PORRIDGE Study; DRKS00013503).

BACKGROUND: Pylorus-preserving partial pancreatoduodenectomy (ppPD) is a treatment for tumors of the pancreatic head. Delayed gastric emptying (DGE) is one of the most common complications following ppPD. In a retrospective analysis, intraoperative endoluminal pyloromyotomy (PM) was shown to be associated with a reduction in DGE rates. OBJECTIVE: The aim of this randomized controlled trial was to investigate the effect of intraoperative endoluminal PM on DGE after ppPD. METHODS: Patients undergoing ppPD were randomized intraoperatively to receive either PM or atraumatic stretching of the pyl…

Recurrent pyloric stenosis.

Infantile hypertrophic pyloric stenosis is the most commonly encountered surgical disease among pediatric patients. Incomplete pyloromyotomy is not uncommon complication of pyloromyotomy. However, recurrent pyloric stenosis is extremely rare. Up until now, there are only five cases reported in the English literature. We report a child with recurrent pyloric stenosis who was managed by redo pyloromyotomy.

Comparison of the effect of laparoscopic and conventional pyloric surgery on gastric emptying in dogs.

The effect of a laparoscopic approach and pyloric surgery on canine gastrointestinal activity, particularly gastric emptying time, is not well understood. The purpose of this study was to compare the effect of laparoscopic and conventional pyloric surgery, in Ramstedt pyloromyotomy and Heineke-Mikulicz pyloroplasty, on complete gastric emptying time in 20 clinically normal dogs. Dogs were divided into four groups of five animals: dogs with laparoscopic Ramstedt pyloromyotomy, conventional Ramstedt pyloromyotomy, or laparoscopic Heineke-Mikulicz pyloroplasty, and the conventional Heineke-Mikuli…

Determinants of good outcome in pyloric stenosis.

Good outcome following pyloromyotomy for pyloric stenosis is dependent on the training of the surgeon, availability of a specialist paediatric anaesthetist, and the quality of preoperative correction of fluid and electrolyte abnormalities. Complications (including death, inadvertent duodenal perforation, incomplete pyloromyotomy, wound dehiscence and infection), higher hospital costs and increased length of hospital stay are all more likely to occur when the pyloromyotomy is performed by a surgeon who has had no speciailist paediatric surgical training.

Perioperative Complications of Surgery for Hypertrophic Pyloric Stenosis.

Pyloromyotomy is the tried and tested surgical procedure for successful operative treatment of pyloric stenosis. Over time, the operative approach has evolved to take advantage of cosmetically superior incisions and more recently minimally invasive surgery. During and following surgery, complications are uncommon. The specific complications of an inadequate pyloromyotomy requiring repeated procedure and mucosal perforation during an overzealous pyloromyotomy represent the ends of a spectrum within which sits the perfect procedure. Here, we discuss these specific complications together with the…

Recurrent pyloric stenosis: to dilate or operate? A preliminary report.

PURPOSE: Idiopathic hypertrophic pyloric stenosis is a common surgical problem in infants, and pyloromyotomy is almost always successful in alleviating the obstruction. There are few reports in the literature that discuss recurrent pyloric stenosis as opposed to incomplete pyloromyotomy. We report 2 such babies with different cures. METHODS: The health records department files were electronically searched for the number of infants at our children's hospital with hypertrophic pyloric stenosis seen over the past 30 years (1973-2003), and the recurrences were reviewed. RESULTS: Recurrent pylori…

[Recurrent hypertrophic pyloric stenosis - diagnostics and treatment].

Hypertrophic pyloric stenosis (HPS) is the most common surgically treated disease causing vomiting in infancy, and pyloromyotomy presents around 30% of all operations in this period. Recurrence of the disease is the most serious complication in its treatment. For 16 years period (2000 - 2015) 154 children with HPS were operated n Pediatric Surgery, Plovdiv. Four of them (2.6%), two boys and two girls, were reoperated due to recurrent HPS. Three of the children were sent by other hospital, one of which having Pediatric Surgery, and the other two from regional hospitals. In one case the pyloromy…

Management of hypertrophic pylorus stenosis with ultrasound guided single shot epidural anaesthesia--a retrospective analysis of 20 cases.

AIM: To retrospectively describe the performance of ultrasound guided thoracic epidural anaesthesia under sedation for anaesthesia management of open pyloromyotomy. BACKGROUND: Anaesthesia management for hypertrophic pylorus stenosis (HPS) is usually performed under general anaesthesia with tracheal intubation. Only a few publications describe avoidance of tracheal intubation in infants by using spinal or caudal anaesthesia. The present retrospective analysis describes the performance of ultrasound guided thoracic epidural anaesthesia under sedation for anaesthetic management of open pyloromy…

Effect of pyloric drainage on the healing of esophagogastric anastomoses in rats.

BACKGROUND AND OBJECTIVES: Esophagogastric anastomotic leaks complicate 5% to 20% of esophagectomies for esophageal cancer and are responsible for approximately one-third of perioperative deaths. Poor gastric emptying is a predisposing factor for anastomotic leakage. An animal experiment was used to test the hypothesis that a pyloric drainage procedure (pyloromyotomy) would have a positive effect on esophagogastric anastomotic healing. METHODS: In 40 rats single-layer esophagogastric anastomoses were constructed with interrupted 7-0 polypropylene sutures. A pyloromyotomy was done in the exper…

Population-level surgical outcomes for infantile hypertrophic pyloric stenosis.

OBJECTIVES: Determine national outcomes for pyloromyotomy; how these are affected by: (i) surgical approach (open/laparoscopic), or (ii) centre type/volume and establish potential benchmarks of quality. METHODS: Hospital Episode Statistics data were analysed for admissions 2002-2011. Data presented as median (IQR). RESULTS: 9686 infants underwent pyloromyotomy (83% male). Surgery was performed in 22 specialist (SpCen) and 39 nonspecialist centres (NonSpCen). The proportion treated in SpCen increased linearly by 0.4%/year (r=0.76, p=0.01). Annual case volume in SpCen vs. NonSpCen was 40 (24-5…

Laparoscopy in pediatric surgery: Implementation in Canada and supporting evidence.

BACKGROUND/PURPOSE: The purpose of this study was to assess the diffusion of laparoscopy usage in Canadian pediatric centers and the relationship between uptake of laparoscopic surgery and the level of evidence supporting its use. METHODS: National data on four pediatric laparoscopic operations (appendectomy, pyloromyotomy, cholecystectomy, splenectomy) were analyzed using the Canadian Institute for Health Information Discharge Database (2002-2013). The highest level of evidence to support the use of each procedure was identified from Cochrane, Embase, and Pubmed databases. Chi-square test fo…

Recurrent hypertrophic pyloric stenosis, diagnosis and early surgery: a comprehensive review.

BACKGROUND: Recurrent pyloric stenosis (RPS) is a very rare condition that occurs after an initially successful pyloromyotomy for hypertrophic pyloric stenosis. Over the last decade, the number of reported cases of recurrent pyloric stenosis has increased considerably. Given the rarity of this condition and the paucity of literature on the subject, there is a certain diagnostic difficulty, particularly at ultrasound level, where it is difficult to differentiate a true recurrence of pyloric stenosis from an initial incomplete pyloromyotomy. METHODS: A systematic literature review was performed…

[The significance of minimally invasive surgery in the treatment of inguinal hernia and hypertrophic pyloric stenosis].

BACKGROUND: Inguinal hernia repair and pyloromyotomy are among the most common operations performed on children. In the last two decades minimally invasive surgery has been employed for an increasing number of these procedures. This review describes the development of the techniques involved, and their current role in therapy. MATERIAL AND METHODS: A systematic review of the paediatric surgical literature since 1990 was performed on laparoscopic inguinal hernia repair and pyloromyotomy. Relevant publications were summarised. RESULTS: The first laparoscopic pyloromyotomy was described in 1991…

[Surgical treatment of hypertrophic stenosis of the pylorus at a general surgery service].

The authors purpose was to demonstrate that pyloromyotomy can be successfully carried out by nonspecialist surgeons. Eighty three cases admitted over a twelve-year period to the Department of Pediatrics of Montecelo Hospital, a district general hospital, were reviewed. Extramucosal pyloromyotomy was performed by general surgeons under general anaesthesia in all cases. There were no deaths. Three infants had postoperative complications and required treatment. All cases underwent prophylactic antibiotics and no wound infections were recorded. When compared with other published series, our study …

Pyloric stenosis a 100 years after Ramstedt.

Conrad Ramstedt performed the first pyloromyotomy for what is now called idiopathic hypertrophic pyloric stenosis 100 years ago. The intervening century has seen the management of this condition transformed but the underlying cause remains a mystery. This article reviews the treatment of this condition before and after the introduction of pyloromyotomy and the advances made subsequently towards understanding its cause.

A natural spreader for the pylorus.

General and pediatric surgeons commonly treat hypertrophic pyloric stenosis. A skillful pyloromyotomy without complications culminates in a gratifying experience. An instrument designed for this purpose should be safe, versatile and easy to use. A modified curved, 6 inch clamp facilitates doing a pyloromyotomy by implying the same action as other more commonly used instruments. The natural spreader for the pylorus is easy to make and it is also relatively inexpensive.

[Adhesive small bowel obstruction after surgery during infancy; a potentially fatal long term complication].

BACKGROUND: Pyloromyotomy, the treatment for infants with hypertrophic pyloric stenosis, is a procedure with a low risk of complications and quick recovery. We describe a rare and fatal complication. CASE DESCRIPTION: A 12-year old boy presents with persistent abdominal pain and vomiting at his general practitioner. After he collapses, cardiopulmonary resuscitation is started and he is brought to the hospital where he died. His medical history mentioned pyloromyotomy, complicated by fascia dehiscence and recurrent abdominal pain since the age of six. No cause was ever found for his abdominal …

Ultrasound assessment of the gastric contents for the guidance of the anaesthetic strategy in infants with hypertrophic pyloric stenosis: a prospective cohort study.

BACKGROUND: Evacuation of gastric content through a nasogastric tube, followed by rapid sequence induction, is usually recommended in infants undergoing pyloromyotomy. However, rapid sequence induction may be challenging, and is therefore controversial. Some anaesthetists regularly perform classical non-rapid induction technique, after blind aspiration of the gastric contents, although this aspiration may have been incomplete. This prospective observational study aimed to assess whether the ultrasound monitoring of the aspiration of the stomach contents, may be useful to appropriately guide th…
